Position Summary:

The primary purpose of the Centralized Intake Coordinator position is to manage all aspects of the patient referrals process, which includes establishing and maintaining positive relationships with customers and referral sources in a timely manner. Assists with ensuring proper medical equipment and supplies are ordered and communities are prepared for clinical needs of patients transitioning from acute care as well as entering referrals, leads, insurance verification and other required information into CRM.


Education/Qualifications:

  • Must have a working clinical knowledge of medical terminology and have exceptional knowledge of SNF clinical capabilities.

  • Must have the ability to read, interpret and analyze referral information to provide appropriate response and have sufficient knowledge of financial payers and pre-cert requirements.

  • Must possess basic computer skills and ability to navigate through various software programs, including referral software and electronic health records software.

  • Must have working knowledge of SNF referral process or SNF experience.


Shifts and Wages:

  • Full-Time Opportunity Available

  • Shift Needed: Days

  • Wage: $60,000 - $65,000 Annually, Based on Skillset and Experience
