Job Type: Hourly – Pay Based on Experience

Job Description:
We are seeking a compassionate and reliable Licensed Vocational Nurse (LVN) to join our growing home health team serving South County in San Luis Obispo County. The LVN will provide skilled nursing care to patients in their homes while supporting their health, safety, and independence.

Responsibilities:

• Provide skilled nursing care in patient homes
• Monitor and document patient conditions and vital signs
• Administer medications and treatments as ordered
• Assist with care coordination and communicate with the healthcare team
• Educate patients and families on care plans and health management
• Maintain accurate and timely documentation

Requirements:

• Current California Licensed Vocational Nurse (LVN) license
• Minimum 1 year of home health experience preferred
• Current CPR certification
• Valid California driver’s license and reliable transportation
• Strong communication and organizational skills
• Ability to work independently in patient homes

Compensation:

• Hourly pay – rate depends on experience
