Medical Assistant

We’re looking for a Certified Medical Assistant (CMA) to join our dynamic Chronic Care Management (CCM) team in Eagle, Idaho. If you’re detail-oriented, compassionate, and self-motivated, this is your chance to make a meaningful difference in the lives of patients with chronic conditions.

What You’ll Do:

• Serve as a key connection between patients and providers
• Monitor patient health status and follow up on provider-directed care plans
• Communicate with patients and families via phone, email, and in-person visits
• Help close gaps in care by coordinating services across various healthcare settings
• Support patients in managing long-term health conditions with empathy and precision

Preferred Qualifications:

• Certified Medical Assistant (CMA)
• BLS Certification
• Strong communication and organizational skills
• Experience in chronic care or primary care settings is a plus
