Job Summary
The MRD Technician is responsible for maintaining accurate, complete, and confidential patient medical records in compliance with hospital policies, NABH standards, and statutory requirements. The role ensures proper documentation, retrieval, storage, and availability of medical records for clinical, legal, audit, and research purposes.
Key ResponsibilitiesMedical Record Management
- Collect, assemble, and analyze inpatient and outpatient medical records.
- Ensure completeness of records (discharge summaries, consent forms, investigation reports, nursing notes, doctor notes).
- Perform deficiency checking and follow up with clinicians for pending documentation.
- Assign unique MRD numbers and maintain patient indexes.
Coding & Indexing
- Perform disease and procedure coding (ICD-10 / ICD-11 as applicable).
- Maintain disease index, operation index, and doctor index.
- Support clinical audits and data analysis.
Record Storage & Retrieval
- File medical records accurately (manual and/or EMR).
- Retrieve records promptly for patient care, medico-legal cases, insurance, audits, and research.
- Ensure safe custody and traceability of files.
Confidentiality & Compliance
- Maintain strict confidentiality of patient information as per hospital policy and legal norms.
- Ensure compliance with NABH, medico-legal, and statutory requirements.
- Handle medico-legal case (MLC) records as per protocol.
Audit & Quality Support
- Assist in internal and external audits (NABH, insurance, legal).
- Maintain retention and disposal records as per policy.
- Support quality indicators related to documentation and records.
Coordination & Communication
- Coordinate with doctors, nurses, billing, insurance, and quality teams.
- Provide timely medical records for TPA/insurance and legal purposes.
- Respond to information requests from authorized personnel only.
Documentation & Reporting
- Maintain daily MRD registers and trackers.
- Prepare statistical reports (admissions, discharges, surgeries, diagnoses).
- Update MRD SOPs and policies when required.
Qualifications & SkillsEducation
- Diploma / Bachelor’s degree in Medical Records Science / Health Information Management
- (BSc MRD / DMRD / HIM preferred)
Job Types: Full-time, Permanent
Pay: ₹22,000.00 - ₹26,000.00 per month
