Clinical Pharmacist

Job Summary: The Clinical Pharmacist is responsible for ensuring the safe, effective, and rational use of medicines through active participation in patient care, clinical rounds, medication review, and collaboration with the healthcare team, in compliance with NABH standards and hospital policies.

Key Duties & Responsibilities

1. Clinical Pharmacy Services

· Participate in clinical ward rounds along with doctors and nursing staff.

· Review medication orders for appropriateness, dosage, route, frequency, interactions, contraindications, and duplications.

· Monitor therapeutic drug outcomes and suggest modifications based on patient response.

· Provide drug information to clinicians, nurses, and patients.

· Assist in antimicrobial stewardship programs and rational antibiotic use.

· Support medication reconciliation at admission, transfer, and discharge.

2. Patient Safety & Medication Management

· Identify, document, and report Medication Errors (ME), Adverse Drug Reactions (ADR), and Adverse Drug Events (ADE) as per hospital policy.

· Participate in Pharmacovigilance activities and ADR monitoring committees.

· Ensure high-risk medications are identified, labeled, and monitored appropriately.

· Promote safe medication practices in compliance with NABH measurable elements.

3. Patient Counseling & Education

· Provide patient counseling on prescribed medications including dosage, administration, precautions, side effects, and storage.

· Educate patients and attendants on discharge medications to improve compliance and outcomes.

· Participate in patient awareness programs on rational drug use.

4. Documentation & NABH Compliance

· Maintain accurate and complete clinical pharmacy documentation, including:

o Drug therapy review records

o ADR and medication error reports

o Clinical intervention logs

· Ensure compliance with NABH 6th Edition standards related to medication management (MMS).

· Assist in internal audits, external audits, and NABH assessments.

· Support policy formulation, revision, and implementation related to pharmacy and medication safety.

5. Training & Quality Improvement

· Conduct training sessions for nursing and clinical staff on medication safety, LASA drugs, and new medications.

· Participate in quality improvement initiatives, indicators, and data analysis.

· Assist in developing SOPs, guidelines, and protocols related to medication use.

6. Coordination & Communication

· Coordinate with doctors, nurses, pharmacy staff, and quality department for effective patient care.

· Actively participate in Hospital Committees such as:

o Pharmacy & Therapeutics Committee

o Infection Control Committee

o Patient Safety Committee

7. Skills & Competencies

· Strong knowledge of clinical pharmacology and therapeutics

· Understanding of NABH standards and medication management system

· Excellent communication and interpersonal skills

· Analytical and problem-solving abilities

· Attention to detail and documentation accuracy

· Ability to work in a multidisciplinary healthcare team

8. Key Performance Indicators (KPIs)

· Number and quality of clinical interventions

· Reduction in medication errors and ADRs

· Compliance with NABH documentation and audits

· Participation in training and quality initiatives

· Patient counseling effectiveness

9. Working Conditions

· Hospital clinical environment

· Shift duties as per hospital policy

· Interaction with patients, clinicians, and healthcare staff

Qualification : Pharm D / M Pharm (Pharmacy Practice / Clinical Pharmacy) (B Pharm with relevant clinical experience may be considered).

Experience Required: 3 Years Experience in a multi-specialty or NABH-accredited hospital
