JOB SUMMARY
 HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible
 and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic
 barriers to healthcare and promote health equity for all. We offer a wide range of services, including
 mental health care, residential and outpatient substance use treatment, and primary health services.
 
Additionally, we provide transitional support for individuals re-entering the community after involvement
 in the criminal justice system. By integrating physical and behavioral health, we empower individuals to
 overcome challenges by addressing social determinants of health, fostering resilience, and facilitating
 recovery.
 
Prototypes CHS-SUD Substance Treatment and Reentry Transition (START) provides direct in-custody SUD
 counseling, group therapy, and case management services to up to 150 women located at the Century
 Regional Detention Center (Lynwood County Jail). START services are evidence-based and culturally
 appropriate that utilize cognitive behavioral therapies and motivational interviewing approaches to treat
 addiction.
 
The Care Coordinator II is an integral part of an interdisciplinary team that supports our clients in their
 treatment and recovery by coordinating services, providing clinical assistance, and ensuring accurate
 documentation. The Care Coordinator II will develop individualized treatment plans and will help our
 clients meet treatment goals, manage crisis, assist with public benefits, and facilitate access to educational
 and community resources. Additionally, this role will advocate for our clients and ensure that they have
 the resources needed for successful reentry and long-term recovery.
 
KEY RESPONSIBILITIES
 Client Services & Coordination
 <ul>
<li>Provide learning experience opportunities and offer clinical support to assist clients in meeting</li>
</ul>
their treatment goals.
 <ul>
<li>Perform crisis intervention and communicate with treatment team as unforeseen situations arise.</li>
<li>Document client updates and incidents in the facility log daily.</li>
<li>Provide individualized intervention, assist in accessing public benefits/Medi-Cal Outreach and</li>
</ul>
Enrollment; offer information/referral regarding access to health; and encourage participation in
 educational opportunities, such as self-help support groups.
 <ul>
<li>Provide follow-up supportive services to enrolled clients in accordance with program policies and</li>
</ul>
procedures.
 <ul>
<li>Provide direct services to incarcerated clients to plan their successful return to the community</li>
</ul>
and assist them with accessing supportive resources following release.
 <ul>
<li>Perform transitions in level of care, coordinate referrals (including connections with and</li>
</ul>
transportation to physical and mental health services), monitor progress in services, and advocate
 for patients.
 <ul>
<li>Work with SUD Counselors/Case Managers to ensure that each client’s transition plan is</li>
</ul>
consistent with their individualized treatment and rehabilitation plan.
 <ul>
<li>Collaborate with Counselors and/or Case Managers and other available internal and external</li>
</ul>
resources to develop/maintain treatment plans; transition plans; progress notes, and appropriate
 updates in support of the health and recovery needs of the client.
 Documentation
 <ul>
<li>Coordinate, prepare, and maintain required charting and documentation in a timely and thorough</li>
</ul>
manner.
 <ul>
<li>Maintain client records according to HIPAA and 42 CFR, adhere to all client confidentiality</li>
</ul>
requirements and standards.
 <ul>
<li>Maintain accurate records by entering data into various electronic systems for all caseload clients</li>
</ul>
in accordance with guidelines established by Prototypes/HealthRIGHT 360 to satisfy internal and
 external evaluating requirements.
 Community Outreach
 <ul>
<li>Create and develop relationships with community providers of housing, employment, education,</li>
</ul>
food assistance, childcare support, substance abuse treatment, primary care, mental health
 treatment, and other supportive services that will benefit the client upon reentry to the
 community.
 <ul>
<li>Maintain copies of the provider’s literature and program materials for clients to review.</li>
</ul>
Other Responsibilities
 <ul>
<li>Attend meetings and actively participate.</li>
<li>Complete assigned training on time.</li>
<li>Read and respond to communications in a timely manner.</li>
<li>Maintain full compliance with registration requirements.</li>
</ul>
And perform other assigned duties.
 
QUALIFICATIONS
 Education, Certification, or Licensure
 <ul>
<li>High school diploma or equivalent required. Bachelor’s degree in Health and Human Services,</li>
</ul>
Social Work, Psychology, or other related field preferred.
 <ul>
<li>Registration as a Substance Use Disorder Counselor from an accredited California agency (CCAPP,</li>
</ul>
CAADE, CADTP).
 <ul>
<li>Valid First Aid and CPR certification or ability to obtain within 30 days of hire.</li>
<li>Valid California Driver’s License and access to registered and insured transportation.</li>
</ul>
Experience
 <ul>
<li>At least 6 months of experience as a registered substance abuse or behavioral disorder counselor</li>
</ul>
or recovery specialist in community mental health setting preferred.
 <ul>
<li>At least 2 years’ experience providing case management or peer recovery support services.</li>
<li>Experience carrying a caseload or has the ability to carry a caseload of clients.</li>
<li>We will consider for employment qualified applicants with arrest and conviction records.</li>
<li>Must complete a background check and livescan.</li>
