<ul><li>The Occupational Therapist (OT) specializes in improving residents' daily living skills and independence in our skilled nursing facility.
</li></ul>Occupational Therapist, OT Responsibilities:
<ul><li>Assessment and Evaluation: Perform comprehensive assessments of residents' functional abilities, with a focus on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
</li><li>Individualized Care Plans: Develop tailored intervention plans to improve residents' independence in daily activities.
</li><li>Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, and Significant Change assessments.
</li><li>Treatment: Administer OT interventions, including ADL training, adaptive equipment provision, and environmental modifications.
</li><li>Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
</li><li>Documentation: Comfortable and knowledgeable with Point of Service (POS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
</li><li>Interdisciplinary Collaboration: Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, for holistic resident care.
</li></ul>Occupational Therapist, OT Qualifications:
<ul><li>State licensure as a Physical Therapist.
</li><li>EMR and MS office applications.
</li><li>CPR/BLS certification.
</li><li>Knowledge of Section GG data collection and documentation requirements.
</li><li>Strong clinical and interpersonal skills.
