Job Summary:</p>
PERFORMS TESTS AND PROCEDURES IN ONE OR MORE AREAS OF THE CLINICAL LABORATORY WITH INDEPENDENT JUDGEMENT REQUIRED. MEDICAL TECHNOLOGISTS ARE ACCOUNTABLE FOR ACCURATE RESULTS AND THE PROPER USE OF QUALITY CONTROL MATERIALS AND EVALUATION OF THOSE RESULTS. THEY MUST RECOGNIZE THE INTERDEPENDENCY OF TESTS AND CONDITIONS THAT AFFECT TEST RESULTS AND PARTICIPATES IN CONTINUING EDUCATION, COMPETENCY AND PROFICIENCY TESTING.<p></p><h6 class="jobSectionHeader"><b><br>

Medical Lab Technician

Tidelands Community Hospice & Palliative Care

<p>The Palliative Care LPN provides remote support to seriously ill patients and their families by monitoring symptoms, managing pain, and offering emotional support over the phone. This role ensures continuity of care, coordinates with healthcare providers, and helps patients navigate their treatment options, advance care planning, and symptom management.</p><p><b>Key Responsibilities:</b></p><p>Patient Monitoring &amp; Symptom Management:</p><ul><li>Conduct scheduled and as-needed telephone assessments to monitor patients&rsquo; symptoms, pain levels, and overall well-being.</li><li>Provide guidance on symptom management, including medication adherence, side effects, and comfort measures.</li><li>As needed, visits patients in person when a provider is not necessary or practical.</li><li>Identify signs of deterioration and escalate cases to providers when necessary.</li></ul><p>Emotional &amp; Psychological Support:</p><ul><li>Offer compassionate support to patients and families dealing with life-limiting illnesses.</li><li>Guide patients and families through difficult discussions regarding disease progression, treatment decisions, and end-of-life care.</li><li>Provide reassurance and coping strategies for anxiety, depression, and emotional distress.</li></ul><p>Care Coordination &amp; Communication:</p><ul><li>Collaborate with providers, case managers, social workers, and other healthcare professionals to ensure a holistic care approach.</li><li>Educate patients and caregivers on disease processes, medications, and available resources.</li><li>Assist with advance care planning, including discussions on goals of care, hospice referrals, and advance directives.</li></ul><p>Documentation &amp; Compliance:</p><ul><li>Maintain accurate, timely, and confidential documentation of patient interactions in electronic medical records (EMRs).</li><li>Document time in EMR in accordance with Principal Care Management (PCM)</li><li>guidelines.</li><li>Follow organizational policies and healthcare regulations regarding patient communication and documentation.</li><li>Participate in team meetings and case reviews to enhance patient care strategies.</li></ul><p><b>Qualifications &amp; Skills:</b></p><ul><li>Education: Licensed Practical Nurse (LPN) with an active nursing license.</li><li>Experience: Minimum of 1-2 years in palliative, hospice, critical care, or telehealth nursing.
