Case Manager II - Driver 
 
The Case Manager II reports directly to the Program Coordinator of the U.S.VETS program to which the Case Manager is assigned. This individual is responsible for providing guidance to veterans to achieve their highest level of independence. Case management is a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the selection of services to optimally meet each individual clients' needs. 
 
FLSA Classification: Non-Exempt 
 
Responsibilities 
 
For Case Managers of all U.S.VETS Programs: 
<ul>
<li>Assesses veterans referred to the program to include any problems, needs, or barriers identified in the following areas: Housing, Employment, Education/Training, Financial, Sobriety Support/Addiction Treatment, Legal, Family/Social, and Medical/Psychiatric</li>
<li>Coordinates care with VA personnel and other community partners on behalf of clients to for eligibility documentation, mental and physical health services</li>
<li>Facilitates client intake process and compiles client files ensuring all program eligibility documentation is complete</li>
<li>Collaborates with clients to complete Individual Action Plans (IAP)
</li><li>Maintains confidential client files in a professional and timely manner ensuring all forms are the most recently revised versions and in compliance with organization standards; all required documents are present, complete, signed by the client and staff as applicable; and documents are placed in the correct order according to organization standards
</li><li>Meets with clients as specified by program requirements and documents progress notes in Data/Assessment/Plan (DAP) format
</li><li>Reviews and documents updates for IAPs monthly</li>
<li>Completes timely discharge summaries
</li><li>Conducts home visits (whether at project-based units or scattered site) to provide case management services and ensure health and welfare of clients
</li><li>Assists in quality management activities, data collection, and preparing reports</li>
<li>Facilitates client life skills classes
</li><li>Provides specialized case management services and life skills groups/classes to veteran subpopulations as applicable (such as female veterans, disabled veterans, Iraq/Afghanistan veterans, etc.)</li>
<li>Conducts random drug and alcohol screenings of clients</li>
<li>Maintains confidentiality in compliance with the Health Insurance Portability and Accountability Act (HIPAA) standards</li>
<li>Completes accurate and timely data entry into the HMIS system as applicable and other required internal and external databases</li>
<li>Documents client referrals for services</li>
<li>Attends scheduled team meetings and trainings</li>
<li>Maintains and submits time sheets, mileage logs, client expenses, and other required documentation accurately and in a timely fashion</li>

U.S.VETS has various programs to which a Case Manager may be assigned. These additional program-specific duties are outlined as follows: 
 
For Case Managers working with Transitional Housing Clients: 
<ul>
<li>Focuses on providing case management services that result in clients transitioning to permanent housing</li>
<li>Ensures clients are accounted for daily through contact, bed check, and/or daily sign-in logs
</li><li>Monitors program rent/participant fee compliance to assist clients in prioritizing housing</li>
<li>Additionally, for programs with scattered-site transitional housing such as the Transition In Place (TIP) Program, locates housing units in the community that meet clients' needs</li> 
</ul> 
 
For Case Managers working with Permanent Housing Clients: 
<ul>
<li>Focuses on providing case management services that result in clients obtaining and maintaining permanent housing</li>
<li>For programs with scattered-site permanent housing, locates housing units in the community that meet clients' needs</li>
<li>For programs that serve families with dependent children, ensures that children and youth are enrolled in school and connected to the appropriate services within the community, including early childhood education programs</li> 
</ul> 
 
For Case Managers working with Workforce Clients: 
<ul>
<li>Assists with Workforce Program Assessment and Enrollment processes</li>
<li>Works with clients to develop an Individual Employment Development Plan; assesses, alleviates, and reduces any barriers to employment; assists the veteran with the planning of short-term and long-term employment goals to include education and training goals</li>
<li>Provides individualized assistance to clients in developing cover letters, resumes and obtaining other supportive services</li>
<li>Facilitates groups or classes, i.e., Job Placement, Job Retention, Employment Barriers, Computer Skills, Motivation, Money Management, etc.</li>
<li>Makes presentations of clients' employment backgrounds to prospective employers</li>
<li>Learns and maintains knowledge of community employment agencies and procedures, along with any modifications to those procedures</li> 
</ul> 
 
For Case Managers working with SSVF Clients: 
<ul>
<li>May report directly to the SSVF Team Leader if applicable</li>
<li>Conducts rapid rehousing and homeless prevention services for veterans and their families</li>
<li>Works with clients to develop a Housing Stability Plan</li>
<li>Provides case management services to family members in the veteran household</li>
<li>Conducts home visits to provide case management services</li>
<li>Ensures that dependent children are enrolled in school and connected to the appropriate services within the community, including early childhood education programs</li>
<li>Maintains compliance with VA regulations and organization protocols in requesting and distributing Temporary Financial Assistance to third-parties on behalf of veteran households to include cross-referencing HMIS
</li><li>Assists in rapidly placing homeless veteran families into housing</li>
<li>Conduct Rent Reasonableness Testing and ensure Habitability Standards are met for veteran families rental units</li>
<li>Coordinates with VA Grant &amp; Per Diem Transitional Housing Programs for immediate placement of clients and case manages them while they are on the Rapid Rehousing Track of the program</li> 
</ul> 
 
For Case Managers working with Aftercare Case Management programs: 
<ul>
<li>Reports to the Permanent Housing Coordinator for sites with this position. Otherwise reports to the Transitional Housing Coordinator or the Director of Behavioral Health
</li><li>Make home visits by the Case Manager to monitor housing stability</li>
<li>Provide or coordinate educational activities related to meal planning, tenant responsibilities, the use of public transportation, community resources, financial management, development of natural supports</li>
<li>Making referrals to needed services, such as mental health, substance use disorder, medical, and employment services</li>
<li>Participating in case conferencing with other service providers who are working with the veteran</li> 
</ul> 
 
For Case Managers working with Incarcerated Veterans Transition Program (IVTP): 
<ul>
<li>Assists with Program Assessment and Enrollment processes for clients currently in jail/prison or recently released from jail/prison</li>

For Case Managers with nursing credentials: 
<ul>
<li>Oversee and track infection control protocols; assist with ensuring protocols related to health and wellness are implemented and maintained</li>
<li>Assist with coordinating health testing (i.e. TB, COVID, Flu, etc.) to include mass testing, self-administered testing, any required annual testing, or other testing as instructed and responsible for tracking results
</li><li>Coordinate off site testing with the VA and other healthcare partners as needed</li>
<li>Track any required vitals (i.e. temperature checks) at client intake, daily as determined by policy, or as otherwise required per company policies
</li><li>Conduct brief screenings of clients who present with flu-like symptoms and document screening results and referrals made</li>
<li>Triage any reported client symptoms for appropriate care</li>
<li>Track staff and client isolation/quarantine periods including approval of isolation/quarantine start and end dates and required timeline symptom-free periods for ending isolation/quarantine
</li><li>Serve as liaison with any healthcare partners related to coordination of care to offsite isolation facilities</li>
<li>Daily check-ins with clients in hospital, or isolation/quarantine on or offsite</li>
<li>Assist with medication storage and dissemination
</li><li>Document and submit qualifying services for medical billing</li>
<li>Conduct universal precaution and blood borne pathogen training for staff annually or as frequent as necessary</li> 
</ul> 
 
<ul><li>Coordinate health-related events at site (i.e. health fairs, mobile medical, healthcare classes)</li>
<li>Collaborate with any contracted nurse or other contracted healthcare professional and/or medical labs as needed to carry out these responsibilities</li>
<li>Must have and maintain required nursing credentials as a Licensed Practical Nurse (LPN) and Registered Nurse (RN)</li>
<li>While U.S.VETS may provide trainings that support the maintenance of credentials at the company's discretion, the employee is responsible for maintaining credentials</li>

For Case Managers working with Legals Services for Clients: 
<ul>
<li>Provide information and education to veterans regarding available resources</li>
<li>Conduct assessments of veterans' legal needs and barriers that are restricting them achieving stability</li>
<li>Collaborate with other Case Managers and program staff to ensure wrap around support for veterans</li>
<li>Collaborate with current community partners and seek out external community legal professionals and organizations to secure legal services for veterans as needed</li>
<li>Assist the site leadership with direct service administrative tasks related to the program, such as data entry, reporting, and grant compliance, including follow up and corrections as needed</li>
<li>Maintain accurate and up-to-date client records and case notes in compliance with organizational standards, grant requirements, and confidentiality guidelines</li>
<li>Compile and submit service requests for grant support for allowable legal fees and other allowable costs for services</li>
<li>Maintain a database to track client legal cases, document client legal case status and outcomes</li>
<li>Enroll clients as participants under the legal services grant ensuring program targets for clients served are achieved by outreaching to clients and programs</li>

Summary: The Case Manager works as a team member of the Care Management Services Department that works to provide care management, coordination, and supportive services to Clinica Romero patients. This position will be responsible for, but is not limited to, screening and assessing clients; creating, implementing and monitoring individual service plans; crisis intervention; connecting clients to medical, mental health, employment, housing and other community resources. This is a full-time role and will primarily work Monday through Friday, 9am to 5pm.  The Case Manager position is essential in supporting individuals aged 18+ who are transitioning from incarceration back into society. Reporting to the President, Vice President of Programs, and JCM Program Manager, the case manager will provide direct services, outreach, case management, cultural mediation, and advocacy to help participants access critical services and regain control of their health and lives. The case manager will work closely with individuals to navigate complex systems and build a better future, with the ultimate goal of reducing recidivism and promoting recovery. This role is perfect for someone with lived experience, a strong connection to the community, and a passion for supporting others in their reintegration and success. The position requires a high level of independence, selfmotivation, and the ability to effectively engage with participants from diverse backgrounds within their communities.
 
 
 
 Duties And Responsibilities
 
 
 
 <ul>
 <li>
 Caseload: Manage a caseload of 30 participants, providing individualized support and case management, while ensuring the timely completion of all required documentation.
 </li>
 <li>
 Advocacy and System Navigation: Provide advocacy services for participants, including attending court dates, DCFS meetings, doctor appointments, etc.
 </li>
 <li>
 Documentation: Maintain accurate and organized participant files, documenting activities, progress, and other contract-mandated information including using multiple case management data tracking systems.
 </li>
 <li>
 Reporting: Sharing program data through regular meetings, check-ins, and assessments.
 </li>
 <li>
 Community Engagement: Helping participants connect with community services and activities.
 </li>
 <li>
 Care Plans &amp; Assessments: Coordinate supportive services for clients, ensuring comprehensive and timely follow-up. Provide referrals for participants to extended community-based services and maintain strategies for ongoing resource connections. Conduct extensive client assessments for the purpose of setting goals and tracking progress.
 </li>
 <li>
 Team Participation: Actively contributing to team-building activities within the CIS and RICMS programs, as well as participating in required events during both weekdays and weekends. Represent JCM at community events such as health fairs, forums, and other public functions to increase awareness and engagement.
 </li>
 <li>
 Transportation: be willing to transport clients and use personal vehicle for work purposes.

 Qualifications &amp; Experience
 
 
 
 <ul>
 <li>
 Lived Experience: Formerly incarcerated or criminal justice system involved preferred.
 </li>
 <li>
 Education &amp; Experience: High school diploma or equivalent experience in case management, social work, or a similar role.
 </li>
 <li>
 Legal Eligibility: Permanent resident or US citizen with proof of work eligibility. Criminal background check is required for work with children and adolescents.
 </li>
 <li>
 Driver&rsquo;s License and Insurance: Must have valid California Class C Driver's License, minimum automobile insurance, reliable transportation, and be willing to transport clients and use personal vehicle for work purposes.
 </li>
 <li>
 Adaptability and Professionalism: Ability to work independently in a community-based environment, while managing a caseload and meeting program requirements.
 </li>
 <li>
 Proficient in Computer and Data Systems: Experienced with utilizing case management software to track and document client interactions, progress and outcomes. Comfortable with basic computer operations including data entry and general software navigation.

 <li>
 Bilingual in Spanish is preferred.
