<div>The Case Manager serves as the primary transition facilitator by providing the coordination of treatment, treatment planning, and the development and implementation of individualized service plans for youth and young adults with mental health needs. The case manager facilitates services &ndash; like school, medical providers, family visits, etc. &ndash; and also assists in skill building through coaching and one-on-one work with young adults ages 16-21.

ESSENTIAL DUTIES &amp; RESPONSIBILITIES
<ul>
<li>Works with the transition team, including the member, their therapist, residential staff, guardian(s) and others to develop and implement a Thresholds Care Plan.</li>
<li>Serves as the primary contact with the members&rsquo; support network to assist with the members&rsquo; goals for transitioning to adulthood.</li>
<li>Provides direct service to members through assessment, case management, group counseling, job support, crisis intervention, transition coordination, and psycho-social rehabilitation services in the community and at Thresholds locations.</li>
<li>Performs and participates in crisis intervention in accordance to Therapeutic Crisis intervention (TCI) and Thresholds standards. Models best practices for TCI to other staff. Performs and supervises physical management when necessary.</li>
<li>Attends all court appearances, administrative case reviews, DCFS meetings, and Thresholds meetings as assigned. Assures that all Thresholds procedures regarding the handling of member&rsquo;s funds are followed and that all safeguards, reports, and accounting meets the highest standards.</li>
<li>Completes all assigned paperwork in a timely fashion. Produces Medicaid compliant service notes. Completes reports for Thresholds, DCFS, ICG, DMH, courts, and others regularly and when required.</li>
</ul>
MARGINAL DUTIES &amp; RESPONSIBILITIES
<ul>
<li>May be required to work flexible schedules and be available to work on weekends or evenings</li>
<li>May be required to be on call.</li>
</ul>
REQUIREMENTS
<ul>
<li>At least 21 years of age</li>
<li>Bachelor's degree in Social Work or related field required, Master's degree preferred</li>
<li>At least 2 years of previous experience working with youth with mental health needs, in a residential and/or mental health setting</li>
<li>A valid driver&rsquo;s license, current car insurance, and daily access to your own car</li>
<li>Willingness to provide transportation to clients</li>
<li>To be able to perform Therapeutic Crisis Intervention therapeutic restraint techniques, which require moderate exertion, being able to lift 40 pounds, and the mobility to lower self to a kneeling position on the floor. This training is provided and required by Thresholds</li>

Family Case Manager

SGA Youth & Family

<div>Description:

Part Time: 15 hours a week at $22hr
Key Areas of Responsibility:<ul><li>
Assists with recruiting and retaining 30 youth into employability skills training, unsubsidized placement, and long-term employment.</li><li>
Works with Director to develop and implement an employer engagement strategy to develop unsubsidized work experiences for youth between the ages of 18 and 24. General range of youth to be placed is 12-14.</li><li>
Unsubsidized placement includes both part- and full-time employment. Employment Verifications must be collected at 30, 60, 90 and 120 days.</li><li>
Markets, recruits and maintains relationships with employers that will expose youth to long-term employment.</li><li>
Help youth identify their career goals and develop an Individualized Career Plan to address areas for improvement; assists with employment placements for specific career goals.
</li><li>Works with SGA leadership to develop and implement hiring events and job fairs for youth.</li><li>
Works with other youth advocates, justice system representatives, etc. to help support youth remain in compliance of all mandates.</li><li>
Co-facilitates employability skills training for all youth that will lead to gainful employment.</li><li>
Works in collaboration within the targeted communities to cultivate relationships with local community resources.</li><li>
Connect clients with job opportunities, training programs, and educational resources to enhance their employability.</li><li>
Track and document clients&rsquo; progress towards their career goals, adjusting plans as necessary to ensure success.</li><li>
Guide clients through career assessments to identify interests, skills and suitable career paths.</li><li>
Identify barriers to employment and provide referrals to appropriate resources to help clients overcome these challenges.</li><li>
Reaches out and re-engage youth that have inconsistent attendance, work with families to bring resources to the home, and connect with programs.</li><li>
Attends mandatory trainings implemented by SGA or funder.</li><li>
Ensures files remain up-to-date and in compliance.</li><li>
Transporting students in vehicle to and from program related activities may be required.</li><li>
Implements other services requested by SGA.</li><li>
Responds in a timely manner to all written correspondence, email and phone calls.</li><li>
Meet program youth and staff timesheet submission, reporting and activity deadlines</li><li>
Attends regular supervision, team meetings and other professional workshops to enhance job skills.</li><li>
Administer SGA and DFSS survey at the end of the program to assess participant experience.</li><li>
Data entry includes but is not limited to youth enrollment and daily youth attendance and activities which is required weekly. components are required in the data system: Work Plan, DFSS Youth Intake Forms completed in hard copy DFSS Youth Enrollment Portal, activity entry and attendance, contractual certification information, outputs, and outcomes.</li><li>
Maintain participant files, Bus Card logs, Timesheets and all required DFSS documentation.</li><li>
Conducts required reports and data material to SGA such as the Employability and Life Skills Assessment.</li><li>
Other duties as assigned.</li></ul>
Requirements:

Attributes/Skills Required/Sought:<ul><li>
Experience working one on one with at risk and youth involved in the juvenile justice system.</li><li>
Ability to display sensitivity and responsiveness to cultural differences present in the service population.</li><li>
Experience in case management or job counseling is often preferred.</li><li>
Knowledge of the job market and career development resources.</li></ul>
Experience/Education Required/Sought:<ul><li>
Bachelor&rsquo;s Degree in social work, criminal justice or related field, preferred.</li><li>
Experience working with young adults 18-24 years of age, preferred.</li><li>
Evening and weekend availability required.</li></ul>

The Mobile Advisor reports to the Safety, Security &amp; Transportation Manager. Responsible for enforcing all laws, rules, and policies authorized by the Safety and Security Department. Position Functions

<li>Enforces all authorized laws, rules, and regulations.</li>
<li>Performs alarm and patrol duties in assigned areas to protect life and property against theft, misappropriation, vandalism, fire, flood, or similar disaster, suspicious acts and/or persons, and other unusual conditions.</li>
<li>Provide vehicle transportation support function under the direction of a supervisor in compliance with government, corporate, and management directives.</li>

Position Requirements
<ul>
<li>Minimum of High School graduate or equivalent.</li>
<li>6 months of related experience. Prefer 1 year of experience in a security department, law enforcement agency, or experience with security duties.</li>
<li>Valid driver's license in the state of employment with an acceptable driving record.</li>
<li>Prefer experience in law enforcement.</li>
<li>Prefer Commercial Driver&rsquo;s License (CDL).</li>

Additional Requirements: Prefer knowledge of riot and crowd control techniques and law enforcement procedures. Must maintain CPR/FA Certification. Ability to obtain a bus driver's license. In addition, must be able to interact with team members and maintain an effective working relationship with all facility staff and departments.
