The Certified Nursing Assistant (CNA) serves as a vital link between patients, healthcare services, and community resources, ensuring seamless access to care and improving the quality-of-service delivery. Qualifications:

This position involves providing high-level care in various critical environments, including the Trauma Floor, Trauma Resuscitation Bays, Surgical Intensive Care Unit (SICU), and in the management of Emergency General Surgery. The role requires a collaborative approach to patient management, working alongside a dynamic team of healthcare professionals. 
 
Key Responsibilities:

In collaboration with medical providers and other members of the care team, the Murrells Inlet Freestanding ER Registered Nurse (RN) provides individualized, comprehensive, and compassionate care using established nursing models such as &ldquo;Assess, Perform, Teach, and Manage.&rdquo; The RN serves as an advocate for patients/families/caregivers and models a commitment to the organization&rsquo;s vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance.
<ul><li>Assesses patient pain regularly to promote effective pain management, including reassessments after appropriate intervention.</li><li>
Performs procedures, monitoring, or other functions as ordered by the medical provider(s). Documents the administration of care in the patient medical record in a timely and thorough manner.</li><li>
Performs exceptional care by responding promptly to patient requests. Strives to anticipate patient needs and resolve them proactively.</li><li>
Teaches patients/families/caregivers about patient medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete understanding.</li><li>
Teaches patients/families/caregivers about any non-medicinal follow-up measures, such as healthy diet and exercise, disease prevention, and/or other lifestyle changes. Prepares patients and families/caregivers for future self-management.</li></ul><h6 class="jobSectionHeader">

<div>We are looking for a dedicated candidate join our team as a Community Health Worker (CHW) to service our patients in the Myrtle Beach area. The Community Health Worker (CHW) serves as a vital link between patients, healthcare services, and community resources, ensuring seamless access to care and improving the quality-of-service delivery. This role includes visiting patients in their homes and Assisted Living Facilities (ALFs) to facilitate visits with providers and specialty services. Additionally, the CHW is responsible for delivering 
medications and durable medical equipment (DME) to ensure patients have timely and safe access to 
essential supplies.
Community Health workers perform visits in homes and facilities (ALF and ILF's) in their designated service area. You must have reliable transportation as travel is required daily. This is a full time, salary-based working 12-hour shifts (7:00AM-7:00PM) or 8-hr shifts (8:00am-5:00pm; Monday-Friday)

What Are We Looking For?

Areas of Responsibility:
<ul><li>Work closely with care team members and other staff to ensure coordinated patient care and compliance with the care plan.</li>
<li>Communicate effectively via phone, email, Microsoft Teams, Athena, in-person, etc.</li>
<li>Facilitate and coordinate visits with PT&rsquo;s, OT&rsquo;s and Providers in Assisted Living Facilities and patients' homes to enhance patient care and support.</li>
<li>Deliver medications to patients&rsquo; homes, review instructions, and perform medication reconciliation, ensuring accuracy between current medication lists and what is found in the home.</li>
<li>Coordinate and facilitate telehealth visits between patients and pharmacists, specialists, therapists, and primary care providers, ensuring patients receive timely care.</li>
<li>Set up training, instruct patients on the use of Remote Patient Monitoring (RPM) equipment, 
providing ongoing support, education and troubleshooting, when necessary.</li>
<li>Deliver durable medical equipment (DME) to patients in homes and ALFs, ensuring proper 
assembly and functionality. Provide training to patients and caregivers on the correct use of 
DME.</li>
<li>Perform routine venipuncture for laboratory testing and ensure proper specimen handling and labeling.</li>
<li>Collect blood specimens during home or facility visits as needed to support patient care and diagnostics.</li>
</ul>
Qualifications:
<ul><li>Certified Medical Assistant, Paramedic, or Licensed Nurse preferred. Certifications or licenses must be in good standing.</li>
<li>Phlebotomy experience preferred. If not experienced in phlebotomy, candidate must be willing to complete training and demonstrate competency in the skill.</li>
<li>Previous experience in public health or home health preferred.</li>
<li>Experience with DME delivery or logistics operations preferred.</li>
<li>Must be proficient in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records.</li>
<li>Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.</li>
<li>Ability to read and communicate effectively. Strong written and verbal communication skills.</li>
<li>Basic computer knowledge.</li>
<li>Must be able to work independently with minimal supervision and direction.</li>
<li>Should demonstrate good interpersonal and communication skills under all conditions and circumstances.</li>
<li>Must be able to work flexible hours and travel between offices, facilities, etc.</li>
<li>Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working condition.</li>
