Job Summary</b></p>
Join Ellison 8, a specialized 36 bed cardiac surgical step down unit caring for adult patients recovering from some of the most advanced heart procedures available. This role offers experienced RNs the opportunity to care for patients with complex cardiac conditions, including coronary artery disease, valvular heart disease, aortic disease, and congenital heart disease, following procedures such as coronary artery bypass grafting, valve replacement, heart transplantation, and ventricular assist device (VAD) placement. Nurses on Ellison 8 play a critical role in both recovery and education, partnering with patients and families to promote cardiac risk factor modification and support long term heart health in a highly collaborative, patient focused environment.<br>

<p><b>Operating Room Clinical Nurse
</b></p><p>The Biosurgery Clinical Specialist provides clinical assessments on adjunctive hemostat device needs, risks, recommendations and implementation recommendations to customers (primarily nurses, educators, risk managers, surgeons and techs). This role plays an integral part in gathering key insights on utilization, techniques, applications based by specialty or service lines, capturing documenting and sharing these insights with the US Marketing team and partnering with local sales team to work collaboratively with sales management and the sales representative to gain appropriate access into aligned pilot accounts. Pivotal to the success of the candidate will be his/her ability to develop effective working relationships with the customers in the assigned territory. The incumbent must be able to work autonomously, be knowledgeable in adjunctive hemostats and confidently interact with clinical personnel and management.</p>
<p>The candidate chosen for this role will be expected to:</p>
<ul><li>Complete training on adjunctive hemostat devices and products to be proficient in product utilization and demonstration.</li>
<li>Demonstrate mastery of all clinical information related to product application and the IFU; must be able to use this mastery to help drive and convert business by upgrading to more advanced brands.</li>
<li>Develop strong, working relationships with surgical leadership and clinicians while providing adjunctive hemostat recommendations and bleeding risk assessments.

Job Summary</b></p>
The MGH Palliative Care RN Navigator (per diem) for the Inpatient Palliative Care Inpatient Consult Service serves as a central coordinator in managing referrals, patient flow, and care transitions for patients with serious or complex illness. This role is responsible for receiving inpatient palliative care consult referrals from a variety of sources, including inpatient providers, outpatient clinics, and the emergency department, and collaborating with referring clinicians to obtain additional clinical and psychosocial information necessary to appropriately triage and prioritize referrals. Prioritization is based on symptom burden, acuity of illness, goals-of-care needs, and patient/family support requirements to ensure patients with the most urgent palliative care needs receive timely intervention. The RN Navigator facilitates communication and coordination among interdisciplinary teams and specialty services to promote continuity, consistency, and alignment of care plans across the continuum. The role may also include participation in interdisciplinary huddles with Emergency Department, Palliative Care, Case Management, and other clinical teams to assess patient appropriateness for established care pathways and program protocols. The RN Navigator educates clinicians and staff on pathway criteria, supports appropriate patient entry into designated programs, and monitors patient progression through the continuum of care to ensure timely placement into the next appropriate level of care or disposition setting. In collaboration with these clinical teams the RN Navigator gathers pertinent clinical and social information to determine the most appropriate service line, level of care, and care setting within the health system, helping to optimize patient flow, resource utilization, and patient-centered outcomes.<br>

This is a per diem, hybrid position, consisting of 8-hour day shifts during the week/on weekends. There may be opportunities for remote work.<br>

Under the supervision of the Nursing Director and as a member of the Unit Based Patient Care Team, the unit secretary performs a wide range of secretarial duties and coordinates unit/floor procedure to ensure professional and efficient operation of the department in providing quality patient care. Performs the following duties to support unit operations and coordination of resources including but not limited to, answering unit phones, collaborating with health care team members and other departments, scheduling patient tests and appointments, maintaining all medical records, initiating and tracking unit requests, coordinating discharges, transfers, and admissions proactively to assure efficient operation and management of resources on the unit. Demonstrate the knowledge and skills necessary to provide indirect patient family centered care appropriate to the age of the patients served on assigned unit. <ul><li>Management of Resources/supplies and unit based needs</li><li>Orders, expedites and distributes unit, clerical and patient care supplies as directed by the Nursing Director to assure par levels required of efficient operations.</li><li>Orders supplies (including Workflow One) necessary for the unit as described by the Nursing Director through eBUY.</li><li>Maintains work area and office equipment in neat and functioning order.</li><li>Initiates and tracks appropriate Facilities, Bio Med, Housekeeping, and IT related problems/replacements/ requests or work orders.</li><li>Notifies Biomed department of malfunctioning equipment and arranges repair requests and for the equipment to be returned to the department.</li><li>Assists Nursing Director to manage and facilitate unit projects.</li><li>Acts as a liaison and assists in problem solving with ancillary services and all other departments which provide services or have an effect on unit activities.</li><li>Provides back-up coverage for patient transport, including escorting discharge patients/families.</li></ul><br>
<ul><li>Management of Information</li><li>Answers, coordinates and manages all phone calls to the unit in consultation with the charge nurse. Takes accurate messages and ensures their delivery to the</li></ul>
appropriate staff in a timely and professional manner. Places pages and phone calls upon request of clinical staff.<br>
<ul><li>Participates in shift-to-shift hand off communication by giving or receiving hand of report to/from the incoming/outgoing unit secretary and charge RN.</li><li>Coordinates and notifies appropriate staff regarding all paperwork coming onto the unit including fax referrals.</li><li>Maintains all patient medical records per hospital policies and procedures in a neat and orderly manner; properly label each page and file testing results and other information in individual medical records accurately and in a timely manner.</li><li>Schedules patient diagnostic tests, appointments, and consults upon the direction of physician or RN.</li><li>Coordinates logistics of patient discharge including transportation, equipment, etc.</li><li>Performs clerical duties to admit, transfer, and discharge patients with the use of bed management system. Make certain that the patient record/chart, information plate</li></ul>
and/or labels, personal belongings, and other pertinent information accompany the patient on transfers.<br>

<ul><li>Quadramed assignment sheet and provides to the charge nurse prior to the beginning of each shift.</li><li>Recovery charges, and or billing information as indicated</li><li>Checks the quadramed list by 11:30am and notifies the charge nurse of any unclassified in-patients as well as reminding staff to complete their quadrameds.</li><li>Copies patient charts and other materials as requested.</li><li>Utilizes Meditech system to: access and update administrative data screens, to enter transport requests, enter PASSPORT orders, print specimen labels, print kardexes,</li></ul>
print medication administration report.<br>
<ul><li>Requests and maintains old medical records as needed.</li><li>Anticipates discharges and transfers and calls them to admitting/nursing supervisor when discharges are known and inputs any tentative discharges in bed management system.</li><li>Updates charge nurse/clinical leader, admitting and nursing supervisor of admissions, discharges, and transfers on 7-3 and 3pm-11pm shifts and inputs any tentative discharges in bed management system.</li><li>Communicates with healthcare team members via VOCERA communication device.</li><li>Performs all other duties as assigned.</li></ul>
