Job Summary: The Housing Stabilization Case Manager for the Streets to Home Indy Initiative provides intensive, housing-focused case management to individuals and families transitioning from street homelessness into permanent housing. Each Case Manager supports a caseload of approximately 15 households, beginning at move-in and continuing for at least 12 months to ensure long-term housing stability. The role involves conducting regular home-based visits, developing individualized housing stability plans, connecting clients with income, healthcare, and social supports, and coordinating with landlords, housing navigators, and community partners. Case Managers also participate in regular case conferencing, maintain accurate records in the Homeless Management Information System (HMIS), and adhere to Continuum of Care standards. This position is dedicated solely to the Streets to Home Indy Initiative and requires strong collaboration, compassion, and problem-solving skills to help clients overcome barriers and maintain permanent housing. 
Scope of Role/Principal Responsibilities: The following responsibilities include, but are not limited to: 


Case Management Duties:

<li>Manage approximately 15 households (not to exceed 20) referred through Streets to Home case conferencing.</li>
<li>Serve clients for the full lifespan of the program (from move-in through at least 12 months of case management support).</li>
<li>If a household exits early, be reassigned to new clients as needed.</li>

Case Conferencing &amp; Planning
<ul>
<li>Actively participate in daily and weekly case management meetings.</li>
<li>Develop individualized housing stability plans tailored to each household&rsquo;s needs, with a clear focus on sustaining housing beyond the 12-month rental assistance period.</li>
<li>Collaborate with CoC workgroups and partner teams to mitigate tenancy risks.</li>
</ul>
Client Support Services
<ul>
<li>Assist with move-in preparation: paperwork, coordination with landlords, housing navigators, and outreach teams.</li>
<li>Provide weekly, home-based case management covering:
<ul><li>Intake/assessment</li>
<li>Housing stabilization and budget planning</li>
<li>Employment or disability benefits assistance</li>
<li>Access to healthcare, insurance, and food benefits</li>
<li>Transportation support</li>
<li>Eviction prevention activities</li>
</ul></li><li>Liaise with community providers for wraparound support (behavioral health, family reunification, skilled nursing, etc.).</li></ul> 


Data &amp; Compliance

<li>Complete timely HMIS enrollments and case notes, documenting all interactions and services provided.</li>
<li>Adhere to Coordinated Entry policies and Continuum of Care standards.</li>

Additional Duties

Required Education, Training, and Professional Licenses &amp; Credentials:
<ul><li>3-5 Years of demonstrated experience in case management or direct service work with individuals experiencing homelessness or housing instability.</li>
<li>Knowledge of and ability to apply housing-first and eviction prevention strategies.</li>
<li>Experience supporting clients with wraparound needs (e.g., healthcare, behavioral health, disability benefits, food assistance, employment supports).</li>
<li>Familiarity with service coordination and data systems (e.g., HMIS or similar case management databases).</li>
<li>Ability to work effectively with individuals of diverse backgrounds regardless of race, ethnicity, religion, disability, age, veteran status, medical condition, or justice system involvement.</li>
<li>Strong organizational, communication, and interpersonal skills with the ability to manage a caseload of 15&ndash;20 households.</li>
<li>Commitment to trauma-informed, client-centered, and equity-based practices.</li>
<li>Proficient in all Microsoft and Google applications and ability to master use of an electronic database;</li>

 Position Summary
 
 
 
 The Pharmacy Technician assists the pharmacists in storing, preparing, dispensing, reconciling, and charging of all medications. Assists the pharmacists in maintaining an adequate and accurate drug inventory as well quality control procedures. Pharmacy Technicians shall work under the direct supervision of a licensed registered pharmacist and will only perform activities that comply with the State Board of Pharmacy practices.

 Accurately prepares medication orders for dispensing by the pharmacist by retrieving, counting, pouring, labeling, and mixing medications

 Verifies the information on the prescription is complete and accurate

 Enters patient and medication order information into the pharmacy system
 </li>
 <li>
 Coordinates billing, processes and submits insurance claims
 </li>

 Understands the basics of the 340B program and how it fits into the pharmacy workflow
 </li>

 Knowledge and understanding of standards for any accreditations which the pharmacy holds
 </li>

 Assists patients and prescribers with the Prior Authorization processes

 Provides quality customer service to patients and other healthcare providers

 Cleans, and maintains equipment and work areas according to pharmacy protocols

 Establishes and maintains patient profiles, including allergies and lists of medications taken by individual patients
 </li>

 Receives and stores incoming inventory and supplies, verifies quantities against invoices, and informs pharmacists of stock needs and shortages

 Performs all other duties assigned by the pharmacy manager

 High School Diploma or GED required

 Licensed Pharmacy Technician in Indiana with license in good standing

 Must have at least 1 year of current retail pharmacy technician experience
 </li>
 <li>
 Strong attention to detail and ability to follow procedures precisely

 </li>
