 Case Managers are responsible for providing direct case management services to children, birth families, foster families and primary caregiver(s) in the home setting, day care, preschool, and/or school to ensure they receive services appropriate to their needs, and continuity of care focused on resolving or preventing the issues of child abuse or neglect. The Case Manager provides assessment, planning, linkage, coordination, monitoring, evaluation and advocacy to the child, family, foster family and primary caregiver. The Case Manager works as a team member with a therapist, foster parents, primary caregiver, guardian ad litem and other appropriate professionals.  Position Responsibilities Include

 Arranges visitation schedule structured upon the completed visitation plan with input from the appropriate parties, supervise visitations and document observations as appropriate

Purpose &amp; Impact:

The purpose of the Child Welfare Case Manager is to provide full wrap around case management services to LSF clients and families while establishing a relationship with them and living the agency&rsquo;s core values.

Essential Functions:
<ul>
<li>Accept all cases assigned by unit supervisor and meet with children in their primary residence within 2 working days of ITR staffing and with identified parents in sufficient time to complete a Family Assessment within 15 working days of ITR staffing.</li>
<li>Negotiate and develop a case plan based on identified strengths and needs of the family, the circumstances bringing the family into care, recommendations provided by the Comprehensive Behavioral Health Assessment and other relevant data. This plan is to be created in collaboration with the parents, Guardian Ad Litem, foster parents and other pertinent parties with the goal of reunification or other permanency for the child.</li>
<li>Visits the child in their primary residence according to required frequency, but no less than once every 25 days. At least one visit each quarter is to be unannounced.</li>
<li>Accurately document all case activities in the Florida Safe Families Network database within 48 hours. Maintain the electronic case file in chronological order, by subject, in accordance with the standardized case file format guidelines.</li>
<li>Submit service requests to the lead agency and provide clients with timely referrals to services. Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources. Maintain regular contact with service providers and document service progress in FSFN.</li>
<li>Complete and submit court documentation within in required time frames. Prepare for, attend, and participate in all court activities as necessary.</li>
<li>Arrange for, attend, and participate in individual case staffings as necessary. Complete all required staffing and application packets.</li>
<li>Conduct initial and/or ongoing child safety and family functioning assessments as required. Prepare initial and on-going safety plans as necessary.</li>
<li>Arrange for emergency placement, emergency medical treatment, and emergency services for children at risk.</li>
<li>Conduct diligent searches for parents and family members when deemed necessary and thoroughly document that the effort has been made to find the parents and family members.</li>
<li>Conduct home studies as required for prospective placements. Provide relevant medical, psychological, behavioral and educational background information about the child or children to prospective care-givers as needed. Plan and facilitate parental and sibling visits as needed and appropriate.</li>
<li>Transport children as needed.</li>
<li>Provide direct supervision of children awaiting placement.</li>
<li>Ensure that all Independent Living functions are completed as required</li>
<li>Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by the supervisor.</li>
<li>Function as agency on-call Case Manager as scheduled.</li>
<li>Organize, prioritize and complete all work assignments by the established deadlines.</li>
</ul>

Pediatric Registered Nurse

Imagine Pediatrics is a tech enabled, pediatrician led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity.</div>
The primary location of this position is remote (EST required) with an expected schedule of 3x12s (7:00am - 7:00pm EST) Monday-Wednesday or Wednesday-Friday.

What You'll Do

The Care Team Assistant provides both clinical and clerical support and ensures the provision of quality and compassionate evidence-based care in a virtual Value-Based Care environment. In this position, you will:
<ul>
<li>Accept incoming phone calls and return calls from patients involving clinical inquiries for responsible providers, perform protocol-based triage as appropriate, and route phone calls in the form of a phone note document in the EHRS to the provider for review and instructions</li>
<li>Place calls and create template-based letters, as directed by the provider, regarding all lab results for delivery to the patient in a timely manner</li>
<li>Review schedule on a daily basis to ensure that all pre-visit preparations have been completed prior to the patient's visit, ensuring lab and imaging results, consultation reports, care transition details, or major changes in health status are available in the patient's medical record; when these documents are not present in the record, it will be the Medical Assistant's responsibility to call the patient, consultants, imaging centers, and/or hospitals to arrange for timely delivery of said information</li>
<li>Call patients deemed to be at risk for a &quot;no-show&quot; prior to their appointment as well as new patients establishing care for their onboarding visit to remind them of both the patient's and Imagines' responsibilities; contact &quot;no-show&quot; patients and inquire as to their status and need for rescheduling, including assessment of barriers to care and initiation of appropriate social service referrals</li>
<li>Initiate completion of forms for review and final signature by provider (disability forms, home health orders, durable medical equipment and supply requests, disease status letters)</li>
<li>Assist care team in regular completion of reports that will allow it to manage and assess the health needs of the patient population; assist care team in recognizing patients who are members of a &quot;vulnerable population&quot; and provide appropriate social service or community-based referrals</li>

First and foremost, you're passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve. You will need:

<li>High-school level education or equivalency</li>
<li>Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an outpatient setting</li>
<li>Spanish fluency preferred</li>
