Job Summary: </b>The Clinical Pharmacy Specialist serves as a leader in clinical pharmacy practice in a specific clinical area, or various clinical areas, as determined by hospital needs. Specialists provide comprehensive clinical and distributive medication management services and will utilize their advanced clinical skills in the provision of evidenced-based, safe, and cost-effective drug therapy to achieve optimal medication therapy outcomes. They serve as a drug information and educational resource for the pharmacy staff, patients, residents/students, medical staff, nursing staff, and other healthcare professionals. The Clinical Pharmacy Specialist will participate in the development of clinical guidelines/protocols, new programs/services, and facilitate professional development activities in their area of expertise. This position will be working Monday to Friday, working the day shift.</p>
<p><b>
Essential Responsibilities:
</b></p><p></p><p>Practices pharmacy in accordance with laws, rules and regulation of the Massachusetts Board of Registration in Pharmacy, Department of Public Health, U.S. Food and Drug Administration, and Drug Enforcement Administration and in accordance with established departmental policies and procedures. Maintains credentials in accordance with the Massachusetts Board of Registration in Pharmacy.</p>
<p>
Monitors patients in a specific unit/specialty service, including collecting patient-specific data, interpreting drug-related laboratory data, performing relevant physical assessments, interviewing patients, and identifying patient goals of therapy.</p>
<p>
Takes part in the development, administration, and improvement of all ambulatory pharmacy programs and services. Independent development of drug use evaluations, ADR reporting, clinic development and management, and creation of business plans</p>
<p>
Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, physical assessments as related to drug monitoring and adverse effect management, review of medication profiles and the patient medical records.</p>
<p>
Documents clinical interventions in the electronic health record in a timely and professional manner.</p>
<p>
Coordinates and interprets laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related laboratory testing.</p>
<p>
Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing ADR's, eliminating unnecessary drug use and duplication.</p>
<p>
Coordinates continuing education activities for medical and nursing staff; orients new employees on pharmacy practices and procedures. Includes submissions to the pharmacy newsletters, participating in journal clubs, and providing in-services to professional staff.</p>
<p>
Precepts PGY1 and PGY2 pharmacy residents, pharmacy students, and contributes to staff development in area of specialty practice. Works collaboratively with other clinical pharmacy specialists to develop the pharmacy skills of pharmacy staff members by teaching and exemplifying those skills to the team members.</p>

Required Qualifications:</b></p>
<p>
Bachelor's degree in Pharmacy required. Doctoral degree in Pharmacy preferred.</p>
<p>
Registration Massachusetts Pharmacist required.</p>
<p>
1-3 years related work experience required.</p>
<p>
Completion of PGY1 accredited pharmacy residency or 3+ years of Clinical Pharmacy experience in hospital setting.</p>
<p>
Must obtain one or more certification (Certified Specialty Pharmacist (CSP), Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP)), within 18 months of hire.</p>

GENERAL SUMMARY/ OVERVIEW STATEMENT: Clinical Pharmacists assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Clinical Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services in a practice setting with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management within specialty patient care services and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacists serve as a departmental resources and liaisons to other departments, hospital personnel, or external groups. Clinical Pharmacists provide medication and practice-related education/training and actively serve as preceptors for pharmacy residents and Doctor of Pharmacy students. PRINCIPAL DUTIES AND RESPONSIBILITIES: Inpatient/Ambulatory Oncology Clinical Pharmacist: An Outpatient Oncology Clinical Pharmacist collaborates with and serves as a resource for nurses, physicians and other clinicians who participate in the management of the oncology/hematology patient as well as other patients receiving care at the MGH Cancer Center. He/she is responsible for coordinating and delivering pharmacy services in compliance with all applicable policies, regulatory agency requirements and state and federal laws. Job-specific Responsibilities: - Reviews and monitors patient treatment (chemotherapy and ancillary) medications for appropriateness based on current clinical and institutional guidelines as well as patient-specific criteria. - Monitors patients&rsquo; medication regimens for drugs interactions, toxicities, and cost-effectiveness. - Assesses all medication orders to ensure they meet established standards and practice guidelines of MGH Cancer Center. Job Summary</b></p>
The MGH Department of Pharmacy is seeking a Clinical Pharmacist to join our Medication History/Transitions of Care team. This position will be focused on transitions of care, starting with admission medication histories, reconciliation, and supporting the continuum of care for patients from admission to discharge. The Clinical Pharmacist will supervise pharmacy technicians/students/residents participating in the daily conduct of this pharmacy program.

Clinical Pharmacists assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Clinical Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacists serve as a departmental resources and liaisons to other departments, hospital personnel, or external groups. They proficiently manage the appropriate use of medications according to professional and regulatory standards. They also conduct practice-related research and patient care, quality, and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice.
