Essential Responsibilities:
</b></p><p></p><p>Practices pharmacy in accordance with laws, rules and regulation of the Massachusetts Board of Registration in Pharmacy, Department of Public Health, U.S. Food and Drug Administration, and Drug Enforcement Administration and in accordance with established departmental policies and procedures. Maintains credentials in accordance with the Massachusetts Board of Registration in Pharmacy.</p>

Monitors patients in a specific unit/specialty service, including collecting patient-specific data, interpreting drug-related laboratory data, performing relevant physical assessments, interviewing patients, and identifying patient goals of therapy.</p>

Takes part in the development, administration, and improvement of all ambulatory pharmacy programs and services. Independent development of drug use evaluations, ADR reporting, clinic development and management, and creation of business plans</p>
<p>
Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, physical assessments as related to drug monitoring and adverse effect management, review of medication profiles and the patient medical records.</p>
<p>
Documents clinical interventions in the electronic health record in a timely and professional manner.</p>
<p>
Coordinates and interprets laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related laboratory testing.</p>
<p>
Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing ADR's, eliminating unnecessary drug use and duplication.</p>
<p>
Coordinates continuing education activities for medical and nursing staff; orients new employees on pharmacy practices and procedures. Includes submissions to the pharmacy newsletters, participating in journal clubs, and providing in-services to professional staff.</p>
<p>
Precepts PGY1 and PGY2 pharmacy residents, pharmacy students, and contributes to staff development in area of specialty practice. Works collaboratively with other clinical pharmacy specialists to develop the pharmacy skills of pharmacy staff members by teaching and exemplifying those skills to the team members.</p>

Required Qualifications:</b></p>

Bachelor's degree in Pharmacy required. Doctoral degree in Pharmacy preferred.</p>

Registration Massachusetts Pharmacist required.</p>

1-3 years related work experience required.</p>

Completion of PGY1 accredited pharmacy residency or 3+ years of Clinical Pharmacy experience in hospital setting.</p>

The Implementation Specialist at Clearway Health will work with new partnership hospitals to integrate and launch new pharmacy services. The Implementation Specialist is deployed to the client at the onset of the partnership and is responsible for providing hands on, in person, integrated care for patients with high medication cost. You'll be responsible for delivering safe and efficient specialty services while training the incoming new team members including Pharmacy Technicians and Pharmacy Liaisons. When not integrated at a new hospital partnership, you will be assigned work from an existing liaison that requires additional support, or other special projects in support of company or departmental initiatives.</p><p></p><p><i><br>
This position requires extensive </i><i>air and/or auto travel to and from the client sites (domestic only). The Implementation Specialist is expected to travel from several weeks to up to one year for each client site. The Implementation Specialist is a full-time travelling position; the qualified candidate must be willing to travel, which will include overnight and extended stays at a client's location. Implementation Specialists are responsible for the filing and renewing of any primary state pharmacy technician license registration. The Implementation Specialist is required to have a valid driver&rsquo;s license with current auto insurance. Clearway will reimburse any state pharmacy technician license registration fees (besides their home state) and travel expenses.</p>

Implementation &amp; Change Management:</b></p>

<li>Provide hands-on expertise, resources, and knowledge to new hospital sites as part of a new client partnership agreement</li>
