The new outpatient clinic/practice will be in a newly built practice location on the BWH Main campus.

About this Outpatient Staff Nurse job:<br>
The Staff Nurse practices at a high level of autonomy and accountability in collaboration with the multidisciplinary care team to provide a continuum of patient care from prevention, early detection, diagnosis, treatment, follow-up, palliation, bereavement and survivorship. They are integral in coordinating patient care through specialized clinical centers and programs, including our partners in radiation oncology, surgery and clinical research, to develop and implement a personalized care plan for each patient aimed at achieving optimum clinical outcomes.<br>
<br>
The Staff Nurse systematically and continuously collects and assesses data related to patient health status to develop, execute, and evaluate the plan of care in the ambulatory/outpatient care setting. They provide necessary assessments, administer treatments, and communicate with the multidisciplinary care team. The Staff Nurse provides patient and family education and psychosocial and emotional support. They ensure that care is safe, holistic, efficient, effective, timely and meets the cultural, spiritual, and ethnic needs of each patient and family. While this position supports an MGBCI cost center operationally, the Staff Nurse functions within the Brigham and Women&rsquo;s Hospital Department of Nursing professional governance structure, and all nursing staff maintain reporting alignment through BWH nursing leadership.<p></p><p><b><br>
Job Summary</b></p>
The Staff Nurse will be responsible for:<br>

<ul><li>The staff nurse serves as contact with patients at home and in the ambulatory/outpatient setting to ensure optimal clinical management in collaboration with attending physicians, advanced practice providers, fellow nurses, support staff and other members of the multidisciplinary care team.
</li><li>Communicates with patients via daily telephone triage and Gateway messages.</li><li>Assists in triage of symptom management and medical urgencies/emergencies to attending physicians and advanced practice providers and guides the patient to the appropriate level of care in the appropriate time frame.</li><li>Monitors patients at home for anticipated chemotherapy/surgery/radiation-associated toxicities, and screening for unanticipated toxicities/events.</li><li>Coordinates home services including, but not limited to, visiting nursing, palliative care and hospice, home infusion, laboratory services and monitoring, etc.</li><li>Coordinates and performs blood draws and routine flushing and maintenance of central lines, including dressing changes.</li><li>Coordinates, performs, and documents injections and vaccinations per protocol.</li><li>Coordinates, performs and documents intravenous therapies within the practice and in collaboration with the Short Stay Infusion Units.</li><li>Assists attending physicians and advanced practice providers with outpatient diagnostic and therapeutic procedures including, but not limited to, bone marrow biopsies, lumbar punctures, etc.</li><li>Educates patients and families about their disease, treatment plan, medications, and home care needs.</li><li>Provides assistance with disability forms, FMLA forms, Advanced Directives, and pre-authorizations as needed.</li><li>Assesses psychosocial needs of patients and families. The Licensed Practice Nurse (LPN) provides support for complete and efficient patient care within practices and/or locations of the Department of Dermatology. Working under the direction of the Practice Manager and/or Practice Coordinator, the LPN will interact with customers in person, by telephone and/or electronically. Since the role has significant impact on patient care, patient satisfaction, clinical operations and clinical revenue, the position requires outstanding customer service skills, attention to detail and having the appropriate technical skills to meet the job requirements in a timely and accurate manner.

While the LPN will be expected to have a working knowledge of these duties and responsibilities, the overall efforts for each will vary based on daily practice operational needs, staffing, location and specific functions. </li><li>Ensure that pathology, microbiology, virology, and other similar tests have been appropriately logged and followed-up per practice, department and hospital standards.
</li><li>Assist with patient education including informed consents, wound care, medication and post-procedure instructions.
</li><li>Prepare instruments, kits and trays for central sterile processing sending and receiving. Monitor instruments for any damage, notify management team when appropriate.

Job Summary</b></p>
GENERAL SUMMARY/ OVERVIEW STATEMENT: As a member of the Room Service team, the individual will be tasked to assist and deliver in a pleasant, friendly and timely manner patient specific high-quality food and exemplary service to patients, guests, and staff. This position requires working with and assisting other members of the Room Service team and Hospital staff to ensure that all patients receive prescribed foods, and guests and staff receive requested food following Federal, State, Local and hospital sanitation and safety procedures. As outlined in the Food Service Department's HAACP program. PRINCIPAL DUTIES AND RESPONSIBILITIES: 1. Act as a liaison to food service for the patient, family and staff. Tasks as liaison: explaining the meal selection process; assist menu selection of patients according to prescribed diet order. To assure patients fed within 45 minutes of ordering the meal. 2. To communicate on the telephone in an efficient, pleasant and respectful manner, even during periods of high call volume, in order to assist patients and guests with their meal service selections. At peak time, a call center operator answers the phone and immediately enters a complete order into the Computrition system. A call center operator's average call time will range between 1 1/2 - 2 minutes. 3. To review therapeutic diet orders and modify the patient&rsquo;s order to meet the diet order parameters while attempting to honor the patient&rsquo;s request and be aesthetically pleasing. 4. To speak with the patient, family member(s) or nurse to explain needed modifications regarding meal orders. To communicate changes and if possible find a satisfactory substitution. To assure a nutritionally balanced meal that meets the dietary restrictions of the patient&rsquo;s diet order. 5. Communicate on the phone or in person to patients, staff, and guests according to department scripting policy and procedures. 6. To communicate patient diet order issues to the Call Center Operator Lead, Diet Tech or clinical dietitian when policy mandates. 7. Identify patients who are not able to place an order via telephone or who have not ordered their meal. Interact with patient family and nursing to enter a complete order in Computrition. Facilitates the meal service or communicate via Computrition the reason for refusal. 8. Acts upon patient, family, or staff meal service requests. These requests may include contacting and following through with the clinical dietitian regarding diet order issues or supplement changes, or room service team members to resolve meal service issues or handle special requests. When necessary escalating requests to management 9. Manage patient nourishment, supplement and tube-feeding delivery. 10. Proficient using the Computrition according department policy and procedures and maintains accurate and updated patient information via the electronic diet office system. 11. Manages difficult or emotional situations and responds promptly topatient needs. 12. Pickup diet order sheets from nursing stations and receive verbal diet order changes. Working in collaboration with Neuro-intensivists, critical care Fellows, and Residents, the Neurocritical Care Nurse Practitioner provides high quality patient care to for neurology and neurosurgery patients in the in the Brigham and Women&rsquo;s Neuroscience Intensive Care Unit.

The Neurocritical Care NP is a member of the multidisciplinary provider team with patient care and management responsibilities similar to those of the interns and junior residents who have month-long rotations in the Neuroscience ICU.<br>

PRINCIPAL DUTIES<br>
1. Elicits a detailed and accurate history, performs an appropriate physical examination, records and presents the pertinent data to the neurocritical care team.<br>
2. Attends, presents and actively participates in daily ICU rounds.<br>
3. Assists with procedures and pre- and postoperative care.<br>
4. Performs and/or prescribes routine laboratory and related studies, medications, and consultations as appropriate and standard for the Neuroscience ICU Service.<br>
5. Enters and signs admission, transfer, discharge, and preoperative orders into Order Entry.<br>
6. Assists in facilitating patient movement/flow through the hospital, particularly as it relates to the admission and discharge process.<br>
7. Serves as a mentor to APP staff in training, residents in training, students, and house staff, educating others while continuing to learn and expand one&rsquo;s own knowledge base.<br>

Completion of a Master&rsquo;s or post Masters&rsquo; degree in Adult program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC).</p></li><li><p>
Satisfactory completion of an approved formal education program which has as its objective the preparation of professional nurses to practice as nurse practitioner.</p></li><li><p>
Current Adult/Gerontology (AGNP) or Family Nurse Practitioner (FNP) Certification by an accredited APRN certifying body required.</p></li><li><p>
DEA registration required and current APRN License to practice in Massachusetts required.</p></li></ul><p></p><p>
Experience</p><ul><li><p>
1. Minimum 5-7 years current nursing or nurse practitioner experience in a Neuro-Critical Care setting required.</p></li><li><p>
2. Nursing or nurse practitioner experience caring for a Neurology focused patient population required.</p></li><li><p>
3. Valid ACLS certification required.</p></li><li><p>
4. CNRN Certification strongly preferred.</p></li></ul><div><br>
