Medical Director Services PC is seeking to hire either a Physiatry PM&amp;R Physician (MD/DO) or a Physiatry PM&amp;R Nurse Practitioner (NP) / Physician Assistant (PA) to provide care at skilled nursing facilities in the Bronx and Brooklyn, NY area.
Prior skilled nursing or hospital experience is a plus.

Position Overview
The selected provider will serve as a key clinical leader delivering physiatry-focused care to short- and long-term residents. This role emphasizes physical medicine and rehabilitation, functional improvement, pain management, and reducing unnecessary hospitalizations.
Key Responsibilities<ul><li>
Provide comprehensive physiatry and medical care to residents in a skilled nursing facility</li><li>
Conduct physical examinations upon admission and on an ongoing/as-needed basis</li><li>
Assess functional status, mobility, pain levels, and rehabilitation needs</li><li>
Develop and implement individualized, evidence-based rehabilitation and treatment plans</li><li>
Collaborate with interdisciplinary teams including nursing staff, therapists, and specialists</li><li>
Participate in care planning, discharge planning, and medical service development</li><li>
Educate residents and families on care plans, rehabilitation goals, and treatment options</li><li>
Prescribe, evaluate, and adjust medications responsibly</li><li>
Ensure compliance with all federal, state, and local regulations</li><li>
Maintain accurate, timely documentation</li><li>
Support staff education and assist with regulatory survey readiness</li></ul>
Requirements
For Physician (MD/DO):<ul><li>
Active New York medical license</li><li>
Board certification or eligibility in Physical Medicine &amp; Rehabilitation (PM&amp;R) preferred</li><li>
Current DEA registration in good standing</li><li>
Skilled nursing or post-acute care experience preferred</li></ul>
