<p>DaVita Kidney Care, is bringing our industry-leading clinical quality to patients on-site at skilled nursing facilities (SNF) through a new team called DaVita Skilled Nursing Dialysis Services. Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line. Ideal teammates will have significant experience within DaVita, get energized by a start-up type environment and improving the lives of patients, and have a track record of finding ways to GSD. Teammates must be comfortable working autonomously and have demonstrated the ability to collaborate with cross-functional resources across the Village.</p><p><b><br>
What you can expect:</b></p><p>
In this role, you take responsibility for the complete ongoing care of each patient to ensure their dialysis treatment stays on track. PCTs will work in conjunction with dialysis RNs to provide dialysis care. You will work 3 days a week - with no required on call, yet your impact on the lives of our patients and their families will be felt every day.</p><p>
Patient Care: Spending the majority of your day in direct one-on-one patient care to provide safe, comfortable and hygienic dialysis treatment.</p><p>
Complexity: DSD Dialysis care is a complex but fulfilling service line. Meeting and/or exceeding clinical outcomes are the goals and multitasking is an expectation to meet the needs of the service, the patients and our SNF partners.
</p><p>Autonomy and Critical Thinking: Provide comprehensive patient care in an independent setting. Plan to be on your toes at all times. You use your mind constantly. You apply your &quot;core&quot; skills to monitor patients before, during and after dialysis treatment including measuring and recording stats, patient observations, and hemodialysis machine setup. There's a lot to track and manage and the joy comes in witnessing how your care impacts patients' lives.</p><p><b><br>

Demonstrate the ability to develop a plan of care that addresses needs across the continuum; develop long and short-term goals with specific time frames for resolution; identify specific services to be provided in the care plan; include the family/caregiver in the plan of care; and show life planning contingencies such as power of attorney and/or advance directives.</p></li></ul></div><div><ul><li><p>

Assess and communicate the patient's social, cultural, emotional, and economic problems to medical staff and other healthcare providers as needed. LCSW-C/LICSW Inpatient Case Manager

<div>About this Job: <b>General Summary of Position</b><br>
The Transporter supports the Manager and staff by performing the following duties; Responsible for the timely courteous and competent transportation of patients throughout MedStar National Rehabilitation Network (MNRN) and MedStar Washington Hospital Center (MWHC) complexes via wheelchair stretcher or bed. He/she will also perform a variety of other support services functions as needed to provide service excellence to patients visitors and hospital team members.<p><b>
Primary Duties and Responsibilities</b></p><br>
<ul><li>
Transports patient his/her personal possessions medical records and attached clinical devices (IV's and oxygen) by wheelchair stretcher bed etc. to and from various hospital locations and to the hospital entrance upon discharge.</li><li>
Ensures the safety of the patient during transport and alerts nursing staff immediately to any signs of patient distress or unfavorable symptoms.</li><li>
Ensures the comfort of the patient by being attentive offering blankets checks modesty and engagement of appropriate conversation with the patient/assists in the lifting positioning and transferring patient to and from examination tables wheelchairs stretchers and /or beds.</li><li>
Provides lift team support as assigned. Assures that patient is comfortable and securely fastened and provides gowns pillows/blankets to patients.</li><li>
Ensures that patient has hospital identification band corresponding with name and transportation record.</li><li>
Wears appropriate protective clothing as needed.</li><li>
Documents arrival and departure times and informs appropriate staff of patient destination location and or time of arrival</li><li>
Delivers and retrieves supplies equipment and records e.g. laboratory specimens results x-ray film unit supplies medical charts pharmacy products parcels and messages.</li><li>
Maintains associated transportation records including name of patient or material transported pickup site destination and departure/ arrival times and notes difficulties encountered and relays to supervisor.</li><li>
Inspects transportation equipment for damage and/or defects.</li><li>
Cleans transportation equipment by wiping surfaces with pre-mixed cleaning solutions. reports damaged or malfunctioning transport equipment as required to service manager and or logs work request and tags and removes equipment from service.</li><li>
Retrieves clean linen and dispose of soiled linen.</li><li>
Restrains patients as directed by holding or strapping with appropriate constraints.</li><li>
Completes transports quickly and efficiently; immediately notifies dispatcher when assignments are initiated completed and when paged by dispatcher.</li></ul>

</li></ul><b>Experience</b><ul><li>
Less than 1 year 6 months of experience transporting patients in a hospital setting. preferred
</li></ul><b>Licenses and Certifications</b><ul><li>
CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required
</li><li>Additional unit/specialty certifications may vary by department or business unit.
</li></ul><b>Knowledge Skills and Abilities</b><ul><li>
The ideal candidate will have a passion for customer service and maintain a professional and courteous manner at all times while interacting with patients family members/caregivers and hospital staff.</li><li>
The Incumbent must be detail oriented and be able to identify safety risks before during and after transport and be attentive and patient with the subject of transport.</li></ul>
