Case Managers are responsible for providing direct case management services to children, birth families, foster families and primary caregiver(s) in the home setting, day care, preschool, and/or school to ensure they receive services appropriate to their needs, and continuity of care focused on resolving or preventing the issues of child abuse or neglect. The Case Manager provides assessment, planning, linkage, coordination, monitoring, evaluation and advocacy to the child, family, foster family and primary caregiver. The Case Manager works as a team member with a therapist, foster parents, primary caregiver, guardian ad litem and other appropriate professionals. The Case Manager will develop, strengthen and maintain a direct working relationship with mental health professionals, community agencies and the child&rsquo;s own support system 
 
Position Responsibilities Include:</div>
<ul><li>Arranges visitation schedule structured upon the completed visitation plan with input from the appropriate parties, supervise visitations and document observations as appropriate</li>
<li>Assists in the provision of appropriate information when applicable and accompanies children to appointments, as necessary, to report progress or lack thereof</li>
<li>Assists the primary caregiver(s) and/or foster family in implementing tasks and activities according to the case plan</li>
<li>Attends and participates in professional development training for professional growth as developed by CBC/Pre Service Training, as required by the state of Florida and in accordance with agency policy and procedures</li>
<li>Identifies strengths, limitations, risk factors, and needs, to refer and coordinate the provision of concrete services to the child, primary caregiver, and biological families of children</li>
<li>Serve as a conduit of information between all parties to the case and identified stakeholders, including medical, educational and mental health providers.</li>
<li>Conduct visits with children, parents and/or caregivers that include the on-going assessment of child safety, permanency and well-being.</li>
<li>Develop and implement a plan that meets the needs of the children and the family.</li>
<li>Facilitate placement and delivery of services in collaboration with biological, foster and adoptive families.</li>
<li>Perform case management responsibilities in accordance with state and federal laws, Department of Children and Families Administrative Codes and Operating Procedures, funding source policies and internal policies on child abuse, abandonment, and neglect within required timeframes.</li>

Purpose &amp; Impact:

The purpose of the Child Welfare Case Manager is to provide full wrap around case management services to LSF clients and families while establishing a relationship with them and living the agency&rsquo;s core values.

Essential Functions:
<ul>
<li>Accept all cases assigned by unit supervisor and meet with children in their primary residence within 2 working days of ITR staffing and with identified parents in sufficient time to complete a Family Assessment within 15 working days of ITR staffing.</li>
<li>Negotiate and develop a case plan based on identified strengths and needs of the family, the circumstances bringing the family into care, recommendations provided by the Comprehensive Behavioral Health Assessment and other relevant data. This plan is to be created in collaboration with the parents, Guardian Ad Litem, foster parents and other pertinent parties with the goal of reunification or other permanency for the child.</li>
<li>Visits the child in their primary residence according to required frequency, but no less than once every 25 days. At least one visit each quarter is to be unannounced.</li>
<li>Accurately document all case activities in the Florida Safe Families Network database within 48 hours. Maintain the physical case file in chronological order, by subject, in accordance with the standardized case file format.</li>
<li>Submit service requests to the lead agency Utilization Management Department and provide clients with timely referrals to services. Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources. Maintain regular contact with service providers and document service progress in FSFN.</li>
<li>Complete and submit court documentation within in required time frames. Prepare for, attend, and participate in all court activities as necessary.</li>
<li>Arrange for, attend, and participate in individual case staffing as necessary. Complete all required staffing and application packets.</li>
<li>Conduct initial and/or ongoing child safety assessments as required. Prepare initial and on-going safety plans as necessary. 
Arrange for emergency placement, emergency medical treatment, and emergency services for children at risk.</li>
<li>Conduct diligent searches for parents and family members when deemed necessary and thoroughly document that the effort has been made to find the parents and family members.</li>
<li>Conduct home studies as required for prospective placements. Provide relevant medical, psychological, behavioral and educational background information about the child or children to prospective caregivers as needed. Plan and facilitate parental and sibling visits as needed and appropriate.</li>
<li>Transport children as needed.</li>
<li>Ensure that all Independent Living functions are completed as required.</li>
<li>Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by the supervisor.</li>
<li>Function as agency on-call Case Manager as scheduled.</li>
<li>Works cooperatively with Program Directors, with other Case Managers, with placement staff, with Protective Investigators, with Child Welfare Legal staff and with agency support staff.</li>
<li>Effectively manages time to ensure that all home visits are completed as required, all documentation is entered into FSFN within 48 hours, court documentation is prepared according to specified time frames and court appearances are attended as necessary.</li>
<li>Follows Florida Statutes, Administrative Code, written policies and orders of the Dependency Court in managing cases toward goals recorded in case plans.</li>
<li>Organize, prioritize and complete all work assignments by the established deadlines. 
All duties are performed in accordance with the following standards: 
Courtesy: Treat clients, the public and staff with courtesy, respect and dignity and presents a positive public image.</li>
<li>Communication Skills: Keep supervisor fully informed of activities, pertinent issues, upcoming events and potential problems. Demonstrate effective oral and written communication skills in daily work.</li>
<li>Teamwork: Support the unit, department and/or organization and work with others in an effort to accomplish the goals of the unit, department and/or organization.</li>
<li>Safety: Employee makes a reasonable effort to adhere to established safety procedures and practices in the work area.</li>
<li>Training: Attend and successfully complete all mandated training courses such as the PDC Assessment and successfully passes the PDC examination, Pre-service Training. MAPP Training, In-service, etc., within the probationary period and as scheduled thereafter. The successful completion of the Field-Based Performance Assessment is an essential performance standard required for continued employment in the class of Case Manager.</li>

Qualifications

Your Responsibilities
<ul>
<li>Serve as the subject matter expert in all areas of recruitment, candidate assessment, and onboarding (full cycle) for multiple locations.</li>
<li>Create, implement, and manage talent acquisition processes to ensure candidates have a positive experience regardless of the outcome.</li>
<li>Strategize and work with the HR team to develop efficient and effective onboarding processes.</li>
<li>Develop processes to fill high-volume positions and applicant pipelines through collaboration with multiple stakeholders.</li>
<li>Build strong collaborative relationships with multiple internal stakeholders such as senior leaders and all hiring managers.</li>
<li>Establish and maintain relationships with external recruitment agencies, schools, and colleges.</li>
<li>Identify, track, analyze, report and act on data to drive system and hiring manager performance, and the applicant experience.</li>
<li>Develop and update job descriptions in partnership with the hiring manager; create job postings accordingly and post them on job boards.</li>
<li>Manage the job postings from beginning to end and ensure vacant positions are successfully filled.</li>
<li>Review all resumes when received and screen them against job requirements. Forward qualified candidates to the appropriate hiring manager.</li>
<li>Coordinate, schedule, and perform initial telephone screenings and thoroughly assess the applicant&rsquo;s salary expectations, experience, skills, and credentials.</li>
<li>Collect feedback from the hiring manager to help push candidates through the hiring process.</li>
<li>Partner with the rest of the HR team to propose, participate, and execute HR projects as needed.</li>
<li>Providing and performing other projects and duties as assigned.</li></ul> 


Our Requirements
<ul>
<li>Bachelor&rsquo;s degree in human resources management, business administration, psychology, communications, or relevant years of experience.
</li><li>2-5+ years of experience required in talent acquisition or relevant experience/roles.</li>
<li>Experience in full-cycle recruiting, sourcing for candidates, and using various interview techniques and evaluation methods.</li>
<li>Strong Excel skills.</li>
<li>Experience with (ATS software/tools/applications).</li>
<li>Strong analytical and problem-solving skills, with the ability to translate complex data into actionable insights.</li>
<li>Must have excellent written and verbal communication, with emphasis during telephone calls, follow-ups, and correspondence.</li>
<li>Reliable transportation to and from work.
</li><li>Ability to calculate basic math.</li>
<li>Must be able to pass a seven-year criminal background check.</li>
<li>Must be able to pass company drug screen and/or substance abuse testing.</li></ul>

<div>Position Highlights:
<ul><li>The Laboratory Specialist is responsible for ensuring proper handling of all specimens received in the clinical and anatomic pathology laboratories and preparation of these specimens for testing. Job duties include computer data entry and accessioning of orders and also specimen processing tasks including centrifugation, aliquoting, slide preparation, packaging, shipment of referral laboratory specimens and transport and delivery of specimens to the testing departments. Additional duties that are of higher level of complexity are also required, including registration and billing functions, assisting on patient procedures, processing samples that must be handled with aseptic or sterile techniques, and troubleshooting issues with samples and orders. These more complex duties may vary by assigned laboratory section.</li>

Responsibilities:
<ul><li>Receive samples electronically in the LIS, while being conscious of duplicate orders, incorrect sample types, and sample requirements. Knowledgeable about panel components. Competent about test order priority, specimen rejection criteria, and shared sample procedure.</li>
<li>Demonstrates a thorough working knowledge of order entry and specimen accessioning functions, including a sound working knowledge of test codes and specimen requirements.</li>
<li>Performs daily, weekly, monthly, and as needed, preventive maintenance procedures including: recording of temperatures, centrifuge rpm and timer checks, and eye wash station maintenance.</li>
<li>Must follow all regulatory requirements in reference to lab safety procedures regarding use of personal protective equipment, universal precautions for sample handling, and safety laboratory practices.</li>
<li>Processes general specimens, ensuring that specimen integrity is acceptable when distributed to lab testing sections.</li>
<li>Provides clerical support for the technical sections to include but not limited to answering phones, recording data, filing slides and paperwork, stocking forms and making copies.</li>
</ul>
Credentials and Qualifications:
<ul><li>High school Diploma or Equivalent required.</li>
<li>Ability to multi-task in a busy, disruptive environment.</li>
<li>Must be a team player, adaptable to change at a moments notice.</li>
<li>A high degree of accuracy and attention to detail required.</li>
<li>Must possess strong oral communications skills and sound reasoning capability based on common sense logic.</li>
<li>Must demonstrate competent computer data entry skills.</li>
