Ensures continuity of pharmaceutical care to and from the home and other patient-care settings. Uses a systematic procedure to communicate pertinent patient information to and from the home and other patient-care settings. Prepares and dispenses medications using appropriate techniques and following Option Care policies and procedures. Accurately calibrates equipment. Prepares medications so they are appropriately concentrated, without incompatibilities, stable, and appropriately stored. Adheres to appropriate safety and quality assurance practices. Prepares labels that conform to the organization's policies and procedures. Medication contains all necessary and/or appropriate ancillary labels. Inspects the final medication before dispensing. Assures presence of authorization to dispense. Manages the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment used in the management of home care patients. Accurately programs, maintains and gives advice to others on the use and maintenance of all medication administration and medication-related equipment used by Option Care. Participates in the process for assessing, managing, and reporting medication-related incidents and equipment error and/or failures. Accurately completes QAR documents according to the Option Care policy. Monitors and reports discrepancies in perpetual inventory systems for schedule II narcotics and other medications subject to diversion.</li><li>
Provides concise, applicable, and timely response to requests for drug information from health care providers and home care patients. Formulates responses to drug information requests based on analysis of the literature. Manages the use of investigational drug products according to established regulations and Option Care policies and procedures. Utilizes pharmacy support personnel effectively. This position serves as an internal resource and expert regarding the definition and interpretation of meaningful use and all other regulatory requirements. This individual will assist the CMIO, System Director of Health Informatics, and System Director of Clinical Informatics in the creation of comprehensive implementation strategies supporting CHRISTUS' compliance with quality and regulatory requirements (i.e., ARRA Meaningful Use, Joint Commission, Medicare Core Measures). Program Manager Clinical Informatics - Converge Clinical Informatics

<div>Description 
Summary:
Responsible for maintaining current and high-quality ICD-10-CM and CPT coding for all Outpatient diagnoses and procedural occurrences, through the review of clinical documentation and diagnostic results, with a consistent coding accuracy rate of 95% or better. The coder will accurately abstract data into any and all appropriate CHRISTUS Health electronic medical record systems, verifying accurate patient dispositions and physician data, following the Official ICD-10-CM Guidelines for Coding and Reporting and CPT Guidelines. Outpatient coding is applicable towards clinical, provider office visits, therapeutic, laboratory, recurring, emergency department, outpatient observation, and ambulatory surgery patient encounters.
Coder will work collaboratively with various CHRISTUS Health departments (Admitting, Charging, Patient Financial Services, HIM, etc.) to resolve charging issues, denials, and physician documentation clarifications, to ensure accurate billing and reduce denials. Coder will also assist in other areas of the department as requested by leadership.
Coder will report directly to their Regional Coding Manager, with additional leadership from the Director of Coding Operations and System HIM/Coding Director.
Responsibilities:

Assign codes for diagnoses, treatments, and procedures according to the ICD-10-CM and CPT Official Guidelines for Coding and Reporting through review of coding critical documentation.</li><li>

Works from assigned coding queue, completing and re-assigning accounts correctly.</li><li>
Manages accounts on ABS Hold, finalizing accounts when corrections have been made, in a timely manner.</li><li>
Meets or exceeds an accuracy rate of 95%.</li><li>
Meets or exceeds the designated CHRISTUS Health Productivity standard per chart type.</li><li>
Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association (AHIMA).</li><li>
Assists in implementing solutions to reduce backend errors.</li><li>
Expertly queries providers for missing or unclear documentation, by working with the HIM department and Clinical Documentation Improvement Specialists.</li><li>

<div>The Clinical Research Coordinator II will ensure that quality research is conducted at the assigned investigative sites in accordance with the sponsor protocol, FDA Regulations, and ICH/GCP guidelines and to provide the best quality data to the sponsor. The CRC II will also ensure study enrollment meets or exceeds Sponsors' expectations. To ensure company goals are achieved on a timely basis. 
 
DUTIES &amp; RESPONSIBILITIES 
1. Completing all the DMCR-required training, including but not limited to ICH-GCP Certification and IATA Certification on a timely basis. 
2. Completing all the relevant training prior to study-start and on a continued basis in a timely manner. This will include but is not limited to:<ul><li>
a. Sponsor-provided and IRB-approved Protocol Training</li><li>
b. All relevant Protocol Amendments Training</li><li>
c. Any study-specific Manuals Training, as applicable</li><li>
d. Sponsor-specified EDC and/or IVRS and any other relevant Electronic Systems training.</li></ul>
3. Conducting study subject visits, and all other relevant protocol-required procedures and documenting these in a timely manner. 
4. Adherence to ALCOA-C Standards with all the relevant clinical trial documentation. 
5. Completing data entry and query resolution in a timely manner as per internal company guidelines and as per sponsor expectations. 
6. Demonstration of appropriate and timely follow-up on the action items, at their respective sites. 
7. Demonstrated understanding and implementation of Laboratory Manuals and protocol-specified laboratory procedures, storage, temperature monitoring, equipment calibration, and laboratory kit inventory, under the direction of the Site/Study Management Team, for assigned protocols. 
8. Liaising with the Laboratory team, Data team, Administrative staff, Clinical Investigators, Research Participants and Sponsor/CRO representatives, under the direction of the Site/Study Management Team, for assigned protocols. 
9. Demonstrating adherence and compliance to the assigned protocols at their respective site(s). 
10. Maintaining a working knowledge of current FDA regulations, ICH-GCP guidelines, all the organizational SOPs, all the guidance documents, and assigned study protocol(s) 
11. Maintaining a working knowledge of the recruitment and retention process for their assigned protocol(s) at their respective site(s). 
12. Demonstrating visit preparedness for all the relevant sponsor and CRO visits for their assigned protocol(s) 
13. Maintaining a working knowledge of the most recent versions of the Study Protocols, Informed consents, Study Manuals, and all the other relevant study-related documents that are utilized and implemented for the assigned protocols at their respective site(s). 
14. Oversee reporting of all Adverse and Serious Adverse Events and any other relevant Safety Information to the appropriate authorities per internal company guidelines, Sponsor, IRB, and ICH-GCP Guidelines. 
15. Completing data entry and query resolution in a timely manner as per internal company guidelines and as per sponsor expectations. 
16. Striving to meet Sponsor subject enrollment goals for their assigned protocols at their respective site(s). 
17. Being prepared for and available at all required company meetings. 
18. Submitting required administrative paperwork per company timelines. 
19. Occasionally attending out-of-town Investigator Meetings 
20. Conduct patient consent discussions and ensure comprehension of medical procedures, risks, benefits, and alternatives 
21. Provide clear explanations of study protocols and ensure patients' understanding before obtaining their consent 
22. Facilitate effective communication between patients, healthcare providers, and research staff 
23. Any other matters as assigned by management.
