Program Overview

The Advanced Care Partner Program creates a team-care structure to collaborate with PCPs who are managing complex patient panels.

The ACP &quot;care team&quot; model pairs a highly clinically experienced provider (ACP) with a PCP to collaborate closely by sharing the care and continuity management of the patient panel. The PCP's task queue provides the focus of this collaborative platform whereby the Advanced Care Partner devotes their work to keeping all categories within SLAs, provides real time responses to patient messages, follows up with chronic care monitoring and management, and intimately understands the patient panel &quot;as their own&quot;, such that complex patients are known well, medical issues are managed efficiently, highest quality conversations engage patients' trust, and continuity of care between the patient and two providers is seamless and exemplary. Further, the ACP care-team model promotes career-sustainability, mitigates the potential for decision fatigue, prevents practice isolation, and addresses burn-out risk factors. The program prioritizes well-being and belonging for both patients and PCPs.

Advanced Care Partner Role:

The Advanced Care Partner is a full-time, remote clinician who works business hours live online with a team of PCP peers, a clinical supervisor, and the Clinical Case Management team of Care Navigators and nurses, as needed, for highly complex cases. The ACP will partner with approximately 5 PCPs and their patient panels, owning these shared panels with longitudinal accountability over time.

The ACP is responsible for maintaining task category SLAs, including but not limited to:

<li>Answering clinical questions from patients and engaging in clinical dialogue to complete these conversations.</li>
<li>Clinical decision-making resulting from information coming from messages, lab results, and consults.</li>
<li>Owning medication changes, titrations, and adjustments for chronic conditions as needed.</li>
<li>Providing video visits and phone follow-up care as needed.</li>
<li>Completing clinical paperwork such as disability, FMLA, home health sign-offs.</li>
<li>Attention to closing patient care gaps, creating orders and follow up as needed</li>
<li>Processing Consults and providing related follow up management/care as needed.</li>
<li>Referrals to and collaboration with Clinical Case Management.</li>
<li>Providing urgent visit patient scheduling as needed.</li>
<li>Providing continuity of care longitudinally.</li>
<li>Participating as an active member of the care team including real-time Slack room responses, attending huddles, presenting cases, providing support to CCM team members.</li>

Job Summary:<ul><li>
The Case Manager &ndash; LEAD role is part of the REACH team that supports ETS&rsquo; mission through fostering cohesion by providing engagement and intensive case management services to individuals suspected of low-level drug offenses and/or prostitution.</li><li>
The Case Manager will provide direct services to a case load of approximately 25-30 individuals. Case managers provide outreach, long-term engagement and supportive services for participants through intensive case management activities and collaboration with LEAD partners, service providers, housing providers and other community organizations.</li><li>
The position is full-time onsite, and the days and hours are Monday-Friday 8:30am-5pm.</li></ul>
Responsibilities:<ul><li>
Help ETS succeed in carrying out our mission through working together with other staff to transform systems of harm and inequity to create different approaches to improving community health and safety through addressing substance use and homelessness.</li><li>
Assist with the organization-wide initiative to reimagine our interconnectedness within our community to overcome the aspects of our society and organizational culture shaped by white supremacy and settler colonialism.</li><li>
Provide Outreach and Intensive Case Management services for assigned participants:</li><li>
Engage participants on the street and at social service provider facilities to establish a working relationship and offer services.</li><li>
Assess participants for severity of chemical dependency and housing status and determine needs for other services, e.g., medical, mental health.</li><li>
Assist participants in gaining access to a variety of funding programs (e.g., SSI, ABD, VA).</li><li>

Develop and implement with the participant&rsquo;s input an individualized Service Plan which addresses the needs of the participant for food, clothing, shelter, and health care and substance use disorder treatment or reduction/elimination of drug/alcohol use through self-change methods. Update this Plan periodically to reflect movement toward or attainment of articulated goals and the emergence of new participant needs and to help the participant move toward the achievement of autonomy.</li><li>
Develop and maintain a working relationship with Sobering Center staff, DSHS workers, chemical dependency treatment providers, mental health providers, health care providers, shelter providers, landlords, detox centers, Assessment Center staff, protective or representative payees, and other community programs which may support participants.</li><li>
Provide structured Intensive Case Management services consistent with program policies.</li><li>
Develop and maintain collaborative relationships with LEAD partners including Seattle Police Department, King County Sheriff, King County and City of Seattle Prosecutor&rsquo;s office.</li><li>
Provide advocacy and support for participants within the criminal justice system including court appearances and written communication.</li><li>
Attend regularly scheduled Operational Work Group Meetings and the staffing of participants with partners.</li><li>
Accompany participants to appointments as needed.</li><li>

Build strong partnerships across Tax, Accounting, Product, and Engineering.</li><li>
Run effective cross-functional forums, clearly communicate status, risks, and decisions, and drive alignment on trade-offs.</li></ul></li><li>
Governance &amp; Delivery Excellence<ul><li>
Maintain integrated plans, RAID logs, and executive-ready reporting.</li><li>
Use Confluence, SharePoint, and Jira to document scope, track delivery, and provide program transparency.</li><li>
Proactively identify risks and issues and drive mitigation plans.

<ul><li>Proven program management experience driving complex, cross-functional technology initiatives end to end.</li><li>
Demonstrated success working with engineering teams in Agile (Scrum and Kanban) environments, including facilitating ceremonies and partnering with engineering leads on planning and execution.</li><li>
Strong requirements analysis skills; ability to translate regulatory/tax/accounting needs into actionable technical work and user stories.</li><li>
Experience working with Tax and Accounting stakeholders, ideally in areas such as tax subledger, tax reporting, or tax form management.</li><li>
Excellent communication, facilitation, and stakeholder management skills; able to work across time zones and functions.</li><li>
Comfortable using Confluence, SharePoint, and Jira for documentation, collaboration, and delivery tracking.</li></ul>
Qualifications<ul><li>
Bachelor&rsquo;s degree or equivalent experience required; advanced degree a plus.</li><li>
5+ years of program or technical program management experience in technology, finance, or tax/finance systems.</li><li>
Experience in tax, accounting, or finance technology programs strongly preferred.</li><li>
Hands-on experience with Agile delivery (Scrum and/or Kanban) required.</li><li>
PMP certification preferred; Agile certifications (e.g., CSM, PMI-ACP, SAFe) a plus.</li><li>
Demonstrated ability to operate independently and drive results in a matrixed, global organization.</li></ul>
The total cash range for this position in Seattle is $146,000.00 to $204,500.00. Employees in this role have the potential to increase their pay up to $233,500.00, which is the top of the range, based on ongoing, demonstrated, and sustained performance in the role.
Starting pay for this role will vary based on multiple factors, including location, available budget, and an individual&rsquo;s knowledge, skills, and experience. Pay ranges may be modified in the future.
