We seek to create a world rooted in self-determination, freedom of expression, and gender justice.Position DescriptionHousing Case Managers at TGIJP are expected to provide comprehensive case management services to clients experiencing housing insecurity or homelessness. This role involves assessing client needs, developing housing plans, advocating for client rights, and connecting clients with appropriate resources and services. Housing Case Managers work closely with clients to empower them to achieve housing stability and improve their overall well-being.Primary Responsibilities1. Client Assessment and Case Planning:<ul><li>Conduct comprehensive assessments of clients' housing needs and barriers.</li><li>Develop individualized housing plans in collaboration with clients.</li><li>Set goals and action steps to address housing challenges and promote stability.</li></ul>2. Resource Referral and Coordination:<ul><li>Connect clients with community resources, social services, and support networks.</li><li>Coordinate referrals for mental health services, substance abuse treatment, and other supportive programs.</li><li>Help clients access financial assistance, food assistance, and other basic needs resources.</li></ul>3. Crisis Intervention and Support:<ul><li>Provide crisis intervention and emotional support to clients in housing crisis situations.</li><li>Recognize early signs of emotional distress, agitation, or conflict</li><li>Maintain a calm and respectful non-confrontational approach</li><li>Collaborate with other team members and external partners to address immediate housing needs and ensure client safety.</li></ul>4. Documentation and Reporting:<ul><li>Maintain accurate and up-to-date client records and case notes.</li><li>Complete required documentation for program reporting and compliance purposes.</li><li>Ensure client confidentiality &amp; privacy in accordance with agency policies &amp; regulations.</li></ul>5. Travel and Home Visits:<ul><li>Entering client homes in a respectful, professional, and non-intrusive manner</li><li>Must have the ability to travel to clients&rsquo; place of residence</li><li>Must be comfortable working in person with clients to assist with needs</li></ul>5. Advocacy and Empowerment:<ul><li>Advocate for clients' rights and interests in housing-related matters as needed.</li><li>Empower clients to self-advocate and navigate housing systems independently.</li><li>Promote client empowerment and self-sufficiency through skill-building and education.</li></ul>6. Budgeting and Analytical Skills:<ul><li>Ability to create and implement client budgets, assist clients in tracking spending and expenses</li><li>Strong organizational and record-keeping skills</li></ul>7.
