Pediatric Integrated Behavioral Health Clinician (LICSW/Psychologist)

<p>The <b>Mental Health / Co-occurring Clinician</b> is responsible for the delivery of mental health and co-occurring enhanced (<b>MH/COE) </b>case management and clinical counseling services to men in recovery <b>from SUD / Addiction AND Moderate to Severe Mental Health Conditions</b> while reintegrating into the community and returning to social, vocation/employment, and/or educational roles. Goal-oriented clinical services are provided in conjunction with onsite psychiatry and onsite medication management to support stabilization and development of skills necessary to achieve recovery.</p><p><b>Job description</b> of what you will do:</p><ul><li>Act in accordance with Hope House, Inc.&rsquo;s Standards of Excellence &amp; Philosophy Care at all times, including maintaining professional ethics, boundaries and client confidentiality and working from a strength-based perspective using evidence-based practices (MAT, MI, CBT, etc.)</li><li>Ensure that all services are delivered in compliance with Federal and State regulations, agency standards, contractual requirements.</li><li><b>Treatment will be trauma responsive and address the intersection between trauma, mental health, and substance use disorders.</b></li><li>Treatment services will include treatment planning, clinical interventions, milieu-based therapy, and family outreach and support when appropriate.</li><li>This <b>MH</b>/<b>COE Counselor will provide care coordination activities (particularly related to the search for housing, at the beginning of treatment), including, but not limited to</b> identifying community resources, i.e., housing, employment, dental, educational/ vocational services, legal/justice related case management, recovery supports, behavioral health clinical services, and pharmacological services for mental health and addiction.</li><li>Obtain signed consent to coordinate all aspects of client treatment.</li><li>Provide supportive and empathic individual and group counseling sessions to clients on a weekly basis, and as needed throughout their stay at the program.</li><li>In a thorough and timely manner maintain effective <b>documentation and record keeping</b>. A <b>clinician </b>must complete an initial biopsychosocial clinical assessment using the six ASAM Dimensions to gain an understanding of addiction severity, co-occurring mental health issues and trauma, physical health issues, family and social supports, housing stability. The initial assessment must conclude with an ICD / DSM diagnosis. The individual treatment plan, must include clinical, psychosocial, and recovery goals, as well as a crisis and/or safety plan; re-assessment; progress notes; referrals and appointments; consent forms; data collection and aftercare / discharge planning.</li><li>Provide information and referral services to clients that must include linking clients to primary care, mental health, Family Support Services (FSS) and other support services. Provide Information and Harm Reduction referral to clients, including linking clients to various types of Medication Assisted Treatment Services (MAT) provided in the Community.</li><li>Develop, implement, and facilitate/co-facilitate assigned psycho-educational groups that include, in part, the nexus <b>between trauma, mental health, and substance use disorders </b>and complete required documentation.</li><li>Conduct intake, screening, and admission of new clients upon request of a supervisor.</li><li>Discuss treatment plans with clients and review in clinical meetings weekly.</li><li>Communicate effectively with the treatment team and any other individuals or agencies involved in the client&rsquo;s treatment.</li><li>Provide milieu coverage as needed, including: monitoring and assessing the integrity of the program&rsquo;s therapeutic milieu; empowering clients to act in accordance with the guidelines of the program; assisting clients in self-administration of medications; conducting controlled medication counts; other client needed services.</li></ul><p><b>Qualifications you MUST meet to be considered for this position</b>:</p><ul><li><b>Degree and experience</b> in one of the following disciplines: clinical psychology, education-counseling, medicine, psychology, psychiatric nursing, rehabilitative counseling, social work, AND who has a MINIMUM OF ONE YEAR OF SUPERVISED SUBSTANCE ABUSE COUNSELING EXPERIENCE.</li><li><b>S</b>uggested minimum of two year&rsquo;s work experience in the field of addiction/SUD treatment and/or mental health. <p>Performs as a working leader in the Patient Transport Service department. Oversees the department&rsquo;s operation in the absence of a manager. Leads employees to exceed customer expectations ensuring patients are being transported in timely fashion. Makes informed decisions, within established guidelines, to serve customers and employees. Communicates and interacts daily with customers (Nursing, Unit Coordinators, Practice Administrators, patients, public, etc.). Transports patients, specimen, deceased patients and other related transport functions. Cleans transport equipment on a daily basis.</p>

Primary Responsibilities:</b><br>
1. In conjunction with the manager/supervisor, schedules and assigns duties for the shift ensuring patients are transported in a timely manner and according to quality standards on the East and West Campus. Monitors daily activities of department work and reports any performance related issues (including conduct, quality, quantity, timeliness) to the supervisor/manager. (essential)</p>

2. Collaborates with transporters to change break times to accommodate patient needs. Safely transports patients, specimens and other items to and from patient rooms and testing and other areas according to departmental protocols. Promptly notifying appropriate personnel of any delays that may affect operational workflow. Transports deceased patients to the morgue. (essential)</p>

3. Ensures all employees are in proper uniform and have identification visible at all times. Reports progress and compliance of all staff and volunteers with department standards to the supervisors/manager. (essential)</p>

4. Monitors and ensure adequate availability of wheelchairs in all lobbies. (essential)</p>

5. Communicates with service response center/ public safety at night as needed. Recognizes and assumes responsibility for reporting patient care or safety concerns to appropriate staff. (essential)</p>

6. In conjunction with the supervisor, conducts initial and follow up training of new employees, volunteers and staff who require additional skill development in accordance with department standards. Actively participates and provides support to Performance Improvement and Service Excellence Teams. (essential)</p>

7. Directs or assumes responsibility for complex, difficult or unusual transport requests. (essential)</p>
<p>
8. Monitors par levels and stocks various locations with equipment (wheelchairs, stretchers). Maintains orderliness and cleanliness of working area, stretchers, wheelchairs, IV poles and other items used in patient transportation. Conducts yearly scheduled equipment cleaning project. 9. Initiates a friendly but respectful relationship with patients and demonstrates responsiveness to patient's physical, psychological and social needs. Makes judgments with regards to patient care and operational flow based on established policies and guidelines. (essential)</p>

<p><b>Additional Responsibilities:</b><br>
1. Performs miscellaneous duties such as collecting and transporting supplies in order to assist in the smooth operation of the department. Performs other work requiring equal skill, effort and responsibility as directed. May perform work of a higher level in preparation for increased responsibility.</p>

<p><b>Required Qualifications:</b><br>
1. Some High School required.<br>
2. License Heartsaver Certificate (CPR) preferred.<br>
3. 1-3 years related work experience required.<br>
4. Ability to collaborate and effectively communicate with physicians and other health care professionals.<br>
5. Ability to follow standard procedures using own discretion and be able to comprehend the needs of patients and react appropriately and with sensitivity.<br>
6. Demonstrated positive interpersonal communication skills. Ability to cooperate with all members of the health care team.<br>
7. Basic familiarity with computers. Ability to navigate at a basic level within web-based applications.</p><br>
