Position Summary
As a Medication Technician with the Hope, Unity, and Freedom (H.U.F.) Center, you are provided the opportunity to make a difference in the daily functioning of adults in need. Med Techs are responsible for completing progress notes, administering medications to clients under the overview of Program Nurses, and ensuring all safety measures are always being adhered to, among other duties. Med Techs are the primary support system for those in their care while at the H.U.F. Center, improving the lives of those around them daily.
Position Responsibilities<ul><li>
Responsible for completing all pertinent clinical documentation.</li><li>
Co-facilitate psychosocial activities and therapeutic integration activities.</li><li>
Responsible for ensuring all safety measures are always adhered to and followed.</li><li>
Monitor clients at all times throughout the premises.</li><li>
Provide direct care services specific to ADLs/IADLs with clients.</li><li>
Participate in weekly team meetings.</li><li>
Transport clients to activities in the community after successfully completing vehicle training.</li><li>
Other duties as assigned by the Program Manager and/or Program Nurse.</li><li>

 At Sentara To Home, medications, home medical equipment, and other healthcare needs can be provided to be prepared after discharge.
 
 
 
 The Pharmacy Tech II : transition specialist that consults with patients about what they will need to have a successful recovery once leaving the hospital. Prior to the day of your discharge, the transition specialist will visit the patient's hospital room to discuss medication needs and self-care requirements. The transition specialist pharmacy technician will arrange for these purchases and coordinate any payment transactions at the bedside and discharge medications and home medical equipment can be delivered to the hospital room before leaving.
 
 
 
 
 The pharmacy tech performs the technician responsibilities in specialized areas of pharmacy practice under the direct supervision of a licensed pharmacist or licensed independent practitioner.

Education 
High School Diploma or Equivalent
Certification/Licensure
<ul><li>American Health Associate BLS Certification required within 90 days of hire.</li>
<li>Minimum requirements: Must have one of the following
<ul><li>Sentara Care Partner Certification Program, or</li>
<li>Successful completion of the Certified Nursing Assistant (CNA), or</li>
<li>National Certified Medical Assistant (NCMA), or</li>
<li>National Patient Care Technician (NCPCT), or</li>
<li>Completion of US Navy Hospital Corps School, or</li>
<li>Emergency Services to include EMR, EMS, EMT, AEMT and Paramedic, or</li>
<li>Current student enrolled in a clinical program (beyond prerequisites) having completed at least one semester of RN, LPN, RT or PT, or</li>
<li>Current work experience within last three years as a PCT or Nursing Assistant.</li>
</ul></li></ul>Experience 
One year of prior health care experience preferred.
