EXAMPLE OF DUTIES:

</li><li>Conduct intake meeting with families-completing required paperwork and initial assessment
</li><li>Request records as needed for service provision
</li><li>Work with interdisciplinary team to ensure referred children are evaluated for eligibility
</li><li>Write Individual Family Service Plans (IFSPs) for eligible children
</li><li>Conduct reviews of IFSPs at least every 6 months
