Under the general direction of the Regional Clinical Manager, Family Services - Tissue, the Tissue Family Services Coordinator 1 (TFSC 1) is responsible for coordinating a wide range of support for potential tissue/eye donor families. Information is to be articulated in an easily understood and accurate manner to families whose ability to process information is impaired by emotionally traumatic circumstances. Infinite Legacy&rsquo;s Tissue Family Services Coordinator 1 coordinates and facilitates emotional support during and after the discussion of the donation process. Rapport is established with donor families, Infinite Legacy team members, and hospital staff involved in the donation process. The Tissue FSC I will be aware and sensitive to diversity of all kinds (cultural, religious, educational, etc.) while working with families and hospital staff. 
Education and Experience:<ul><li>
Master&rsquo;s degree at an accredited college or university in social work, counseling, psychology, nursing, or related degree. Bachelor&rsquo;s degree will be considered if relevant experience is met.</li><li>
1- 2 years&rsquo; experience in an acute health care setting (preferred), hospice, bereavement counseling center, crisis intervention, etc. where there was active involvement in issues involving death, dying, grief and loss.</li></ul> 
Required Skills/Abilities:<ul><li>
Ability to interact with grieving families in a supportive, empathic manner while taking into consideration actual or potential cultural diversities.</li><li>

Duties/Responsibilities:<ul><li>
The Tissue FSC I will establish initial contact over the phone with the potential donor family and offer the option of tissue and eye donation in accordance with all state, federal and industry related regulations. Provides the family with a copy of an authorization or disclosure, Document of Gift if applicable and completes the medical/social history proficiently.</li><li>
The Tissue FSC I will provide trauma, grief, and bereavement support services for the family over the phone during initial contact. Tissue FSC I will identify if additional support is needed and connect the family with local hospital or community resources to provide additional support. In addition, the plans for grief and bereavement services will be communicated to the Aftercare Team at Infinite Legacy who is responsible for following the donor family after donation.</li><li>
Completes all necessary documentation within the time limits established by Infinite Legacy and its regulatory bodies.</li><li>
Collaborates with local funeral homes and whole-body donation programs to ensure that the donor family is supported, and the funeral home or whole-body donation program is aware of the tissue recovery process.</li><li>
Works in conjunction with appropriate Infinite Legacy staff during local tissue and eye cases. Collaborates closely with clinical personnel, DSC, Tissue AOC, HS, and TR during donor management to ensure that donor families&rsquo; needs are communicated to the Infinite Legacy team in addition to ensuring all case logistics are being communicated effectively to support donor and family needs during the donation process.</li><li>
Triages phone calls from families that are received from the Donor Services Center as it pertains to tissue donation needs, hospice referrals and medical evaluation for donation. At times collaboration with Hospice RNs, Nursing Homes or Assisted Living Facilities are needed to ensure the donors wishes can be honored.</li><li>
Navigates sensitive conversations as it pertains to the rejection of tissues post donation process. Initiates follow-up support over the phone to donor families as it relates to offering donor in spirit post donation should tissues be rejected or not able to be transplanted.</li><li>
Facilitates communication with outside OPO&rsquo;s who require local registry checks within IL&rsquo;s DSA. This includes DC, VA, and MD registry checks in a timely manner.</li><li>
Initiates follow-up support to donor and non-donor families when appropriate and is responsible for ensuring effective communication and documentation of aftercare components within the time limits established by Infinite Legacy and its regulatory bodies.</li><li>
Participates in continuing education as identified by the Family Services Manager &ndash; Tissue. This could be but is not limited to conversation coaching, SIM, educational seminars, and classes.</li><li>
Attend regularly scheduled Infinite Legacy staff meetings, FS team meetings, and case review.</li></ul> 
Working Conditions:<ul><li>
Frequent prolonged use of computer screens may produce visual fatigue.</li><li>

Under the supervision of licensed nursing personnel, the Patient Care Technician I delivers direct patient care, including obtaining vital signs, assessing surgical dressings, assisting with activities of daily living, performing phlebotomy, and collecting specimen samples. This position requires careful observation and communication of patient status to accurately identify needs and facilitate appropriate care, working collaboratively with the interdisciplinary healthcare team to ensure patient safety and comfort. Additional responsibilities include supporting patient admissions, transfers, and discharges, maintaining patient environments and supplies, providing wound care and management of orthopedic devices, monitoring blood glucose levels, and promptly responding to patient alarms. Work Experience 
 
Candidates must hold a high school diploma or its equivalent, possess current Maryland state certification as a Certified Nursing Assistant (CNA) or Geriatric Nursing Assistant (GNA), and maintain Basic Life Support (BLS) certification. Although prior experience is not mandatory, preference will be given to candidates with direct patient care experience in a healthcare setting. Successful applicants will exhibit strong verbal and written communication skills, demonstrate the ability to foster effective interpersonal relationships and collaborate within a team, and show proficiency in prioritizing tasks and managing multiple responsibilities under pressure. Furthermore, candidates should be capable of learning and utilizing computer systems, accurately performing blood glucose testing and documentation, and effectively executing delegated nursing tasks that address the diverse needs of patients.</div>

We are currently seeking a Full-Time Day Substance Use Disorder (SUD) Assessor to join our team at Baltimore Central Booking and Intake Center (BCBIC) located in Baltimore, Maryland.
The Substance Use Disorder (SUD) Assessor conducts initial and ongoing assessments of patients in substance use programs to determine substance use treatment needs. They conduct initial screening assessments for those entering the facility and refer patients for addiction medical, substance use counseling and re-entry services as needed. They are also responsible for following up with the patient to make sure they are receiving the desired resources. SUD Assessors also help the team identify people missed on the initial screening through working with the medical team, custody staff and making rounds on the housing tiers. SUD Assessors refer patients for SUD counseling and for coincident medical, mental health and re-entry services as needed.
Shift: Day 7:30a-4:00p<ul><li>
Peer Recovery Specialist or bachelor&rsquo;s degree or higher in Behavioral or Social Sciences (Psychology, Social Work, Counseling or a related field)</li><li>
