 The Pharmacy Tech II : t
 
 
 ransition specialist that consults with patients about what they will need to have a successful recovery once leaving the hospital. Prior to the day of your discharge, the transition specialist will visit the patient's hospital room to discuss medication needs and self-care requirements. The transition specialist pharmacy technician will arrange for these purchases and coordinate any payment transactions at the bedside and discharge medications and home medical equipment can be delivered to the hospital room before leaving.
 
 
 
 
 The pharmacy tech performs the technician responsibilities in specialized areas of pharmacy practice under the direct supervision of a licensed pharmacist or licensed independent practitioner.

A Technician is responsible for contributing to safe and effective operations of the pharmacy, including but not limited to:<ul><li>
