<div>The RN Case Manager is primarily responsible for the daily management and support of the Case Management strategies for care coordination for a group of members who are associated with a Medicare Advantage plan. Qualifications/Requirements:
<ul>
<li>Minimum of 2 years of experience in clinical nursing or rehabilitation for the geriatric population.</li>
<li>2-years managed care experience required.</li>
<li>Minimum of 3-5 years&rsquo; experience doing case management in a managed care environment preferably with a managed care organization or like facility, Preferred.</li>

Essential Functions:

<li>Performs improvement projects involving development of monitoring/collection tools, review of medical records, data entry, analysis, and preparation of audit findings and reports.</li>
<li>Participates in patient care conferences, committee meetings, staff development and educational programs to increase or maintain professional competency.</li>
<li>Correctly applies medical management criteria.</li>
<li>Researches clinical questions from employers, members and payers as required.</li>
<li>Educate members on health access options.</li>
<li>Responds, manages, and resolves day-to-day problems presented in care coordination and communicates effectively with the Facility/Home.</li>

It is expected that the position leads collaborative discussion with hospital leadership, fostering engagement through the enhancement and adaptation of operational strategies to achieve successful care coordination in furtherance of the value-based purposes of the hospital and JV enterprise. This position is responsible for leadership, oversight and training of the clinical liaison and CTA staff and facilitates strong collaboration between all components of service delivery including operations, clinical services, and all shared services.  Navigate conversations with patients seeking insight on Galileo&rsquo;s care model

 Develop an understanding of various health plan contracts / goals, Galileo markets, and needs of various patient populations

Key Responsibilities:<ul><li>
Perform all appropriate nursing assessments per service line requirements, utilizing a high level of clinical competence.</li><li>
Evaluate the client/patient&rsquo;s physical condition and the need for services.</li><li>
Complete required screenings, record the client/patient&rsquo;s initial medical histories and vital signs; coordinate any required action planning based on results.</li><li>

Arrange or monitor special dietary needs for medical conditions.</li><li>
