<li>Implement the individualized Plan Of Care as directed</li>
<li>Identify significant changes in the condition of residents and takes necessary action per facility policies and procedures</li>
<li>Oversees and monitors Certified Nurse Aides</li>
<li>Discharge residents per facility</li>
<li>Completion of Nursing Assessment for Physician Calls prior to calling Physician on any change in resident condition</li>
<li>Communicate with resident’s Point of Contact when they ask for status updates or if there is a change in condition</li>
<li>Provide training to other staff as needed</li>
<li>Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette</li>

<div><p>The consultant will work on the successful delivery of projects for clients as a member of project teams within a market program. This includes providing support to the project manager with initiating, planning, executing, monitoring, and controlling and closing projects. As part of a project team, the consultant will maintain relationships with clients and key decision makers to help identify follow-on business opportunities and maintain customer intelligence.
