Expertise in women&rsquo;s mental health, trauma informed care, and cognitive behavioral therapy are desirable. The ambulatory practice is a major teaching site for the BIDMC Harvard Psychiatry Residency Training Program and is a training site for Harvard medical students, Applicants should be licensed in Massachusetts. The Infusion Unit APP Specialist will work in a recently developed role embedded within our adult infusion unit to serve as an infusion unit first responder, promote infusion services access and efficiency, provide oncology urgent care/symptom management and provide generalist coverage within our ambulatory solid tumor disease centers or hematologic malignancies programs for anticipated leaves and absences.

With an interdisciplinary team approach, this provider will promote a collaborative relationship between providers, professional nurses, research personnel and administrative staff, to provide safe, quality care in compliance with clinical trial protocols across the care continuum.

<ul><li>While on the adult infusion unit, the APP oversees the care of patients with hematologic and oncologic disorders, serves as a first responder for acute reactions and medical emergencies, partners with primary disease center providers to care for complex oncology patients awaiting hospital admission and collaborates with the Cancer Institute weekend first responder team to provide access for urgent evaluation and symptom management during off-hours.
</li><li>The NP/PA is also responsible for collaborative care of the patient with benign hematological disorders receiving care in the adult infusion unit.
</li><li>While in the cancer institute ambulatory practices the APP Specialist is responsible for the care of oncology patients in the outpatient setting, specializing in either solid tumor malignancies or hematologic malignancies in partnership with primary disease center APPs, physicians, practices nurses and administrative support team members.
</li><li>Flexibility in scheduling as this position may require weekend, holiday and evening coverage as well as variable weekday assignments.

Job Summary</b></p>
The MGH Department of Pharmacy is seeking a Transitions of Care/Boarders Clinical Pharmacist to join our team at the Main Campus. This position will be focused on collaborating with multidisciplinary team members to optimize care for Boarder patients (primarily medicine patients) residing in the Emergency Department and supervise pharmacy technicians/learners participating in the daily conduct of medication histories. This is an exciting opportunity for those looking to lead, build, and expand future services to include all aspects of care transitions for patients, and optimize care for Boarder patients at our Main Campus. Clinical Pharmacists assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Clinical Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events.
