Identify and coordinate appropriate resources post discharge for patients requiring homecare, outpatient, or placement in an alternate level of care.<br>
<br>
Perform ongoing evaluation for appropriateness for the facility&rsquo;s level of care by applying knowledge of level of care criteria with active collaboration with patients insurance payer<br>
<br>
Comply with all applicable State and Federal regulations as well as other credentialing bodies, including but not limited to, Joint Commission and CARF, regarding the case management process.<br>

EDUCATION AND/OR EXPERIENCE</b></p><ul><li><p>

Two years of Case Manager experience is preferred.</p></li><li><p>
Must successfully complete a background check.</p></li><li><p>
Proficient in Microsoft Office suite.</p></li><li><p>
Ability to work independently</p></li><li><p>
Valid Driver&rsquo;s License and the ability to travel and transport.</p></li><li><p>
Strong professional ethics.</p></li><li><p>

<p>84868BR<br/></p><p>Otolaryngology and Communication Enhancement<br/></p><p>Job Posting Description<br/></p><p>Brief Overview<br/>Physician Assistant Otolaryngology<br/>Key Responsibilities:<br/>Providing comprehensive healthcare services to patients in Pediatric Otolaryngology.<br/>Obtaining comprehensive patient histories and performing physical exams for both new and return patients.<br/>Performing pre-operative, post operative and urgent ORL evaluations in the ORL clinic.<br/>Assisting with procedures both in the clinic setting and in the operating room.
