Job Summary</b></p>
The MGH Palliative Care RN Navigator (per diem) for the Inpatient Palliative Care Inpatient Consult Service serves as a central coordinator in managing referrals, patient flow, and care transitions for patients with serious or complex illness. This role is responsible for receiving inpatient palliative care consult referrals from a variety of sources, including inpatient providers, outpatient clinics, and the emergency department, and collaborating with referring clinicians to obtain additional clinical and psychosocial information necessary to appropriately triage and prioritize referrals. Prioritization is based on symptom burden, acuity of illness, goals-of-care needs, and patient/family support requirements to ensure patients with the most urgent palliative care needs receive timely intervention. The RN Navigator facilitates communication and coordination among interdisciplinary teams and specialty services to promote continuity, consistency, and alignment of care plans across the continuum. The role may also include participation in interdisciplinary huddles with Emergency Department, Palliative Care, Case Management, and other clinical teams to assess patient appropriateness for established care pathways and program protocols. The RN Navigator educates clinicians and staff on pathway criteria, supports appropriate patient entry into designated programs, and monitors patient progression through the continuum of care to ensure timely placement into the next appropriate level of care or disposition setting. In collaboration with these clinical teams the RN Navigator gathers pertinent clinical and social information to determine the most appropriate service line, level of care, and care setting within the health system, helping to optimize patient flow, resource utilization, and patient-centered outcomes.<br>

This is a per diem, hybrid position, consisting of 8-hour day shifts during the week/on weekends. There may be opportunities for remote work.<br>

The Case Manager works as part of an intensive case management team serving tenants who may be experiencing untreated mental illness, active drug and alcohol addiction, and other major health challenges. The Case Manager uses a Harm Reduction, Trauma Informed approach to working with participants, including the utilization of Stages of Change Theory and Motivational Interviewing techniques. The Case Manager is responsible for a specific caseload of tenants, and also for collaborating with other housing staff on program-wide efforts to stabilize and promote the individual growth of all PSI supported tenants. Additional case management responsibilities include but are not limited to assisting tenants with all required program documentation; assisting tenants with obtaining benefits, health care, educational opportunities, employment opportunities, and legal aid; supporting the development of tenants&rsquo; activities of daily living skills and creating individual service plans with tenants and maintaining all necessary documentation.</p><p><b>

Job Summary</b></p>
The Channing/Harvard Cohorts Biorepository (CHCB) manages sample collections and distributions for some of the largest and most intensively studied longitudinal cohorts in the world, including the Nurses' Health Studies, the Health Professionals Follow-up Study, the Growing Up Today Study, and the Physicians' Health Study I. CHCB safely stores more than 3 million research biospecimens and distributes thousands of specimens annually for research.<br>
We are hiring a full-time Technical Research Assistant I to perform laboratory and office duties associated with CHCB under the supervision of the Senior Biobank Manager, Collections Manager, and Distribution Project Manager. Responsibilities include building and shipping collection kits to study participants; picking up collection kits from receiving; logging kit information; processing biological specimens; storing samples; conducting data entry; and inspecting and processing questionnaires. This is a full-time position, Monday through Friday, 8:00 a.m. to 4:30 p.m.<br>

<ul><li>Collections: Responsible on a day-to-day basis, under the Collection Laboratory Manager, for picking up, opening, and checking incoming specimen collection kits for completeness; logging received kits and specimens into the Laboratory Information Management System (LIMS) and logbooks; processing biospecimens according to standardized SOPs; and storing samples in liquid nitrogen freezers. Assist with the design of quality control testing and perform random QC checks to ensure sample type, location, and sample IDs are accurate. Conduct data entry manually or by scanning Scantron questionnaires, and perform QC checks on the entered data. Build and ship collection kits to participants enrolled in cohort studies.</li><li>Production and Distribution (P/D): As part of a team, execute production and distribution projects by preparing the necessary computer files; pulling specimens from LN2 freezers; aliquoting specimens; adding quality control samples to projects; double-checking project materials; creating shipping lists; arranging shipments; and recording all details and steps for each project.</li><li>Data Recordation: The incumbent will be expected to become a proficient Laboratory Information Management System (LIMS) user and assist management regularly by discussing processes, improvement needs, and workflow.</li><li>Infrastructure: As part of a team, the incumbent will assist with weekly monitoring of the health of 130 liquid nitrogen freezers, LN2 piping system levels and pressure, and external bulk tank levels and deliveries. Responsibilities also include consolidating freezers when warranted, troubleshooting issues, and following SOPs to carry out freezer updates.</li><li>General: As needed, the incumbent will contribute to other general activities, such as writing Standard Operating Procedures (SOPs), maintaining inventory of supplies, and recommending items that need to be ordered.</li></ul><br>
