Assess each client addressing the barriers to self-sufficiency and employment, including personal and financial issues, emotional and physical issues, career decision making and planning issues. Develop and update case management plans, biopsychosocial assessments in collaboration with the clients and within the required time frame. Make referrals, when necessary, to mainstream agencies based on the identified needs of clients for services such as emergency assistance; health and mental health; substance abuse recovery; parenting/life skills training; career/job readiness training; financial literacy counseling; employment placement; and tenant and landlord education. Monitor the progress of clients on assigned case load and maintain weekly contact with customers via case management sessions, telephone contact, home visits, etc. Complete case progress notes and update client files accordingly. Ensure that all progress notes of engagement activities, client contacts, and clinical notations are recorded in client electronic records in the Housing the Homeless QuickBase application administered by DHS within 48 hours of service delivery. Liaise with property management companies for PSHP clients to ensure living arrangements are suitable, maintenance repairs are timely, and that clients&rsquo; needs and concerns are addressed. Maintain case files documenting all supportive services, rental payments, and referrals identified and services delivered on behalf of the client. Assist clients with housing placement and relocation. Complete all housing applications with clients ensuring that the packet is comprehensive with all required documents. Prepare various reports as required, perform research for the program, and maintain relevant files. Monitor case notes documentation to track client progress and outcomes.  Provides management, guidance, and evaluation of assigned caseload. Conducts intake interviews with newly admitted inmates in assigned area to obtain pertinent data, and to orient inmates to the institutional rules, regulations, procedures and program opportunities. Conducts Risk and Needs assessment in order to help inmates to identify goals they want to achieve; and during incarceration, help inmates connect with relevant community service providers.

 Gathers information from new inmates within 72 hours of admission in order to conduct classification to determine the custody level of the inmates according to their respective charges. Reclassifies inmates every 90 days to ensure the correct classification/custody level is maintained. Recommends programming identified for pre-release inmates that are designed to assist with readjustment to life in the community. Refers inmates to therapeutic services, including but not limited to, stress management, life skills, substance use and anger management programs, while still incarcerated.

Prepares and when required, presents reports based on observed inmate behavior, conformity to jail policies and procedures, and other evaluative outcomes. Identifies and assesses significant factors or variables in the inmate&rsquo;s history that may have contributed to criminal behavior.

 Considers and evaluates such factors as educational background, family relationships, socioeconomic factors prior to arrest history, substance abuse and where applicable, physical/mental status, etc. Develops a program designed to meet/challenge the inmate during the period of incarceration, with the goal of rehabilitation in thinking, behavior, and outlook. Makes recommendations annually regarding changes to inmate programs based on the review and reclassification of inmates assigned to caseloads.

<div><p>The <b>Readmissions Reduction Case Manager &amp; Hospital Coordinator</b> is responsible for the oversight, coordination, and administration of the Readmissions Reduction Program on-site at the hospital(s). This is an implementation-focused role, with an emphasis on relationship building with hospital staff and adapting Readmissions Reduction Program to fit in this particular hospital setting. Specifically, this work includes, but is not limited to, coordinating with hospital staff, participating in hospital rounds, and intaking potential participants. Additionally, managing the administrative/operational flow of participants into the program. Further, serves as case manager for clients with mental illness and/or substance use disorders as program needs require. The section on Essential Functions is split into (1) Program Coordination-specific functions and (2) Case Manager-specific functions.</p>

2-4 years of relevant experience in the behavioral health field providing case management, counseling, and/or crisis intervention</li><li>
Knowledge of community resources, social determinants of health, mental health and/or substance use disorders/disabilities, trauma-informed care, recovery/resiliency-oriented services, benefits, and social issues.</li><li>
