 Position Summary
 
 
 
 The Case Manager is responsible for developing a therapeutic relationship with the program participants, assess strengths and needs, creates goals, implements and monitors the case management plan. MIRIAM&rsquo;S KITCHEN MISSION
Miriam&rsquo;s Kitchen works to end chronic homelessness in Washington, DC. We advocate for permanent supportive housing as a long-term solution, while meeting short-term needs by providing healthy meals and high-quality social services to individuals who are chronically homeless.
POSITION SUMMARY
The Bridge Housing program is a new non-congregate bridge housing program expected to open in 2025. Bridge Housing&rsquo;s primary goal is to help residents exit to permanent housing within 3-6 months. Bridge Housing will serve up to 190 residents at full capacity. Bridge Housing is open 24/7 and residents primarily stay in semi-private, dormitory style rooms. Bridge Housing will serve adults of all genders. Target populations will include individuals who are currently matched to a permanent housing resource and individuals who can&rsquo;t be served well by existing low-barrier shelters. Residents need to be referred through city systems.
The Bilingual Case Manager at Miriam&rsquo;s Kitchen&rsquo;s E Street Bridge Housing Program plays a key role in supporting residents experiencing homelessness as they work toward permanent housing and improved well-being. Each Case Manager will maintain a caseload of approximately 25 residents and provide comprehensive, client-centered, and Housing First-aligned services, including housing navigation, recovery supports, benefit access, and other individualized care. Services will be guided by collaboratively developed case plans that center residents' strengths, goals, and the primary outcome of exiting to stable housing. The Case Manager is also responsible for timely and accurate documentation in accordance with contractual and agency standards. Our bilingual case manager will work with both English- and Spanish-speaking clients to support them in the Bridge Housing program.
This is an in-person, on-site position.
The position is Monday-Friday primarily during business hours, but there will be a rotating (weekly) &lsquo;late day&rsquo; for each case management team member including supervisors (eg 11am-7pm or 12pm-8pm one day per week).
DUTIES AND RESPONSIBILITIES<ul><li>
Deliver client-centered, trauma-informed case management grounded in Housing First principles, with the goal of helping residents exit to permanent, stable housing.</li><li>
Maintain a caseload of approximately 25 residents, providing consistent, individualized support to meet their goals.</li><li>
Conduct intake and needs assessments using tools such as the VI-SPDAT, SPDAT, and benefit eligibility screenings per contract and agency guidelines.</li><li>
Develop and implement individualized case plans with residents, addressing goals related to housing, health, mental health, substance use recovery, income, and social supports.</li><li>
Provide regular case management engagements and follow-up to ensure case plan progress and effective coordination of services.</li><li>
Assist residents in gathering and securing required documentation, including IDs, income verification, and medical records, to support service enrollment and housing readiness.</li><li>
Complete timely, accurate documentation of all services, assessments, and client interactions in required databases and formats.</li><li>
Facilitate referrals and warm handoffs to both internal and external providers, including medical, behavioral health, legal, employment, and housing programs.</li><li>
Collaborate with any on-site and community partners to ensure continuity of care and wraparound support for residents.</li><li>
Monitor and respond to residents&rsquo; physical and mental health needs; provide crisis de-escalation and emergency response when necessary.</li><li>
Draft participant notices and other correspondence related to program expectations, policy violations, exits, or extensions, in alignment with contract requirements.</li><li>
Actively engage in multidisciplinary team meetings, case conferences, and supervisions to ensure holistic, team-based service delivery.</li><li>
Attend all required training and development sessions as outlined by Miriam&rsquo;s Kitchen and contract mandates.</li><li>
Contribute to program development efforts and new initiatives that enhance resident services and outcomes.</li><li>

Familiarity with the homeless services and housing landscape in Washington, DC.</li><li>
Knowledge, experience, and ability to connect with individuals experiencing mental health issues and/or substance abuse.</li><li>

Requirements:

Experience and Education<ul><li>
At least 1 year of experience working in services for homelessness, mental health, substance abuse, HIV/AIDS, and/or case management for vulnerable populations.</li><li>
A minimum of an Associate&rsquo;s degree or two years of college. In lieu of the education requirement, the case manager may have a minimum two years of experience with homeless services programs or provision of social services. Both Associate&rsquo;s degree/college and work experience will be waived if an individual has received a Peer Case Management Certification through DHS&rsquo;s Peer Case Management Institute.</li><li>
Specialized experience, certification, or training in relevant fields will be strongly preferred (eg mental health, substance use, SOAR, etc).</li></ul>

Join our team as a Social Service Specialist 2 within the Home and Community Living Administration (HCLA), Home and Community Services (HCS) Division, working with Medicaid applicants residing in their own homes seeking in-home services to maintain their independence in that setting. This population are high-risk, frail, vulnerable persons aged 18 years and older who are medically and financially dependent on services from HCS.
There are 6 project positions that will be funded until June 30, 2028 and one permanent Social Service Specialist 2 Intake position.
For the permanent position, you will receive and process intake requests through multiple channels, support disability and Supplemental Security Income facilitation and help connect clients to the full range of HCS programs.
We are looking for multiple positions statewide throughout Washington State.
This position works effectively and collaboratively with our community agency and residential partners, fostering good communication and demonstrating reliability.
The Presumptive Eligibility (PE) case manager position conducts PE assessments telephonically using a laptop computerized system in CARE to determine functionally eligibility for applicants seeking PE.
Case manager will provide case management tasks until such time case can be transferred to an in-home worker for a full functional assessment.
This position requires excellent organizational skills to manage a large volume of new referrals and provide person centered care planning.
Will back up Intake Team as needed.
Some of what you'll do:<ul><li>
Conduct functional assessments using the department&rsquo;s Comprehensive Assessment Reporting and Evaluation (CARE Web) to determine level of care and eligibility for the PE along with doing a financial pre-screen for financial eligibility using client self-attestation</li><li>
The assessment will occur telephonically. The PE assessment will be completed meeting the quality standards as set forth in the Long-Term Care Manual</li><li>
Serve as the primary point of contact for Intake, providing front-door services by phone, email, fax and in person and determining appropriate Information and Referral (I&amp;R) or DSHS program pathways</li><li>
Develop appropriate individualized service plans in conjunction with the client and/or representative to meet the needs of the client</li><li>
Authorize appropriate services for Medicaid clients utilizing qualified providers if the client chooses to receive their long-term care services in an in-home setting</li><li>
Provide skilled case management services to resolve issues, concerns, and diversional activities to prevent hospitalization or nursing facility placement</li><li>
Provide case management tasks until case can be transferred to an in-home worker for a full functional assessment</li><li>
Monitor progress of referrals, address any barriers and confirm the start of services</li><li>
Conduct intake interviews and initial risk assessment with clients or collateral contacts by phone or in-person to evaluate the client care needs and make a referral to the appropriate Home and Community services program and/or other appropriate services</li><li>
Coordinate and provide financial support services including assisting clients to fill out forms and application for HCS benefits, Non-Grant Medical Assistance (NGMA)</li><li>
Compute client&rsquo;s estimated responsibility when appropriate</li><li>
Prioritize caseload according to office policy, timeframes, and workload expectations</li><li>
Organize, handle, and submit documents per program procedures and office specifications: ensure documents and forms are complete, accurate and up to date</li><li>
Participate in unit meetings, staffing with collateral contacts and community partners, mandatory on-line or in-person training, workshops, webinars, teleconferences, video conferences and meetings</li></ul>

Triage referrals and client care/management</li><li>
Link clients and family to agency and community resources</li><li>
Resolve complaints between clients and providers</li><li>
Identify and evaluate social dynamics affecting client choice and decision-making</li><li>
Effectively interview clients with or without an interpreter</li><li>
Demonstrate flexibility by adapting to change, respecting differences in opinion and values</li><li>
Use Microsoft Office and other computer applications for research, scheduling, correspondence, and documentation</li></ul> 
Who should apply? 
We are seeking professionals who meet one of the following qualification options:<ul><li>
Twelve months of experience as a Social Service Specialist 1 AND successful completion of the agency&rsquo;s Social Service Specialist Training Program 
OR</li><li>
A master&rsquo;s degree in social services, human services, behavioral sciences, criminal law or justice, or a closely related field AND one year of experience as a Social Service Specialist 1 or equivalent paid social service experience 
OR</li><li>
A bachelor&rsquo;s degree in social services, human services, behavioral sciences, criminal law or justice, or a closely related field AND two years of paid social service experience performing duties equivalent to a Social Service Specialist 1</li><li>
NOTE: A two-year master&rsquo;s degree in one of the fields listed above that included a practicum may substitute for one year of paid social service experience</li><li>
NOTE: Employees must successfully complete the formal training program within eighteen months of appointment</li><li>
NOTE: Equivalent education and experience totaling six or more years may be considered while performing functions equivalent to a Social Service Specialist 1</li></ul>
