JOB SUMMARY: The Clinical Care Coordinator provides assessment of consumers&rsquo; functioning and leads development and documentation of individualized rehabilitation plans based on the Recovery Model for adults who have mental illnesses.
ESSENTIAL DUTIES and RESPONSIBILITIES:<ul><li>
Provide services to consumers in accordance with the principles and practices of the ACT Fidelity Model.</li></ul>
 a. Lead the process of developing, updating, and guiding implementation of consumers&rsquo; Individual Recovery Plan (IRP), in collaboration with consumer, Team Leader and other significant parties of the ACT team.
 b. Coordinate treatment planning and discharge meetings at area hospitals to ensure a smooth transition to the community.
 c. Develop and maintain a professional and clinical relationship with area hospital staff to ensure a smooth transition for agency consumers to and from the hospital.
 d. Educate and assist consumers with learning or re-learning activities of daily living (ADL) skills necessary to improve the quality of life including acquisition of consumer health care benefits.
 e. Update Consumer&rsquo;s IRP, Functional Psychosocial Assessment (FPSA), Level of Care Utilization System (LOCUS), Global Appraisal of Individual Needs (GAINS) and Crisis Plan every 180 days.
 f. Provide an average of 4.25 hours of services per day to ACT consumers.
 g. Establish constructive relationships with consumers to promote acquisition of rehabilitative goals.
 h. Maintain adherence to all policies and procedures consistent with Dept. of Mental health (DMH) and the District of Columbia board of social work.<ul><li>
Participate in treatment team meetings and conduct follows ups as indicated in the following areas.</li></ul>
 a. Crisis assessment and intervention.
 b. Symptom assessment, management and individual supportive therapy.
 c. Assistance in the treatment of substance abuse for consumers with co-occurring addictive disorders.
 d. Psychosocial rehabilitation and skill development.
 e. Recommend and sometimes make decisions about the best course of action for a particular service user.
 f. Provide support services to ACT team and consumers.
 g. Develop IRP and other care plans as indicated. Conduct needs and risk assessment.
 i. Provide resolution and referrals services for consumers.
 j. Conduct psychosocial assessments using the Functional Psychosocial Assessment (FPSA) every 180 days for consumers served by the ACT team
 k. Education, support and consultation to consumers&rsquo; families and/or their support system, which is directed exclusively to the well-being and benefit of the consumer.<ul><li>

In the role of Construction Coordinator/EIT, we'll count on you to:<ul><li>
Assist leadership in processing paperwork to expedite construction activities.</li><li>
Gain exposure to standard construction features to understand conformance with contract documents.</li><li>
Gain exposure to project schedules, pay requests, progress reports, and cost/budget reports.</li><li>
Assist with project field coordination, documentation, cost monitoring, bidding, and scheduling.</li><li>
Receive and track RFIs, submittals and other required data for CM.</li><li>
Perform other duties including on-site inspection assistance, document control, monitoring daily logs, and status reports as needed.</li></ul></div><div><div>
Preferred Qualifications

The Operations Coordinator serves as a key link between the Customer Service Center (CSC), internal teams, and partner airlines. Main Accountabilities</h2></div><div><ul><li>
Act as the primary liaison between internal departments and airline partners to address missing or requested items promptly.</li><li>
Monitor and maintain multiple operational platforms (e.g., Ground Star&mdash;Flight Family, CBASE, etc.) for both LSG and customers.</li><li>
Support department supervisors with daily manpower planning and employee scheduling.</li><li>
Adjust delivery and backhaul assignments to accommodate unforeseen adjustments to flight plans</li><li>
Oversee daily inventory management for all operational equipment and ensure timely completion of checklists.</li><li>
Assist in preparing operational data analyses, reports, and performance summaries.</li><li>
Coordinate with controllers and supervisors to manage flight assignment updates and ensure accurate team allocation.</li><li>
Collaborate closely with Customer Service, Operations, and other internal teams to ensure excellent service delivery and customer satisfaction.</li><li>
Ensure all equipment changes are properly staged and meet service-ready deadlines.</li><li>
Oversee and support the loading of flights and equipment, assisting teams under tight timelines.</li><li>
