The APP will work collaboratively with an interdisciplinary team including faculty, fellows, residents, and nursing staff to develop, implement, and coordinate individualized plans of care informed by current clinical guidelines, best practices, and patient-centered goals.
</p><p>In addition to clinical care, the APP will play an active role in supporting a culture of education, peer mentoring, and professional development, helping to coach and guide learners, and contribute to ongoing quality and clinical excellence initiatives within the ICU environment.
</p><p>This role is within the Department of Anesthesia in Critical Care, providing coverage across both the Neuro ICU and Medical ICU. Off-hour, weekend, and holiday coverage is required.</p>

We are expanding our Advanced Practice Provider team in a state of the art 44 bed Cardiac Intensive Care Unit in a new Heart Center here at Boston Children&rsquo;s Hospital. We are looking for diverse and motivated individuals to join our team here in Boston. When you join our team you will work with a supportive group of Advanced Practice Providers, Attending Physicians and Fellows. You will be responsible for taking care of Pre and Post-operative highly acute cardiac patients, along with children who have cardiac medical needs in the intensive care unit. We have an extensive Orientation Program to prepare you for taking care of these critically ill patients and to thrive here at Boston Children&rsquo;s. <b>Key Responsibilities:</b><ul><li>
Providing care to infants, children, and adults with congenital and acquired heart disease.</li><li>
Caring for patients with both acute and chronic cardiac disease.</li><li>
Collaborating with the CICU Attending physician, to conduct patient assessment, medical history, physical exams, and diagnose health and developmental problems.</li><li>
Participating in CICU medical rounds, including presentation of clinical information, plan development and continued management and modification of the plan for patients throughout the shift.</li><li>
Pre and Post-Op care of critically ill cardiac patients that involves management of ventilators, inotropic support, ECMO, CVVH, emergency situations and sedation titration.</li><li>
Procedures such as Intubation and Central lines, will be taught in both Simulation and during real-time, hands-on events.</li><li>
Extensive Orientation Program with a designated preceptor, simulation and didactic classes along with hands-on experience. Access to in-person and Zoom lectures by Heart Center experts as well as electronic recordings on OpenPediatrics.</li><li>

NURSE PRACTITIONER MS - PER DIEM Rotating Shift - Cardiac Electrophysiology<br>

About this job:<br>
The Cardiac Electrophysiology (EP) Nurse Practitioner (NP) will practice in three settings: inpatient, outpatient/ambulatory, and procedural settings. The NP will be involved in all aspects of care with the primary goal of ensuring the effective delivery of care to patients.<br>
<br>
Inpatient and peri-procedural care<br>
<ul><li>The Nurse Practitioner will be responsible for obtaining and coordinating the following general items: history and physical, laboratory results, charting of clinical findings, registry documentation, orders, sedation assessment, nursing instructions, consent, and other required elements. The NP, in collaboration with the Laboratory Flow Managers, may be asked to help coordinate admissions and transfers to the hospital. The NP will perform pre-procedure assessments in the recovery room/holding area or outpatient setting as needed.</li><li>The Nurse Practitioner (NP) will be knowledgeable about the Cardiac Electrophysiology Laboratory scheduling, procedure room turnover and availability of resources. They will be called upon to manage patients after their procedures (as appropriate and in collaboration with the consult Fellow) while they await transfer to patient floor or discharge. The NP will assist in family liaison.</li><li>Following procedures, the Nurse Practitioner (NP) will manage vascular access, monitor and manage arrhythmias, pacing, drug therapy, O2 therapy, and other non-cardiac medications, under the supervision of the attending physician. The NP will assist in the evaluation and management of clinical events such as fever, altered hemodynamics, CHF, pulmonary complications, abrupt vessel closure, arrhythmias, access site complications and others. The NP will assist in transfers to and from the intensive care units.</li><li>The Nurse Practitioner (NP) will perform patient education regarding discharge including preventive care and risk factor reduction. They will write discharge orders, prescriptions and complete discharge summaries in collaboration with the attending physician. The NP will act as a liaison between the attending cardiologist and family members and will schedule follow-up care as necessary, including VNA if needed.</li></ul><br>
Ambulatory care<br>
<ul><li>The Nurse Practitioner (NP) will collaborate with the team in preparing patients for admissions/procedures. Educate patients regarding a variety of electrophysiology treatments and procedures. Works collaboratively with nursing/clinic staff to assist in the care of patients. This includes, but is not limited to: symptom management, evaluation and management, prescription writing, ordering and interpreting diagnostic studies with review by attending MD, patient calls and questions related to electrophysiology clinical care. The NP will provide care via in person clinic visits, virtual clinic visits, and telehealth.</li></ul>
Procedural care<br>
<ul><li>The Nurse Practitioner (NP) will be responsible for many aspects of care under the supervision of the attending physician, including but not limited to: cardioversions, suturing, cardiac subcutaneous monitor implant/explant, first assisting during diagnostic EP studies/ablations, device implantation and research procedures performed in the EP laboratory, OR and other clinical areas, as well as fluid and electrolyte management, nutrition, pain management, transfusion therapy, anticoagulation management, education, and post-discharge planning.</li></ul><br>
General/Working Conditions<br>
<ul><li>The Nurse Practitioner (NP) participates in departmental and organizational committees and programs as appropriate and attends all required meetings, in-services, and professional trainings. Remains actively involved in continuing education with a commitment to self and departmental growth. Identifies areas for professional growth. Formulates professional goals, objectives and methods for accomplishing these. Supports initiatives within BWH, the Department of Medicine and the Cardiovascular Division.</li><li>Work settings may include ambulatory clinics and treatment rooms, inpatient areas, intensive care units, operating rooms, and/or procedural areas. There may be loud noises, noxious odors, and temperature extremes. A large portion of the workday may be spent either standing or walking and the position may require heavy lifting of equipment and patients.</li></ul>

<ul><li>The Nurse Practitioner (NP) may be exposed to radiation, potentially infectious bodily fluids, and sharp instruments such as needles and scalpels. Knowledge of Universal Precautions and proper use of surgical sharp instruments may be required. As a member of the Acute Psychiatry Service (APS) Provider Team, the Advanced Practice Provider will assume the role of responding clinician in the assessment and care of patients with psychiatric and substance use disorders, including diagnosis, treatment, education, and management of the patient, in particular the patient awaiting admission to a psychiatric inpatient facility. The patient may be evaluated or seen in follow-up while in the APS, the main ED or an associated observation/ boarding unit.<br>

</li><li>Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
</li><li>Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
</li><li>Performs bedside procedures as are appropriate to the patient population.
