 The Case Manager is a non-exempt position responsible for direct case management services to children, birth families, foster families and primary caregiver(s) to ensure they receive services appropriate to their needs and continuity of care focused on resolving or preventing child abuse or neglect. Purpose &amp; Impact:

The purpose of the Child Welfare Case Manager is to provide full wrap around case management services to LSF clients and families while establishing a relationship with them and living the agency&rsquo;s core values.

Essential Functions:
<ul>
<li>Accept all cases assigned by unit supervisor and meet with children in their primary residence within 2 working days of ITR staffing and with identified parents in sufficient time to complete a Family Assessment within 15 working days of ITR staffing.</li>
<li>Negotiate and develop a case plan based on identified strengths and needs of the family, the circumstances bringing the family into care, recommendations provided by the Comprehensive Behavioral Health Assessment and other relevant data. This plan is to be created in collaboration with the parents, Guardian Ad Litem, foster parents and other pertinent parties with the goal of reunification or other permanency for the child.</li>
<li>Visits the child in their primary residence according to required frequency, but no less than once every 25 days. At least one visit each quarter is to be unannounced.</li>
<li>Accurately document all case activities in the Florida Safe Families Network database within 48 hours. Maintain the physical case file in chronological order, by subject, in accordance with the standardized case file format.</li>
<li>Submit service requests to the lead agency Utilization Management Department and provide clients with timely referrals to services. Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources. Maintain regular contact with service providers and document service progress in FSFN.</li>
<li>Complete and submit court documentation within in required time frames. Prepare for, attend, and participate in all court activities as necessary.</li>
<li>Arrange for, attend, and participate in individual case staffing as necessary. Complete all required staffing and application packets.</li>
<li>Conduct initial and/or ongoing child safety assessments as required. Prepare initial and on-going safety plans as necessary. 
Arrange for emergency placement, emergency medical treatment, and emergency services for children at risk.</li>
<li>Conduct diligent searches for parents and family members when deemed necessary and thoroughly document that the effort has been made to find the parents and family members.</li>
<li>Conduct home studies as required for prospective placements. Provide relevant medical, psychological, behavioral and educational background information about the child or children to prospective caregivers as needed. Plan and facilitate parental and sibling visits as needed and appropriate.</li>
<li>Transport children as needed.</li>
<li>Ensure that all Independent Living functions are completed as required.</li>
<li>Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by the supervisor.</li>
<li>Function as agency on-call Case Manager as scheduled.</li>
<li>Works cooperatively with Program Directors, with other Case Managers, with placement staff, with Protective Investigators, with Child Welfare Legal staff and with agency support staff.</li>
<li>Effectively manages time to ensure that all home visits are completed as required, all documentation is entered into FSFN within 48 hours, court documentation is prepared according to specified time frames and court appearances are attended as necessary.</li>
<li>Follows Florida Statutes, Administrative Code, written policies and orders of the Dependency Court in managing cases toward goals recorded in case plans.</li>
<li>Organize, prioritize and complete all work assignments by the established deadlines. 
All duties are performed in accordance with the following standards:

<li>Communication Skills: Keep supervisor fully informed of activities, pertinent issues, upcoming events and potential problems. Demonstrate effective oral and written communication skills in daily work.</li>

Qualifications

Physical Requirements:

Manage the daily operations of the Intensive Family Services Dependency Case Management (IFST) staff, which serve children and families through early engagement intensive case management, while partnering with the Department of Children and Families protective investigators. Review and monitor case files for both internal and external quality assurance standards</li><li>
Supervise staff including regular individual and group staff meetings and case consultation</li><li>

Working Hours
Children&rsquo;s Network of Hillsborough&rsquo;s standard business hours are 8:00 a.m. to 5:00 p.m., Monday through Friday. This position is an in-office based position with fieldwork requirements, that averages 40 hours per week. Work hours may vary outside of these standard hours, including evenings and weekends, as needed to meet operational demands. All hours worked will be compensated in accordance with the Fair Labor Standards Act (FLSA) and agency policies, including applicable overtime and other compensation requirements.
Travel
Travel within and outside of Hillsborough County is required. The employee must be able to drive and have reliable transportation to perform the essential functions of the position.
Training
All employees are required to complete fifteen (15) hours of training annually, which may be completed online or in a classroom setting. Employees who hold a credential or license relevant to their position must ensure that all training and continuing education requirements are met to maintain their credential or licensure.
Supervisory Responsibilities
This position is responsible for providing direct supervision of the Child Welfare Case Management staff.
Education and Experience<ul><li>
Bachelor&rsquo;s degree from an accredited college or university in Human Services or another related field; and</li><li>
A minimum of two (2) years of child welfare related experience</li><li>
Current Child Welfare Certification or eligibility upon hire</li><li>
Prior supervisory experience preferred</li></ul>

The Case Manager is responsible for all customer service functions and quality control of the final investigative product, also serving as the liaison between the client and the field staff who conduct the investigation. Case Managers are also responsible for meeting &ldquo;reopen&quot; sales goals on a monthly basis, creating accurate funding records for all items billable to the client, and sound management of their assigned case inventory. Case Managers must utilize discretion and independent judgment to resolve conflicts between clients and/or investigators. 
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:<ul><li>
Ensure timely closing of all files on which the client requires no additional investigative activity. Upon closure of files that have unused authorization monies, a detailed description of the reasons for this unused authorization must be provided</li>
<li>Use independent judgment to define and confirm client objectives (including due dates) on all new, reopen, and investigative referral cases; objectives must be clear and completely defined in the &ldquo;Client objectives&quot; section of CAM (proprietary business software)</li>
<li>Use sound management of their assigned case inventory utilizing discretion and independent judgment to resolve conflicts between clients and/or investigators; respond to customer concerns (negotiating as needed) and forward field operation issues to appropriate Filed Manager as needed</li></ul>
