Continually assesses claim status to determine if problem cases or those exceeding protocols should be referred to Claims Service Team and/or would benefit from, MP RN review or other medical /claims resources. Arranges Independent Medical Exam and Peer Review as necessary.</li><li>
Maintains accurate records and handles administrative responsibilities associated with processing and payment of claims. Records and updates status notes; documents results of contacts, relevant medical reports, and duration information per file posting standards including making appropriate medical information viewable to customers in Electronic Document Management (EDM). Generates form letters following set guidelines (i.e., letters to physicians projecting disability, letters confirming medical treatment and disability and letters outlining expected outcome to employers).</li><li>
Authorizes payment of medical payments and/or medical treatment.</li><li>
Recognizes potential subrogation cases, prepares cases for subrogation and refers these cases to the Subrogation Units.</li></ul></div>
Qualifications<div><ul><li>

Purpose &amp; Impact:

The purpose of the Child Welfare Case Manager is to provide full wrap around case management services to LSF clients and families while establishing a relationship with them and living the agency&rsquo;s core values.

Essential Functions:
<ul>
<li>Accept all cases assigned by unit supervisor and meet with children in their primary residence within 2 working days of ITR staffing and with identified parents in sufficient time to complete a Family Assessment within 15 working days of ITR staffing.</li>
<li>Negotiate and develop a case plan based on identified strengths and needs of the family, the circumstances bringing the family into care, recommendations provided by the Comprehensive Behavioral Health Assessment and other relevant data. This plan is to be created in collaboration with the parents, Guardian Ad Litem, foster parents and other pertinent parties with the goal of reunification or other permanency for the child.</li>
<li>Visits the child in their primary residence according to required frequency, but no less than once every 25 days. At least one visit each quarter is to be unannounced.</li>
<li>Accurately document all case activities in the Florida Safe Families Network database within 48 hours. Maintain the physical case file in chronological order, by subject, in accordance with the standardized case file format.</li>
<li>Submit service requests to the lead agency Utilization Management Department and provide clients with timely referrals to services. Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources. Maintain regular contact with service providers and document service progress in FSFN.</li>
<li>Complete and submit court documentation within in required time frames. Prepare for, attend, and participate in all court activities as necessary.</li>

<li>Works cooperatively with Program Directors, with other Case Managers, with placement staff, with Protective Investigators, with Child Welfare Legal staff and with agency support staff.</li>
<li>Effectively manages time to ensure that all home visits are completed as required, all documentation is entered into FSFN within 48 hours, court documentation is prepared according to specified time frames and court appearances are attended as necessary.</li>
<li>Follows Florida Statutes, Administrative Code, written policies and orders of the Dependency Court in managing cases toward goals recorded in case plans.</li>
<li>Organize, prioritize and complete all work assignments by the established deadlines.

MAPP Training, In-service, etc., within the probationary period and as scheduled thereafter. The successful completion of the Field-Based Performance Assessment is an essential performance standard required for continued employment in the class of Case Manager.</li>

Qualifications

Physical Requirements:

<li>Valid Florida Driver&rsquo;s License and Insurance</li>
<li>Ability to travel locally, and out of the area, in the execution of professional duties, trainings and/or conferences.</li>
<li>Ability to operate a computer, sit for long periods of time, and develop coherent written correspondence and progress notes.</li>
<li>Ability to adapt to irregular hours, perform some light lifting, and be flexible to rotate on call, as needed.</li>
<li>Certified First Aid/CPR, and crisis intervention.</li>

Education/Experience
<ul>
<li>Must have a minimum of one year of relevant experience and certified or become certified within one year.</li>
<li>Must possess a bachelor&rsquo;s degree in a Human Services field. Degree in Social Work preferred.</li>

<li>Excellent written and verbal communication skills.</li>
<li>Familiarity with and ability to use Microsoft Office programs Word and Excel.</li>
<li>Ability to drive both locally and throughout the state in connection with the duties of this position.</li>
<li>To fully understand case ownership responsibility as the integrator of all services and supports identified for each child, including therapy, other mental health services, health and dentistry, developmental services, educational support, permanency and safety; as well as their responsibility to make trauma sensitive transitions when it is determined that a caretaker lacks the needed level of responsibility to care for their children.</li>
<li>Must demonstrate sensitivity to our service population&rsquo;s cultural and socioeconomic characteristics and needs.</li>

Case Managers are responsible for providing direct case management services to children, birth families, foster families and primary caregiver(s) in the home setting, day care, preschool, and/or school to ensure they receive services appropriate to their needs, and continuity of care focused on resolving or preventing the issues of child abuse or neglect. The Case Manager provides assessment, planning, linkage, coordination, monitoring, evaluation and advocacy to the child, family, foster family and primary caregiver. The Case Manager works as a team member with a therapist, foster parents, primary caregiver, guardian ad litem and other appropriate professionals. The Case Manager will develop, strengthen and maintain a direct working relationship with mental health professionals, community agencies and the child&rsquo;s own support system 
 
Position Responsibilities Include:</div>
<ul><li>Arranges visitation schedule structured upon the completed visitation plan with input from the appropriate parties, supervise visitations and document observations as appropriate</li>
<li>Assists in the provision of appropriate information when applicable and accompanies children to appointments, as necessary, to report progress or lack thereof</li>
<li>Assists the primary caregiver(s) and/or foster family in implementing tasks and activities according to the case plan</li>
<li>Attends and participates in professional development training for professional growth as developed by CBC/Pre Service Training, as required by the state of Florida and in accordance with agency policy and procedures</li>
<li>Identifies strengths, limitations, risk factors, and needs, to refer and coordinate the provision of concrete services to the child, primary caregiver, and biological families of children</li>
<li>Serve as a conduit of information between all parties to the case and identified stakeholders, including medical, educational and mental health providers.</li>
<li>Conduct visits with children, parents and/or caregivers that include the on-going assessment of child safety, permanency and well-being.</li>
<li>Develop and implement a plan that meets the needs of the children and the family.</li>
<li>Facilitate placement and delivery of services in collaboration with biological, foster and adoptive families.</li>
<li>Perform case management responsibilities in accordance with state and federal laws, Department of Children and Families Administrative Codes and Operating Procedures, funding source policies and internal policies on child abuse, abandonment, and neglect within required timeframes.</li>
<li>Attend court hearings, prepare and file all court documents within required time frames.</li>
<li>Intervenes and assists OHU/CBC staff in crisis situations</li>
<li>Meet with families in their homes at various hours of the day including evenings, nights and weekends, in potentially high risk neighborhoods.</li>
