Role Description
The purpose of a Medical Scribe at Oak Street Health is to support our primary care providers with clinical documentation so that they can focus on providing exceptional care to our patients. Scribes assist providers throughout the patient care journey - huddling each morning to plan for the day's visits, joining them in the exam room to observe and document, and touching base after the visit to assist with next steps.
Beyond the typical Scribe role, these important care team members serve as clinical documentation assistants to their paired provider. Internally, we call them CISs (Clinic Informatics Specialists) in recognition of their important role in supporting accurate, specific, and timely clinical documentation. In addition to observing and documenting all patient encounters in real time, our Scribes become experts in our value-based care model and the documentation and care of chronic conditions, including ICD-10 and CPT coding. Scribes use this expertise to help providers identify and help close care gaps. Scribes receive extensive on-the-job training in clinical workflows, value-based medicine, preventative care for chronic conditions, accurate and specific documentation, population health data streams, and team based care.
Because our patients and providers rely on our Scribes, the ideal candidate should commit at least 1-2 years to this role.  Medical Assistants are an integral part of delivering kind and compassionate care and optimizing patient experience. They direct patient flow in and out of exam rooms and provide support to providers by assisting in medical procedures and examinations.
 
 
 
 Essential Functions
 
 
 
 <ul>
 <li>
 Work alongside providers in the exam room to provide care.
 </li>
 <li>
 Measure and record vital signs, height, and weight for patients.
 </li>
 <li>
 Obtain chief complaint and gather medical history information from patient prior to being seen by provider.
 </li>
 <li>
 Perform basic clinical duties and procedures, including, but not limited to, blood draws, vital signs, preparing vaccines, recording EKGs, glucose monitoring, collecting urine samples, influenza testing, pregnancy tests, vision/hearing tests, etc.
 </li>
 <li>
 Prepare exam room for patient visit ensuring that medical supplies are well stocked.
 </li>
 <li>
 Clean and sterilize instruments.
 </li>
 <li>
 Perform duties to help improve quality measures which may include identifying care gaps, calling patients for follow up, documentation, etc.
 </li>
 <li>
 Help maintain clinic compliance &amp; protocols, such as freezer setting, biohazards, sharps, cleaning, infection protocols, etc.
 </li>
 <li>
 Other related duties as assigned by office manager.

 Qualifications
 
 
 
 <ul>
 <li>
 Bilingual in English and Chinese required.
 </li>
 <li>
 Phlebotomist certification preferred.
 </li>
 <li>
 High level of professionalism and strong sense of responsibility.
 </li>
 <li>
 Good communication skills: kind, compassionate, patient interaction with patients and team
 </li>
 <li>
 Multitasking ability.
 </li>
 <li>
 Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
 </li>
 <li>
 Prior experience working in medical/clinical setting preferred.
 </li>
 <li>
 Familiarity with EMR systems a plus.

Ensures medical record is accurate, complete and meets medical coding requirements. Assists in coordinating organization of care to meet patient needs.

Job Responsibility
<ul><li>Collaborates with members of the health care team in coordinating and implementing plans for patient; may record reason for visit and related patient information for practitioner review.</li><li>Enters &amp; documents information from the patient visit into the patient's electronic medical record or written chart</li><li>Performs vital signs, height, weight, and screening procedures which may include, but are not limited to, head circumference, audiometry, and vision testing; documents findings in the medical record.</li><li>Collects and assists providers in collection of specimens, as required, using appropriate protocols such as infection control; completes requisitions, labels and routes specimens according to standard procedure.</li><li>Performs and documents daily controls and calibration of laboratory equipment when needed; informs the supervisor of when inaccurate controls are noted.</li><li>Performs electrocardiograms and phlebotomy in accordance with provider orders; adheres to infection control standards including, but not limited to, hand hygiene, use of PPE, isolation of patient with known infectious disease, and equipment disinfection/sterilization.</li><li>Assists physicians and nurses with special procedures and examinations including, but not limited to lumbar puncture, pelvic exam, etc. following appropriate protocols such as infection control, universal protocol, and patient identification; secures supplies, ensures patient is properly undressed and draped, provides instruments to physicians, holds patient, as required and assists patient after procedure/examination.</li><li>Maintains orderliness and cleanliness of examination rooms; stocks assigned rooms and cupboards and ensures par levels of supplies are maintained; checks and completes required logs and equipment including refrigerator logs, eyewash logs, sample medications, AED, suction and other required checks.</li><li>May participate in pre-visit chart preparation by ensuring required lab results and documentation are available in the medical record for practitioner review (for practices using paper medical records).</li></ul>
Performs variety of other clerical functions such as filing of paper medical records, processing insurance forms, preparing financial records, photocopying, answering phones, and assuring efficient flow of patients through the suite, etc.

Job Qualification
<ul><li>High School Diploma or GED, required
</li></ul>&bull; Successful completion of a six-month (6) to one (1) year course in Medical Assisting at an approved school, required. &bull; Working knowledge of medical terminology, CPT and ICD-9 coding, required
