</b><br>
The Case Manager will be responsible for providing residential and long term support services to homeless and at-risk adults transitioning into permanent housing. The Case Manager will carry a caseload of approximately 20 to 25 formerly homeless and/or low income individuals.<br>
Case Manager will be required to have frequent contact with tenants, weekly and/or monthly face-to-face meetings, telephone conversations, and aftercare contacts. Frequent electronic exchanges with colleagues is expected <b>.

<b>Responsibilities:
</b><br>
<ul><li>Provide direct case management services (on-site and in the field). Case management services will include but not limited to mental health and substance use assessments, developing and updating service plans, assistance with obtaining public assistance, Social Security benefits/entitlements and other publicly available resources that may be beneficial to the tenant.</li><li>Using a person centered approach, provide case management services and assessments rooted in evidence based models such as Motivational Interviewing, Critical Time Intervention and Harm Reduction.</li><li>Utilize electronic health record. Maintain accurate and up-to-date tenant files/charts.</li><li>Coordinate and document clinical case management and psychosocial services while also documenting the overall effectiveness of the case management services provided. Case Managers will develop an Individual Service Plan and review the plan periodically to document process toward goals.</li><li>Serve as a liaison between tenant and community providers.</li><li>Facilitate groups with a focus on life skills, education and support, relapse prevention and mental health counseling.</li><li>Complete referrals to medical, mental health, and other outside providers.</li><li>Collaborate with Workforce Opportunities Staff to assist tenant in developing employment related goals, where applicable.</li><li>Ensure overall compliance with organizational and funder regulations. Adhere to documentation requirements as assigned by leadership and funder regulation/standard.</li><li>Coordinate aftercare services and follow-up contacts for post-discharge updates.</li><li>Conduct outreach and engagement including apartment visits and inspections.</li><li>Participate in community development efforts such as Community Meetings and supporting the residence in establishing partnerships with other providers in the community.</li><li>Work with Property Management staff to assist tenant to meet terms of their lease including timely payment of rent and avoidance of any nuisance behaviors. Mediate landlord-tenant disputes that threaten housing stability.</li><li>Utilize trauma informed interventions and culturally responsive engagement techniques to assist tenants with overcoming barriers to housing stability.</li><li>Participate in community initiatives to fulfill mission to end homelessness.</li><li>Provide crisis intervention as needed.</li><li>Provide all services in a manner that respects cultural differences amongst tenants.</li><li>Carry out other duties as assigned by leadership.<b>

</p><p><b>POSITION SUMMARY</b></p><p>
The Transitional Housing Case Manager will provide comprehensive case management services to LGBTQ+ homeless youth ages 21-24. This role delivers holistic, trauma-informed, culturally responsive care by assessing ongoing client needs, developing Individualized Service Plans, coordinating with internal and external resources, connecting clients to necessary resources and long term housing to prepare them for independent living.</p><p>

The Jewish Board’s Adult Residential Division supports adults with serious and persistent mental illness to live in the community with as much independence as possible. Using a non-judgmental, harm-reduction approach, staff model and guide residents to create and meet personalized recovery goals, develop independent living skills, and live in a healthy and secure manner. The Case Manager is the most senior level of direct care staff working within the Jewish Board’s supportive housing programs to help residents live in the community as independently as possible.

Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.

But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.

Reporting to the Assistant Program Director, Transitional Housing the Clinical Coordinator, Transitional Housing supervises Case Managers and ensures the utilization of Harm Reduction and Housing First strategies and other best-practice clinical interventions to ensure the highest quality care for clients. The Clinical Coordinator is responsible for client records are kept in compliance with regulatory and agency standards.
<br>The Clinical Coordinator works primarily during business hours Monday-Friday, with off hours availability in case of emergency or for special projects/initiatives.
<br><br>
<b>ESSENTIAL DUTIES AND RESPONSIBILITIES:</b><br>
<ul>
<li>Supervise a team of Case Managers</li>
<li>Provide regular, structured supervision meetings for direct reports</li>
<li>Promote effective strategies for case management following Housing First, Harm Reduction, Crisis Prevention Institute (CPI), Motivational Interviewing and trauma-informed care models of practice</li>
<li>Development and implement individual service plans; ensure documentation of client contact and progress is comprehensive and timely</li>
<li>Interview and assess potential clients</li>
<li>Provide clinical direction and leadership to the program, help troubleshoot, problem solve, and strategize solutions around client engagement and challenging behaviors</li>
<li>Promote a staff and resident culture that emphasizes housing placement as a measurement of success; ensure that clients are ready for housing placement</li>
<li>Oversee development and implementation of individual service plans; ensure documentation of client contact and progress is comprehensive and timely</li>
<li>Assess and evaluate client functioning</li>
<li>Supervise and conduct case conferences</li>
<li>Coordinate delivery of care with multiple service providers, particularly outreach teams.</li>
<li>Work collaboratively with outreach teams to ensure individuals succeed in transitional housing and avoid return to homelessness.</li>
<li>Provide support and guidance to front line staff in managing difficult situations and successfully de-escalating conflict</li>
<li>Perform other duties as assigned</li>

<b>MINIMUM QUALIFICATIONS:</b><br>
<ul>
<li>NYS Licensed Masters Social Worker (LMSW) required</li>
<li>Minimum 3 years of experience with related populations; previous supervisory and administrative experience preferred</li>
<li>Thorough clinical understanding of relevant service delivery concepts and structures including Housing First, client centered motivational interviewing, Harm Reduction models, behavioral and medical health systems, and the ability to access and negotiate the full range of services for recipients</li>
<li>Experience with crisis intervention, including, risk assessments and incident management</li>
<li>Ability to work successfully with a wide range of internal and external stakeholders</li>
<li>Demonstrated success in working in a fast-paced environment with multiple priorities</li>
<li>Ability to delegate and motivate staff to achieve deliverables</li>
<li>Ability to communicate (verbally and written) with diverse populations and stakeholders</li>
<li>Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams; familiarity with Access-based databases and the ability to learn new programs are required</li>

Qualifications: - An understanding of the litigation lifecycle and electronic discovery/document review processes, procedures and practices is required. - 7+ years of experience in a litigation support, electronic discovery and/or technology support environment, preferably within the Government, but a law firm or top eDiscovery vendor is also acceptable. - Experience performing Administrator and Case Manager functions in Relativity. - Experience with providing end-user support on all Litigation Support applications, including but not limited to assisting with end-user training. - Experience with Relativity, Nuix, Concordance, IPRO and other applicable eDiscovery software platforms. - Must be a US Citizen. - Must be able to obtain a favorably adjudicated Public Trust Clearance.
