About the Position
 
The Case Manager will be responsible for providing residential and long term support services to homeless and at-risk adults transitioning into permanent housing. The Case Manager will carry a caseload of approximately 20 to 25 formerly homeless and/or low income individuals. 
Case Manager will be required to have frequent contact with tenants, weekly and/or monthly face-to-face meetings, telephone conversations, and aftercare contacts. Frequent electronic exchanges with colleagues is expected .
 
 
Responsibilities:
 
<ul><li>Provide direct case management services (on-site and in the field). Case management services will include but not limited to mental health and substance use assessments, developing and updating service plans, assistance with obtaining public assistance, Social Security benefits/entitlements and other publicly available resources that may be beneficial to the tenant.</li><li>Using a person centered approach, provide case management services and assessments rooted in evidence based models such as Motivational Interviewing, Critical Time Intervention and Harm Reduction.</li><li>Utilize electronic health record. Maintain accurate and up-to-date tenant files/charts.</li><li>Coordinate and document clinical case management and psychosocial services while also documenting the overall effectiveness of the case management services provided. Case Managers will develop an Individual Service Plan and review the plan periodically to document process toward goals.</li><li>Serve as a liaison between tenant and community providers.</li><li>Facilitate groups with a focus on life skills, education and support, relapse prevention and mental health counseling.</li><li>Complete referrals to medical, mental health, and other outside providers.</li><li>Collaborate with Workforce Opportunities Staff to assist tenant in developing employment related goals, where applicable.</li><li>Ensure overall compliance with organizational and funder regulations. Adhere to documentation requirements as assigned by leadership and funder regulation/standard.</li><li>Coordinate aftercare services and follow-up contacts for post-discharge updates.</li><li>Conduct outreach and engagement including apartment visits and inspections.</li><li>Participate in community development efforts such as Community Meetings and supporting the residence in establishing partnerships with other providers in the community.</li><li>Work with Property Management staff to assist tenant to meet terms of their lease including timely payment of rent and avoidance of any nuisance behaviors. Mediate landlord-tenant disputes that threaten housing stability.</li><li>Utilize trauma informed interventions and culturally responsive engagement techniques to assist tenants with overcoming barriers to housing stability.</li><li>Participate in community initiatives to fulfill mission to end homelessness.</li><li>Provide crisis intervention as needed.</li><li>Provide all services in a manner that respects cultural differences amongst tenants.</li><li>Carry out other duties as assigned by leadership.

Requirements:
 
<ul><li>A Bachelor's degree in human services or related field.</li><li>Must possess strong organizational, writing, communication, and computer skills.</li><li>Some relevant experience working with low-income or homeless populations.</li><li>Fluent in Spanish is required.</li><li>Candidates must be self-starters with the skills and energy to work within the community.</li><li>Case Manager is regularly required to, type, use hands and fingers, reach with hands and arm. The Case Manager is regularly required to walk, stoop, and kneel. The Case Manager is often required to spend up to six (6) hours sitting at a desk during a normal eight (8) hour shift. <div>The Case Manager provides a full range of direct services to clients living on the streets and pursuing permanent housing. The Case Manager will work within an interdisciplinary team focusing on moving people off the streets of Upper Manhattan into permanent housing and successfully reintegrating into their communities. The Case Manager&rsquo;s responsibilities will include outreach to clients living on the street as well assisting their caseload with obtaining permanent housing, helping clients live full and satisfying lives in the community, helping their team and program to function well and meeting their contractual obligations including achieving contracted housing placement goals, and striving as appropriate to their role, to fulfill the CUCS programs&rsquo; core purpose, core values and vision statements.</div>

Essential functions include, but are not limited to:

Develop and maintain collaborative, professional relationships with clients while creating, implementing, and regularly updating individualized service plans that reflect each client's strengths, goals, and needs.</li><li>
Conduct ongoing assessments and monitor client progress, adjusting service plans as appropriate to support long-term stability and recovery.</li><li>

The Jewish Board’s Adult Residential Division supports adults with serious and persistent mental illness to live in the community with as much independence as possible. Using a non-judgmental, harm-reduction approach, staff model and guide residents to create and meet personalized recovery goals, develop independent living skills, and live in a healthy and secure manner. POSITION OVERVIEW: 
The Case Manager is the most senior level of direct care staff working within the Jewish Board’s supportive housing programs to help residents live in the community as independently as possible.
 
KEY ESSENTIAL FUNCTIONS: 
Resident Support

<li>Carries a caseload and practices person-centered and collaborative service planning with residents.</li>
<li>Is assigned the most complex clients; carries a larger caseload than case associates and case assistants</li>

POSITION SUMMARY
The Transitional Housing Case Manager will provide comprehensive case management services to LGBTQ+ homeless youth ages 21-24. This role delivers holistic, trauma-informed, culturally responsive care by assessing ongoing client needs, developing Individualized Service Plans, coordinating with internal and external resources, connecting clients to necessary resources and long term housing to prepare them for independent living.
The primary focus areas include housing stability, crisis response, and service coordination.
WORK SCHEDULE: Monday-Friday 10am-6pm with one late night 12p-8p. Some weekends and holidays required
WORK FLEXIBILITY: 1 remote day per week 
KEY RESPONSIBILITIES<ul><li>

<ul><li>Manage daily office routines to ensure efficient and effective office operations. Triage incoming calls and serve as first point of contact. Provide DOSO staff with all relevant information to provide timely responses and follow-up. Assist the DOSO in planning, organizing prioritizing and coordinating the timely assignment, follow-up and completion of work and projects. Track and record leave time for DOSO. Ensure that new employees are appropriately processed for payroll and proper procedures are followed for employment separations</li>
<li>Plan and maintain complex, busy and shifting schedules of appointments, meetings and travel. Ensure members of the DOSO staff are prepared with essential information and background for each meeting and/or trip. Draft travel itineraries, reserve lodging, set up meeting rooms, and handle all logistics associated with these activities. Prepare and process travel and expense reports in accordance with Institute policies</li>
<li>Record Management: Develop and maintain an efficient, confidential records management system to ensure immediate availability of materials. Assist with the maintenance and coordination of judicial and/or student intervention case files in case management system (Maxient)</li>
<li>Train, supervise and schedule work-study students. Assure that work is done in a timely manner and evaluate performance. Prepare all work-study paperwork coordinating with financial aid and payroll. Prepare student, bi-weekly and semi-monthly payroll</li>
<li>Utilize enterprise-wide Institute systems (Banner and Data Warehouse) and subsidiary systems to monitor financial activity and provide reports to the DOSO and the Student Experience Business Manager. Monitor spending to ensure adherence to general policies and guidelines. Identify and resolve discrepancies in accordance with Institute and DOSO portfolio standards and policies</li>
<li>Maintain inventory of office supplies which includes ordering and invoice processing through OSCAR and Banner</li>
<li>Coordinate communication, marketing, and social media efforts for any DOSO programming and educational initiatives. Arrange event details including but not limited to purchasing supplies or equipment, catering, reserving space, setting up and tearing down</li>
<li>Ensure that public spaces in Academy Hall are adequately maintained by inspecting public areas on a weekly basis and initiating service requests as needed. Monitor any possible safety concerns. Coordinate with the Student Experience Operations Manager if required to manage larger repair projects</li>
<li>Other duties and special projects as assigned</li>

Note that this is an administrative position and not a legal role.Oversee the day-to-day operations of New York County Lawyers Association&rsquo;s Part 137 Attorney-Client Fee Dispute Resolution Program.
