About the Position
 
The Case Manager will be responsible for providing residential and long term support services to homeless and at-risk adults transitioning into permanent housing. The Case Manager will carry a caseload of approximately 20 to 25 formerly homeless and/or low income individuals. 
Case Manager will be required to have frequent contact with tenants, weekly and/or monthly face-to-face meetings, telephone conversations, and aftercare contacts. Frequent electronic exchanges with colleagues is expected .
 
 
Responsibilities:
 
<ul><li>Provide direct case management services (on-site and in the field). Case management services will include but not limited to mental health and substance use assessments, developing and updating service plans, assistance with obtaining public assistance, Social Security benefits/entitlements and other publicly available resources that may be beneficial to the tenant.</li><li>Using a person centered approach, provide case management services and assessments rooted in evidence based models such as Motivational Interviewing, Critical Time Intervention and Harm Reduction.</li><li>Utilize electronic health record. Maintain accurate and up-to-date tenant files/charts.</li><li>Coordinate and document clinical case management and psychosocial services while also documenting the overall effectiveness of the case management services provided. Case Managers will develop an Individual Service Plan and review the plan periodically to document process toward goals.</li><li>Serve as a liaison between tenant and community providers.</li><li>Facilitate groups with a focus on life skills, education and support, relapse prevention and mental health counseling.</li><li>Complete referrals to medical, mental health, and other outside providers.</li><li>Collaborate with Workforce Opportunities Staff to assist tenant in developing employment related goals, where applicable.</li><li>Ensure overall compliance with organizational and funder regulations. Adhere to documentation requirements as assigned by leadership and funder regulation/standard.</li><li>Coordinate aftercare services and follow-up contacts for post-discharge updates.</li><li>Conduct outreach and engagement including apartment visits and inspections.</li><li>Participate in community development efforts such as Community Meetings and supporting the residence in establishing partnerships with other providers in the community.</li><li>Work with Property Management staff to assist tenant to meet terms of their lease including timely payment of rent and avoidance of any nuisance behaviors. Mediate landlord-tenant disputes that threaten housing stability.</li><li>Utilize trauma informed interventions and culturally responsive engagement techniques to assist tenants with overcoming barriers to housing stability.</li><li>Participate in community initiatives to fulfill mission to end homelessness.</li><li>Provide crisis intervention as needed.</li><li>Provide all services in a manner that respects cultural differences amongst tenants.</li><li>Carry out other duties as assigned by leadership.

Requirements:
 
<ul><li>A Bachelor's degree in human services or related field.</li><li>Must possess strong organizational, writing, communication, and computer skills.</li><li>Some relevant experience working with low-income or homeless populations.</li><li>Fluent in Spanish is required.</li><li>Candidates must be self-starters with the skills and energy to work within the community.</li><li>Case Manager is regularly required to, type, use hands and fingers, reach with hands and arm. CUCS is the lead support agency in the Manhattan Outreach Consortium (MOC) that uses the Housing First model. <div>The Case Manager provides a full range of direct services to clients living on the streets and pursuing permanent housing. The Case Manager will work within an interdisciplinary team focusing on moving people off the streets of Upper Manhattan into permanent housing and successfully reintegrating into their communities. The Case Manager&rsquo;s responsibilities will include outreach to clients living on the street as well assisting their caseload with obtaining permanent housing, helping clients live full and satisfying lives in the community, helping their team and program to function well and meeting their contractual obligations including achieving contracted housing placement goals, and striving as appropriate to their role, to fulfill the CUCS programs&rsquo; core purpose, core values and vision statements.</div>

Essential functions include, but are not limited to:

Develop and maintain collaborative, professional relationships with clients while creating, implementing, and regularly updating individualized service plans that reflect each client's strengths, goals, and needs.</li><li>

POSITION SUMMARY
The Transitional Housing Case Manager will provide comprehensive case management services to LGBTQ+ homeless youth ages 21-24. This role delivers holistic, trauma-informed, culturally responsive care by assessing ongoing client needs, developing Individualized Service Plans, coordinating with internal and external resources, connecting clients to necessary resources and long term housing to prepare them for independent living.

Qualifications: - An understanding of the litigation lifecycle and electronic discovery/document review processes, procedures and practices is required. - 7+ years of experience in a litigation support, electronic discovery and/or technology support environment, preferably within the Government, but a law firm or top eDiscovery vendor is also acceptable. - Experience performing Administrator and Case Manager functions in Relativity. - Experience with providing end-user support on all Litigation Support applications, including but not limited to assisting with end-user training. - Experience with Relativity, Nuix, Concordance, IPRO and other applicable eDiscovery software platforms. - Must be a US Citizen. - Must be able to obtain a favorably adjudicated Public Trust Clearance. The Jewish Board’s Adult Residential Division supports adults with serious and persistent mental illness to live in the community with as much independence as possible. Using a non-judgmental, harm-reduction approach, staff model and guide residents to create and meet personalized recovery goals, develop independent living skills, and live in a healthy and secure manner. Our work is guided by principles that underscore the critical importance of stable housing, use a culturally competent and anti-racist lens, and respect the dignity and legitimacy of each individual’s journey.
 
POSITION OVERVIEW: 
The Case Manager is the most senior level of direct care staff working within the Jewish Board’s supportive housing programs to help residents live in the community as independently as possible.
 
KEY ESSENTIAL FUNCTIONS: 
Resident Support
 <ul>
<li>Carries a caseload and practices person-centered and collaborative service planning with residents.</li>
<li>Is assigned the most complex clients; carries a larger caseload than case associates and case assistants</li>
<li>Ensures service plans are completed and signed by residents according to regulatory guidelines and standards.</li>
<li>Evaluates and assesses independent living skills using a detailed functional assessment tool or supports residents in using tool for self-assessment. Supports residents in meeting individual goals.</li>
<li>Supports residents toward the goal of medication management independence, including appropriate person-centered interventions.</li>
<li>Advocates on behalf of residents with other services providers and in a way that takes into account health disparities and systemic racism.</li>
<li>Interprets and explains agency policies and procedures to residents.</li>
<li>May run resident support groups.</li>
<li>Participates in monthly collaborative walk-throughs of unit/apartment with residents.</li>
<li>Escorts residents to appointments, as needed.</li>
<li>Facilitates monthly roommate meetings, when appropriate.</li>
<li>Assists residents with physically demanding tasks, such as carrying groceries, laundry, and other household items, as needed.</li>
<li>Heats fully cooked foods, on occasion.</li>
<li>Resolves crises and de-escalates situations threatening resident comfort and well-being.</li>
<li>Creates management plans to improve safety and prevent ongoing crises.</li>
<li>For congregate settings only: Performs safety rounds of the program and wellness checks, as necessary.</li>

Administrative Support and Coordination

<li>Provides services to those residents within the scope of program regulations and agency mission.</li>
<li>Independently completes regular and timely documentation of all resident interactions, including assessments, service plans and progress, and informational notes.</li>
<li>Ensures progress notes meet monthly billing documentation requirements, if necessary.</li>
<li>Files incidents reports, as appropriate.</li>
<li>Links residents to community-based treatment and collaborates with other services providers when coordinating services.</li>
<li>Participates in planning and execution of special and holiday events.</li>
<li>Participates in regular staff meetings and trainings.</li>
<li>Travels to off-site meetings and trainings, as needed.</li>
<li>Participates in intake, assessments, discharge, and follow-up conferences; may take the lead in presenting cases in these settings or in high-risk case meetings</li>
