As URI&rsquo;s inaugural supportive housing initiative, Innovative Urban Village requires the development of a comprehensive program model, policies, procedures, and service framework from inception through full implementation.

POSITION OVERVIEW

The Case Manager provides comprehensive, trauma informed, and culturally responsive supportive services to families residing in the Innovative Urban Village supportive housing program. Case Managers conduct assessments, develop individualized service plans, provide counseling and crisis intervention, and coordinate referrals to promote long term housing stability, safety, and independence.

This role requires strong engagement skills, the ability to support families impacted by domestic violence, and a commitment to evidence informed practices. Case Managers collaborate closely with Housing Specialists, the Economic Empowerment Specialist, and the Director of Clinical Services to ensure integrated, high quality service delivery aligned with ESSHI and OTDA standards.

Salary: $60,000 - $70,000

MAJOR DUTIES AND RESPONSIBILITIES

Direct Service &amp; Case Management
<ul>
<li>Conduct comprehensive intakes, biopsychosocial assessments, and individualized service plans for assigned residents.</li>

POSITION OVERVIEW

The Case Manager is responsible for direct service provision to residents. Direct services include individual counseling, group counseling, advocacy services, referrals and follow-up, home management and financial counseling. Individual must demonstrate sensitivity to issues related homelessness. In addition, the Case Manager will make all necessary linkages to community agencies to provide proper placement of residents in areas of permanent housing, referral to engage in after care services prior to leaving the agency, vocational and educational services, and to assist families in securing public assistance, Medicaid or other benefits.

MAJOR DUTIES AND RESPONSIBILITIES

<li>Maintain caseload of 25-30 families.</li>
<li>Conduct intake interviews, as assigned, to assess and evaluate residents for admission using DHS CARES system and as outlined by URI</li>
<li>Engage residents in initial and ongoing treatment processes, through individual and group counseling, under clinical supervision of the Social Work Supervisor.</li>
<li>Conduct independent living plans on at least a biweekly basis in DHS CARES system</li>

The HLPS Case Manager (a &ldquo;CM&rdquo;) is an essential team member that develops and executes housing navigation and placement services including follow-up post placement for clients referred by NYC Health + Hospitals (H+H). The CM meets with the client in person to develop a housing plan and complete the housing packet and works to support the client to find and move into permanent housing. The CM checks in at consistent, regular intervals during and post move-in/placement and will help the client renew their lease or reapply for vouchers if necessary. The CM will document all interactions, activities, and status changes. The CM will also coordinate services with other contacts to support the client as needed, including shelter/safe haven housing specialist, H + H care team, etc. and as needed, referrals to external providers. Our Story
The West Side Federation for Senior and Supportive Housing, Inc. (WSFSSH) is a community‐based organization whose mission is to provide safe, affordable housing with supportive services within a residential setting which enhances the independence and dignity of each person. WSFSSH develops, manages, and provides housing for low‐ income older persons, many of whom live with mental illness and/or have experienced homelessness.
Position Description
Under the supervision of the Property Manager, the front desk staff is responsible for providing a range of services and supervision to residents with the goal of enabling residents to live as safely and independently as possible. Additionally, the front desk staff will work to maintain an environment of respect and compassion, which enhances the dignity of the residents, staff, and visitors.
This is a full-time position (5) days a week on-site.
Responsibilities
Resident Supervision<ul><li>
Monitor the well-being of residents</li><li>
Monitor residents entering and leaving the building, responding to behaviors which might be dangerous to self or others (for example: if a resident is leaving the building, but appears disoriented and confused, response might include talking with the resident and encouraging him/her to stay in the lobby, notifying social service staff, or even calling 911 depending on the circumstances).</li><li>
Respond to emergency calls from resident rooms, relay requests to appropriate staff and/or implement emergency procedures as needed.</li><li>

 The Jewish Board’s Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resiliency of our communities.
 
PROGRAM DESCRIPTION
 The ACT programs provide an integrated set of evidenced based treatment, rehabilitation, case management and support services to individuals residing in NYC, in Shelter programs, and/or living in supportive housing programs or their own residences in the community who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Through a mobile, multi-disciplinary team of staff, recipients are supported in their recovery through an individualized, person-centered model of care.
 
POSITION OVERVIEW: 
This multi- faceted position works directly with adults living with severe and persistent mental illness (SPMI) and substance use disorders (SUD) living in NYC. In addition to providing direct care, this position provides administrative oversight and support to the treatment team to ensure the delivery of services is in accordance with regulatory and payer requirements. Clinical Coordinators use a recovery oriented approach to link recipients to the services they need to obtain housing and to achieve wellness self-management and increasing independence. Working in a team setting and primarily in the community, Clinical Coordinators assess risk and needs, develop person centered care plans, provide care management services, track and arrange appointments, educate clients and coordinate other aspects of clients’ health and community services. KEY ESSENTIAL FUNCTIONS: 
<ul>
<li>Assume administrative tasks such as chart auditing, collaborating with outside partners, facilitating team meetings and being the point person in crisis situations when Program and Team Director are not available.</li>
<li>Supervise students</li>
<li>Assist with on-boarding/orientation of new staff</li>
<li>When needed/requested, lead team in-service training on topics that will improve service to clients.</li>
<li>Comprehensive assessment of client within 45-60 days of entry into program.</li>
<li>Work closely with team to visit each client a minimum of 6 times monthly.</li>
<li>Integration of medical, specialized and behavioral health services in addition to social support and/or educational support services</li>
<li>Periodic reassessment of a client’s medical and behavioral health needs and update of, adherence to recommended treatments, and need for care coordination</li>
<li>Attend scheduled team meetings and one-on-one supervisions</li>
<li>Attend mandatory ACT trainings and Jewish Board MKSEI 13 Module course training.</li>
<li>Collaborative development of an Individualized Care Plan (ICP) with the client, the client’s family and/or caregivers in addition to other service providers, and review and revise ICP per regulatory standards or when there are significant changes in functioning or clinical status.</li>
<li>Provide care management services</li>
<li>Track all specialty medical, behavioral and support service referrals made for patient using Health Information Technology (HIT) provided.</li>
<li>Assure that client has access to, engages in and retains needed services as defined in the ICP.</li>
<li>Provide outreach services to clients to increase engagement in services identified on individualized care plan</li>
<li>Complete all required documentation</li>
<li>Provide after-hours emergency coverage via cell phone on rotating basis.</li>
</ul>
EDUCATIONAL / TRAINING REQUIRED: 
<ul>
<li>Obtained a Master’s level degree in human services field with appropriate NYS licensure (e.g. MSW, LMSW, LMHC, LCSW, LMFT, Creative Arts Therapist LCAT, licensed Occupational therapist, Certified Psychiatric Rehabilitation Counselor, Rehabilitation Counselor, licensed psychologist or hold a Master’s Degree as a Therapeutic Recreation Specialist or Psychologist or Pastoral Counselor or Master’s degree in a related field.</li>
<li>Previous work on an ACT team or similar type of community based/ mobile based program for at least one year.</li>
</ul>
VISUAL AND MANUAL DEXIERITY: 
<ul>
<li>Able to read data and documents, including spreadsheets, reports and Electronic Health Records in printed form and on computer screens.</li>
<li>Able to input data into the Electronic Health Record.</li>
<li>Limited applications of manual dexterity and hand-eye coordination.</li>

Position Summary

The Office of Institutional Equity (&ldquo;OIE&rdquo;) is Columbia University&rsquo;s central office for addressing all reports of discrimination and discriminatory harassment. OIE&rsquo;s jurisdiction includes matters arising under the University&rsquo;s Discrimination and Discriminatory Harassment Policies. OIE manages and addresses concerns raised by students, faculty, staff, and affiliates that reach OIE either through direct reporting or through referrals from schools, departments, and campus partners.

The Director of the Discrimination and Discriminatory Harassment Faculty &amp; Staff Division, who reports to the Vice Provost for the Office of Institutional Equity (&ldquo;OIE&rdquo;), is responsible for preventing and responding to discrimination, discriminatory harassment, and retaliation involving University faculty, staff, or affiliates. The Director develops and administers policies and procedures addressing prohibited conduct in accordance with applicable federal, state, and local anti-discrimination laws, including Title VI, VII and IX. The Director provides support to individuals seeking to bring complaints against University faculty, staff, or affiliates, and oversees resolution processes including Informal Resolutions, Formal Investigations, and the enforcement of the University&rsquo;s Anti-Discrimination and Discriminatory Harassment Policies and Procedures for Faculty and Staff.

Responsibilities
<ul>
<li>Oversees and conducts investigations into alleged violations of the Anti-Discrimination and Discriminatory Harassment Policies and Procedures for Faculty and Staff (&ldquo;the Policy&rdquo;) and facilitates informal, formal, or other resolutions of any alleged violations;</li>
<li>Oversees and ensures the provision of supportive or remedial measures to students, faculty and staff impacted by potential violations of applicable policies or applicable laws;</li>
<li>Provides guidance to students, faculty and staff, concerning compliance with the University&rsquo;s Policy;</li>
<li>Works closely and collaboratively with the University&rsquo;s Office of General Counsel, Deans of the schools, and University stakeholders.</li>
<li>Daily direct supervision and oversight of the Division&rsquo;s Investigators and their caseloads.</li>
<li>Oversees the team&rsquo;s handling of each investigation to ensure progression and timely completion, reviews and provides feedback on drafts of memos, reports and other correspondence.</li>
<li>Lead weekly case/investigation status update meetings with Investigators, Case Managers, and the Operations team; serve as liaison between investigative team and senior administrators to provide updates, discuss the need for interim measures/supportive accommodations, and address other concerns raised during the course of the investigation.</li>
<li>Revise policy based on changes to federal, state, and local laws and regulations, as well as feedback from the University community and investigative team.</li>
<li>Work with Operations and Case Management staff to create standard operating procedures and best practices and collaborate with OIE Division Directors to advance the work of the office.</li>
<li>Review and provide feedback on training materials, OIE policies and procedures, and various reports, including the Annual Report.</li>
<li>Discuss overlapping cases with the other OIE investigative teams and coordinate next steps.</li>
<li>Work closely with the Director of Training and Education on resolving matters informally.</li>

Minimum Qualifications
<ul>
<li>J.D. (New York Bar admission required);</li>
<li>Seven years of relevant experience, such as litigation, including substantial courtroom experience, experience investigating protected class discrimination, harassment, or related criminal conduct;</li>
<li>Knowledge of, and experience with, Title VI, VII, Title IX and FERPA as well as state and local anti-discrimination laws;</li>
<li>Supervisory and investigative experience;</li>
<li>Experience providing training and guidance on compliance with nondiscrimination laws;</li>
<li>Experience providing supportive and remedial measures for violations of prohibited conduct under Title VI, Title VII, Title IX, and nondiscrimination laws.</li>
</ul>
Preferred Qualifications
<ul>
<li>Familiarity and experience with Title VI, Title VII, the ADA, the ADEA and, the New York State and New York City Human Rights Laws, the Clery Act, the VAWA and relevant aspects of the New York State Education Law;</li>
<li>Experience conducting and supervising law enforcement investigations;</li>
<li>Experience in an educational environment, ideally a higher education environment;</li>
<li>Experience with mediation or alternative resolution practices;</li>
<li>Experience working with people who have experienced discrimination, bias, harassment, and/or violence based on their identity including sexual harassment/sexual violence;</li>
<li>Ability to demonstrate fairness and trust in responding to complaints and conflict management;</li>
<li>Strong organizational skills and the ability to manage and prioritize multiple projects simultaneously, to adapt to changing environments and shifting priorities, to meet fixed and short deadlines and manage stressful situation;</li>
<li>Ability to work effectively and collaboratively with people from diverse backgrounds;</li>
<li>Strong research skills and the ability to synthesize a wide variety of data and compile appropriate reports using spreadsheets and other appropriate software. Preferred experience working with a case management database such as Maxient;</li>
<li>Demonstrated ability to communicate effectively both verbally and in writing, multitask and meet sensitive timelines, use discretion and good judgment; maintain a high level of confidentiality, and work collaboratively with diverse constituencies including students, faculty, staff, administrators, and community partners.</li>
