POSITION OVERVIEW

The Case Manager provides comprehensive, trauma informed, and culturally responsive supportive services to families residing in the Innovative Urban Village supportive housing program. Case Managers conduct assessments, develop individualized service plans, provide counseling and crisis intervention, and coordinate referrals to promote long term housing stability, safety, and independence.

This role requires strong engagement skills, the ability to support families impacted by domestic violence, and a commitment to evidence informed practices. Case Managers collaborate closely with Housing Specialists, the Economic Empowerment Specialist, and the Director of Clinical Services to ensure integrated, high quality service delivery aligned with ESSHI and OTDA standards.

Salary: $60,000 - $70,000

MAJOR DUTIES AND RESPONSIBILITIES

Direct Service &amp; Case Management

POSITION OVERVIEW

The Case Manager is responsible for direct service provision to residents. Direct services include individual counseling, group counseling, advocacy services, referrals and follow-up, home management and financial counseling. Individual must demonstrate sensitivity to issues related homelessness. In addition, the Case Manager will make all necessary linkages to community agencies to provide proper placement of residents in areas of permanent housing, referral to engage in after care services prior to leaving the agency, vocational and educational services, and to assist families in securing public assistance, Medicaid or other benefits.

MAJOR DUTIES AND RESPONSIBILITIES
<ul>
<li>Maintain caseload of 25-30 families.</li>
<li>Conduct intake interviews, as assigned, to assess and evaluate residents for admission using DHS CARES system and as outlined by URI</li>
<li>Engage residents in initial and ongoing treatment processes, through individual and group counseling, under clinical supervision of the Social Work Supervisor.</li>
<li>Conduct independent living plans on at least a biweekly basis in DHS CARES system</li>
<li>Monitor case plans for effectiveness and individual client needs, functional capabilities and assistance needed to increase independent functioning</li>
<li>Develop and maintain housing referral resources, assist residents in the process of locating and securing permanent housing, in conjunction with Housing Specialist.</li>
<li>Assists all residents in the process of referral and placement in community based resources including after care services</li>
<li>Assess residents&rsquo; vocational and educational needs, aptitudes and interests through interviews and testing. Develop and maintain placement and follow-up as appropriate after assessment and counseling.</li>
<li>Develop and maintain liaison activities with DHS, HRA. Public Assistance and Medicaid Administrators and Workers, to assist eligible residents in securing Public Assistance and Medicaid by acting as a personal advocate through telephone or personal visit/contact</li>

<li>Assist in the performance of weekly unit inspections of all units, including child welfare-involved families with children from birth to six(6) months of age and all other units on a biweekly basis</li>

<li>Make appointments for residents&rsquo; physical exams.</li>

<ul><li>SCOPE OF ROLE:</li></ul><div>
The HLPS Case Manager (a &ldquo;CM&rdquo;) is an essential team member that develops and executes housing navigation and placement services including follow-up post placement for clients referred by NYC Health + Hospitals (H+H). The CM meets with the client in person to develop a housing plan and complete the housing packet and works to support the client to find and move into permanent housing. The CM checks in at consistent, regular intervals during and post move-in/placement and will help the client renew their lease or reapply for vouchers if necessary. The CM will document all interactions, activities, and status changes. The CM will also coordinate services with other contacts to support the client as needed, including shelter/safe haven housing specialist, H + H care team, etc. and as needed, referrals to external providers. The CM plays a key role in executing and updating housing plans, identifying needs, preferences and eligibilities, monitoring client behaviors and practices, and trouble-shooting incidents.</div>

Regularly schedule follow-up after initial placement to identify further support needs, trouble-shoot incidents, confirm payment, and reapply for vouches and support in lease renewals as needed.</li><li>

Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.
 
But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
 
W. 83rd Street Safe Haven
 Located on the Upper West Side of Manhattan, the W. 83rd Street Safe Haven is comprised of 108 units for individuals not yet ready to transition into permanent housing, as well as those who may have struggled in other housing programs. West 83rd Street offers an alternative to life on the street: private, safe, clean, and affordable short-term accommodations. On-site case management and clinical services are available to meet the needs of its residents and to help those ready to move into secure permanent housing.
 Reporting to the Facility Maintenance Supervisor, the Housekeeper is a part of the department that maintains the common areas of the safe haven. Examples of common areas include offices, multi-purpose rooms, roof deck, garden, trash collection area, lobby, fitness room and sidewalks. Client areas will also be cleaned.
 This position has a 35-hour work week, onsite.
 
ESSENTIAL DUTIES AND RESPONSIBILITES: 
<ul>
<li>Sweep, mop, wash, dust, and vacuum</li>
<li>Seal, wax, and buff floors and hard surfaces</li>
<li>Maintain sidewalks, including sweeping, raking leaves, and shoveling snow</li>
<li>Clean individual units when vacated or requested by work order</li>
<li>Sort and package recyclables to comply with NYC regulations</li>
<li>Operate compactor</li>
<li>Set up and clean up for meetings and other events</li>
<li>Assist painters and engineers when assigned</li>
<li>Perform work at other facilities as required</li>
<li>Perform other related duties as assigned</li>
</ul>
MINIMUM QUALIFICATIONS: 
<ul>
<li>Comparable work experience</li>
<li>Experience operating of vacuum cleaner, high speed buffer</li>
<li>Ability to lift and move furniture, appliances, etc.</li>
<li>High School Diploma, GED, Trade School Diploma, or equivalent</li>
<li>Ability to stand for several hours during shift and to climb stairs in the event of an emergency</li>
<li>Experience working with special needs populations a plus</li>
<li>Basic knowledge of using a computer is a plus, but is not required</li>

Reporting to the Senior Vice President of Housing Development, the Project Manager, Design and Construction is part of the Housing Development team guiding Breaking Ground’s portfolio of architecturally significant supportive and affordable housing from implementation through the design process, construction, project and turnover to the Property Management team.
 
Travel within the five boroughs of NYC will be required.
 
ESSENTIAL DUTIES AND RESPONSIBILITES: 
<ul>
<li>Support the Design and Construction and Program Development teams on all aspects of new and existing projects</li>
<li>Manage contractors, vendors, and professionals towards successful project completion</li>
<li>Prepare feasibility analyses regarding zoning and design for acquisition opportunities</li>
<li>Recruit and prequalify architects and contractors</li>
<li>Work collaboratively with all members of the Real Estate Development team on new programs/initiatives</li>
<li>Set up, maintain and administer spreadsheets, databases, and shared drive project files</li>
<li>Maintain project documentation including construction documents, RFP responses, governmental agency approvals, official correspondence, and other project related materials</li>
<li>Record and distribute meeting minutes, track follow up items to completion in a timely fashion</li>
<li>Produce monthly project reports</li>
<li>Solicit proposals, analyze, and prepare contracts with environmental, zoning, and other consultants</li>
<li>Track consultant contracts and interface with Finance for contract administration</li>
<li>Coordinate with NYC and NYS governmental agencies, e.g., NYC HPD, NYC DOB</li>
<li>Report on project progress to the Housing Development Team</li>
<li>Insure timeliness and quality of contract deliverables from consultants</li>
<li>Review construction documents for compliance with organizational standards</li>
<li>Review project documents to ensure that they are complete</li>
<li>Prepare RFPs for purchase of furnishings and equipment for new and existing projects</li>
<li>Coordinate provision of phone, cable, security systems and IT for new and existing projects</li>
<li>Evaluate cost and value decisions in design and construction with the project team and insure funding exists to pay for the same</li>
<li>Regularly visit construction sites as needed</li>
<li>Monitor project schedules and assist with problem solving to ensure on-time delivery of projects</li>
<li>Manage project closeout including monitoring punch-lists to ensure timely completion for occupancy</li>
<li>Coordinate closeout materials for turnover of new projects to Asset Management</li>
<li>Perform other duties as assigned</li>
</ul>
MINIMUM QUALIFICATION: 
<ul>
<li>Bachelor’s degree in Architecture, Engineering, or Construction Management preferred</li>
<li>Minimum of 3 years of construction planning and administration required</li>
<li>Familiarity with Federal, State and Local Affordable and Supportive Housing funder design requirements</li>
<li>Ability to read and understand construction drawings and specifications, as well as submittals, shop drawings, and any other related materials</li>
<li>Demonstrate an understanding of building materials, systems, specifications, zoning, codes, ADA requirements, details, and construction techniques</li>
<li>Strong sustainability experience</li>
<li>Familiarity with LEED, Passive House, and Enterprise Green Communities</li>
<li>Interior design and furnishings experience</li>
<li>Ability to organize, complete, and track multiple tasks and projects simultaneously</li>
<li>Proficiency with AutoCAD, Revit and Microsoft Office Suite of programs including Word, Excel, Outlook, and Microsoft Team</li>

 2. Work with clients and their medical and behavioral health providers to document disability to support SSI/SSD applications and appeals.
 3. Provide individualized money management and budgeting coaching and as needed connect clients to services to adult guardianship or representative payee services.
 4. Develop and deliver workshops on topics related to benefits, budgets and money management.
 5. Utilize best practice approaches and evidence-based interventions to maximize client outcomes.
 6. Maintain service documentation to include client progress notes, observations and interactions about clients and incident reporting when necessary.
 7. Participate in internal and external case conferencing activities.
