</p><p><b>POSITION SUMMARY</b></p><p>
The Transitional Housing Case Manager will provide comprehensive case management services to LGBTQ+ homeless youth ages 21-24. This role delivers holistic, trauma-informed, culturally responsive care by assessing ongoing client needs, developing Individualized Service Plans, coordinating with internal and external resources, connecting clients to necessary resources and long term housing to prepare them for independent living.</p><p>
The primary focus areas include housing stability, crisis response, and service coordination.</p><p>

Reporting to the Assistant Program Director, Transitional Housing the Clinical Coordinator, Transitional Housing supervises Case Managers and ensures the utilization of Harm Reduction and Housing First strategies and other best-practice clinical interventions to ensure the highest quality care for clients. The Clinical Coordinator is responsible for client records are kept in compliance with regulatory and agency standards.

<b>ESSENTIAL DUTIES AND RESPONSIBILITIES:</b><br>

Position Description</p><p>
The Staff Engagement and Culture Manager plays a key role in cultivating a connected, inclusive, and engaging employee experience by leading programs and initiatives that strengthen culture, recognition, and community at WSFSSH.</p><p>

Lead the implementation and ongoing management of WSFSSH's signature employee engagement programs and culture-building initiatives.</li><li>
Partner with the Managing Director to develop annual engagement strategies, work plans, event calendars, and implementation timelines aligned with organizational priorities.</li><li>
Identify opportunities to enhance and expand employee engagement programs, recognition strategies, and culture-building initiatives.</li><li>
Champion a culture of recognition, inclusion, collaboration, and belonging through programs that foster meaningful employee experiences across the organization.</li><li>
Coordinate and support employee affinity groups, employee-led initiatives, and other community-building programs that advance the organization's Diversity, Equity, and Inclusion goals byprovidingoperational guidance, resources, and logistical support.</li><li>
Oversee all programlogistics, including scheduling, cross-site coordination, vendor management, communications, registrations, catering, and day-of execution.</li></ul><p>

DUTIES &amp; RESPONSIBILITIES:</b></p><div><ul><li>
Perform daily inspections of all clients&rsquo; rooms</li><li>
Escort clients to appointments as needed</li><li>
Interact with and monitor clients' behavior, maintaining a good rapport by providing excellent customer service. Encourage clients to comply with the facility's policies and procedures.</li><li>
Control access at the main entrance by screening all clients and visitors with a magnetometer or x-ray machine to detect and prevent weapons from entering the site.
</li><li>Perform Crisis Intervention to de-escalate potential crises and document all such situations and interventions.</li><li>
Make hourly rounds throughout the entire facility, including all hallways, stairwells, floors, bathrooms, laundry rooms, kitchen, and office spaces.</li><li>

Assist with client intakes</li><li>
Perform daily pack-ups and logging of client property belonging to AWOL clients</li><li>
Store and remove client property once the mandated storage time has expired</li><li>
Maintain a clean and organized storage area</li><li>
Complete logs, incident reports, and all other required documentation.</li><li>
Make calls to police, fire department, and other first responders as directed.</li><li>
Perform administrative and receptionist duties at the operations desk; monitor video surveillance cameras, and the fire prevention/detection alarm notification system.</li><li>
Provide duties related to fire incidents, evacuation of the building when necessary, and utilizing Fire procedures.</li><li>
