JOB SUMMARY: Under the direct supervision of the Manager of Integrated Care, the Medical Social Worker (MSW) will be responsible for providing psychosocial care management services and the development of resources for case management clients. The primary focus of the MSW is to develop, implement, track, and supervise components of the psychosocial care plan. The MSW will work with the Integrated Clinic Program team to provide a multidisciplinary integrated approach to care and wellness.
ESSENTIAL FUNCTIONS:<ul><li> Serve as part of a multidisciplinary care team to provide case management and patient care services;</li><li> Provide brief mental health interventions for chronic diseases including motivational interviewing, crisis assessment and intervention, suicide risk assessment and intervention;</li><li> Utilize training and experience to conduct comprehensive psychosocial assessments including history of trauma, attachment styles, substance abuse, cognitive functioning, and activities of daily living;</li><li> Oversee all psychosocial aspects of clients&rsquo; treatment in Integrated Clinic Program;</li><li> Meet clients in the field to support and provide needed social services and address social determinates of health;</li><li> Provide appropriate information on benefits, social services, and referrals while serving as an advocate;</li><li> Provide consultation utilizing mental health expertise on multi-disciplinary ICP treatment teams;</li><li> Autonomously develop comprehensive care plans for clients and utilize training in motivational and behavioral intervention to provide support and guidance in the implementation of care plans which may include health-related behavior, housing, benefits, transportation, food/groceries, and other social services as needed;</li><li> Independently identify patient needs, strengths, and readiness to change;</li><li> Streamline care coordination to improve patients&rsquo; access to services including medical, mental health, and substance abuse programs;</li><li> Promote linkage to and retention in medical, mental health, and substance abuse treatment;</li><li> Identify and address barriers to treatment adherence and risk reduction to help improve health outcomes;</li><li> Conduct follow-up activities to ensure clients receive appropriate support for referred services and ongoing needs;</li><li> Act as liaison between clients and LA LGBT Center services/programs, and other community agencies/programs;</li><li> Coordinate necessary documents for public and private assistance programs;</li><li> Oversee the transportation needs for ICP clients within the ICP program;</li><li> Provide referral and linkage to partner services, e.g., domestic violence, medical services, HIV testing;</li><li> Make appropriate referrals to ancillary team members, i.e., Housing Case Management Staff, Nutritionist, Pharmacist, etc.;</li><li> Present clients in weekly case conferences;</li><li> Consult with staff on service coordination and service plans for clients referred to ICP program;</li><li> Maintain complete and accurate files documenting all care and services provided in the client&rsquo;s health record and all other required documentation methods;</li><li> Interact in a professional manner with health services staff and patients;</li><li> Exercise appropriate judgment and decision-making skills;</li><li> Use standard office machinery including personal computers and printers in performance of duties;</li><li> Assist in data auditing tasks, prepare reports as required, and submit in a timely manner;</li><li> Participate in staff meetings, agency meetings, planning meetings, and other meetings as needed;</li><li> Comply with requirements of OSHA, HIPAA, and other applicable regulations;</li><li> Other duties as assigned.</li></ul>

 Position Summary:
A Mental Health Worker/Case Manager will assist in the development of treatment plan(s) that are appropriate to the needs and desires of the client. The treatment plan(s) will aim to promote recovery and stability for community living and helping the client to achieve higher levels of function. 


Core Duties And Responsibilities:
<ul><li>
Monitor client compliance with treatment as prescribed by treatment team- this may or may not be done in the office or by means of field services.</li><li>
Develop individualized case management treatment plan(s) with the clients in the Full Service Partnership (FSP) Program.</li><li>
Assist with establishing coordination with local agencies in the provision of services.</li><li>
Assist with the development and maintain resource directory for agency.</li><li>
Update and maintain client records.</li><li>
Arrange for medical care-exams, emergency shelter, food and/or education services for clients at large.</li><li>
Facilitate support/self-help groups for clients, family members and the community at large.</li><li>
Participate in case disposition meetings on a routine basis with the assigned treatment team.</li><li>
Provide and document services in accordance with the Rehabilitation Option/Targeted Case Management requirements.</li><li>
Participate in the Mental Health education program provided to the community.</li><li>
Coordinate client care with other providers both within and outside the agency.</li><li>
Attend and participate in all in-service training as necessary.</li><li>
Responsible for timely completion of documentation in the client&rsquo;s clinical record per Enki, LA County Department of Mental Health and legal requirements.</li><li>
Responsible for meeting assigned minimum productivity requirements on a monthly basis.</li><li>
Other duties as assigned.</li><li>
Comply with all Company policies and procedures as applicable.</li></ul>
Minimum Education And Experience Preferred:
<ul><li>
High school diploma plus two (2) years experience in a mental health setting; OR Bachelor&rsquo;s Degree from accredited college/university in a related field (i.e. social work, psychology or sociology).</li></ul> 


Licenses/Certifications Preferred:
<ul><li>
Valid California driver&rsquo;s license, current automobile insurance, and eligibility to drive for the Company preferred.</li></ul> 


Knowledge, Skills And Abilities:
<ul><li>
Ability to perform in a positive, courteous, flexible and adaptive manner.</li><li>
Able to exercise independent judgment and initiative.</li><li>
Computer literate and experienced with computer programs such as Microsoft Office Word, Outlook and Excel.</li><li>
Experience using an Electronic Medical Health Record databases is a plus.</li><li>
Must be able to communicate clearly, both orally and written.</li><li>
Bilingual English/Spanish is a plus.</li></ul> 


Physical Requirements:
<ul><li>
See well enough (with or without corrected vision) to read fine print, hand-written materials, and a computer screen.</li><li>
Speak and hear well enough (with or without assistive device) to converse in person and by telephone.</li><li>
Finger, handle, feel, and reach with hands and arms, including ability to use a computer keyboard.</li><li>
Have the mobility (with or without assistive device) to stand, sit, crouch, and move from one work area to another.</li><li>
Have the strength to lift, carry or push office equipment and supplies used in normal job functions up to 20 lbs.</li><li>
Stamina to effectively perform work activities.</li></ul>
