Information is to be articulated in an easily understood and accurate manner to families whose ability to process information is impaired by emotionally traumatic circumstances. Infinite Legacy&rsquo;s Tissue Family Services Coordinator 1 coordinates and facilitates emotional support during and after the discussion of the donation process. Rapport is established with donor families, Infinite Legacy team members, and hospital staff involved in the donation process. The Tissue FSC I will be aware and sensitive to diversity of all kinds (cultural, religious, educational, etc.) while working with families and hospital staff.</p><p></p><p><b><br>
Education and Experience:</b></p><ul><li>
Master&rsquo;s degree at an accredited college or university in social work, counseling, psychology, nursing, or related degree. Bachelor&rsquo;s degree will be considered if relevant experience is met.</li><li>

The Tissue FSC I will provide trauma, grief, and bereavement support services for the family over the phone during initial contact. Tissue FSC I will identify if additional support is needed and connect the family with local hospital or community resources to provide additional support. In addition, the plans for grief and bereavement services will be communicated to the Aftercare Team at Infinite Legacy who is responsible for following the donor family after donation.</li><li>
Completes all necessary documentation within the time limits established by Infinite Legacy and its regulatory bodies.</li><li>
Collaborates with local funeral homes and whole-body donation programs to ensure that the donor family is supported, and the funeral home or whole-body donation program is aware of the tissue recovery process.</li><li>
Works in conjunction with appropriate Infinite Legacy staff during local tissue and eye cases. Collaborates closely with clinical personnel, DSC, Tissue AOC, HS, and TR during donor management to ensure that donor families&rsquo; needs are communicated to the Infinite Legacy team in addition to ensuring all case logistics are being communicated effectively to support donor and family needs during the donation process.</li><li>
Triages phone calls from families that are received from the Donor Services Center as it pertains to tissue donation needs, hospice referrals and medical evaluation for donation. At times collaboration with Hospice RNs, Nursing Homes or Assisted Living Facilities are needed to ensure the donors wishes can be honored.</li><li>
Navigates sensitive conversations as it pertains to the rejection of tissues post donation process. Initiates follow-up support over the phone to donor families as it relates to offering donor in spirit post donation should tissues be rejected or not able to be transplanted.</li><li>
Facilitates communication with outside OPO&rsquo;s who require local registry checks within IL&rsquo;s DSA. This includes DC, VA, and MD registry checks in a timely manner.</li><li>
Initiates follow-up support to donor and non-donor families when appropriate and is responsible for ensuring effective communication and documentation of aftercare components within the time limits established by Infinite Legacy and its regulatory bodies.</li><li>
Participates in continuing education as identified by the Family Services Manager &ndash; Tissue. This could be but is not limited to conversation coaching, SIM, educational seminars, and classes.</li><li>

A.D.N degree from accredited school of nursing required; B.S.N preferred; three-year diploma from accredited diploma program may be substituted for nursing degree</p></li><li><p>
Current Registered Nurse (RN) license in the state of practice required</p></li><li><p>
Continuing education credits maintained as required by state of practice required</p></li><li><p>
Minimum of five (5) years&rsquo; experience in clinical nursing required</p></li><li><p>
Minimum of three (3) years&rsquo; experience in renal nursing preferred</p></li><li><p>
Case management experience is strongly preferred, with demonstrated ability to assess patient needs, develop and manage individualized care plans, coordinate services across interdisciplinary teams, and support patients through complex healthcare transitions.</p></li><li><p>
Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred</p></li><li><p>
Demonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes</p></li><li><p>
Demonstrated experience and effectiveness in change agent role</p></li><li><p>
Demonstrated knowledge and understanding of CQI techniques</p></li><li><p>
Previous experience in healthcare performance coaching required</p></li><li><p>
Current CPR certification required</p></li><li><p>
Ability to modify personal practice patterns to adapt to new / electronic processes and increased productivity expectations as it pertains to Capella implementation</p></li><li><p>
Current driver&rsquo;s license in state employed with positive driving record and able to meet requirements of insurance coverage required</p></li><li><p>
Basic computer skills and proficiency in MS Word and Outlook required</p></li><li><p>
Functional proficiency with DaVita specific clinical software programs, including Capella, required within 90 days of employment</p></li><li><p>
