Rapport is established with donor families, Infinite Legacy team members, and hospital staff involved in the donation process. Duties/Responsibilities:</b></p><ul><li>
The Tissue FSC I will establish initial contact over the phone with the potential donor family and offer the option of tissue and eye donation in accordance with all state, federal and industry related regulations. Provides the family with a copy of an authorization or disclosure, Document of Gift if applicable and completes the medical/social history proficiently.</li><li>
The Tissue FSC I will provide trauma, grief, and bereavement support services for the family over the phone during initial contact. Tissue FSC I will identify if additional support is needed and connect the family with local hospital or community resources to provide additional support. In addition, the plans for grief and bereavement services will be communicated to the Aftercare Team at Infinite Legacy who is responsible for following the donor family after donation.</li><li>
Completes all necessary documentation within the time limits established by Infinite Legacy and its regulatory bodies.</li><li>
Collaborates with local funeral homes and whole-body donation programs to ensure that the donor family is supported, and the funeral home or whole-body donation program is aware of the tissue recovery process.</li><li>
Works in conjunction with appropriate Infinite Legacy staff during local tissue and eye cases. Collaborates closely with clinical personnel, DSC, Tissue AOC, HS, and TR during donor management to ensure that donor families&rsquo; needs are communicated to the Infinite Legacy team in addition to ensuring all case logistics are being communicated effectively to support donor and family needs during the donation process.</li><li>
Triages phone calls from families that are received from the Donor Services Center as it pertains to tissue donation needs, hospice referrals and medical evaluation for donation. At times collaboration with Hospice RNs, Nursing Homes or Assisted Living Facilities are needed to ensure the donors wishes can be honored.</li><li>
Navigates sensitive conversations as it pertains to the rejection of tissues post donation process. Initiates follow-up support over the phone to donor families as it relates to offering donor in spirit post donation should tissues be rejected or not able to be transplanted.</li><li>
Facilitates communication with outside OPO&rsquo;s who require local registry checks within IL&rsquo;s DSA. This includes DC, VA, and MD registry checks in a timely manner.</li><li>
Initiates follow-up support to donor and non-donor families when appropriate and is responsible for ensuring effective communication and documentation of aftercare components within the time limits established by Infinite Legacy and its regulatory bodies.</li><li>
Participates in continuing education as identified by the Family Services Manager &ndash; Tissue. This could be but is not limited to conversation coaching, SIM, educational seminars, and classes.</li><li>
Attend regularly scheduled Infinite Legacy staff meetings, FS team meetings, and case review.</li></ul><p></p><p><b><br>
Working Conditions:</b></p><ul><li>
Frequent prolonged use of computer screens may produce visual fatigue.</li><li>
Requires potential travel to donor hospitals, donor&rsquo;s family home for support during maternal blood draw process, off-site meetings, and Infinite Legacy functions in all weather conditions.</li><li>
Possible exposure to communicable diseases, hazardous materials, pharmacological agents with little likelihood of harm if established health precautions are followed.</li><li>
This position does not meet criteria to be eligible for Infinite Legacy&rsquo;s flexible scheduling program.</li><li>
Tissue FSCs are considered essential personnel and are expected to be available to work in inclement weather conditions.</li><li>
This position has a workload that is 100% clinical.</li><li>
Manual dexterity sufficient to operate telephones and computers.</li><li>
Ability to accurately communicate, converse, and exchange information in English over the telephone and in person.</li><li>
Computer literacy in a Microsoft Windows environment and demonstrated competency in the use of Microsoft Office software programs.</li><li>

</div></div><div><div><p>The Department of Medicine, Division of General Internal Medicine is seeking a <b><i>Nurse Practitioner (NP)</i></b> for the outpatient faculty practice located at the Levi Watkins Outpatient Center. The NP will work collaboratively to provide comprehensive and age-appropriate care to well and/or ill patients. In addition to care for small patient panel, the NP will provide urgent and ongoing care to patients empaneled by faculty primary care providers. Extended visit times and lower visit volumes than seen in typical primary care practices are the standard in order for the NP to develop meaningful relationships with the patients and other primary care providers. The NP will collaborate and coordinate care with interdisciplinary care teams to ensure the plan of care is in line with and supports the patient&rsquo;s goals of care, physical and social needs. The NP will be responsible for documentation of visits and follow up re-assessment(s) in the electronic medical record, along with recommendations and/or prescriptions.</p><br>

<p><b>Specific Duties &amp; Responsibilities</b></p>
<ul><li>Work schedule includes 40 hours per week, being comprised of six (6) in person or telemedicine clinic sessions and two (2) administrative sessions per week. Administrative time is allotted to allow the provider ample time to coordinate care and interface with other team members.</li>
<li>Develop and maintain a primary care panel focused on diagnosis, management, and treatment of medical ailments as well as wellness and disease prevention.</li>
<li>Assist with providing comprehensive and episodic care to adult patients whose primary care provider is a faculty member in general internal medicine. This includes reviewing the patient chart, taking a history, completing a physical assessment and interpreting diagnostic and radiologic testing along with communicating those findings to the PCP.</li>
<li>Perform preoperative evaluations.</li>
<li>Proactive patient communication, education and coordination of care between visits and schedule patient appointments for telehealth or in-person visits when appropriate.</li>
<li>Provide post discharge coordination to reduce hospital readmission rates and emergency room utilization.</li>
<li>Counsel and teach patients and family members concerning disease process and expected outcomes.</li>
<li>Make referrals to other specialists as indicated by the patients&rsquo; condition.</li>
<li>Document clinic notes and other pertinent data into EMR that is necessary in the care of the patient. Provide information to referring physicians.</li>
<li>Collaborate with clinic leadership to identify in-need patients.</li>
<li>Provide regular feedback to clinic leadership on PCP and NP collaboration, assisting to make workflow changes or provide education on program where appropriate.</li>
