 CircleLink Health is looking for passionate, tech savvy registered nurses to work remotely and serve patients enrolled in Medicare&rsquo;s Chronic Care Management Program. In this
 
 part time role
 
 (20-25 hrs. per week), an RN Care Coach will be assigned a group of patients that they will be following and calling each month. In these monthly calls you will provide education, coordinate care, close preventive care gaps, and coach on strategies for self-management to keep them out of the hospital.

 Key Responsibilities
 
 
 
 <ul>

 Utilize our specialized care management software to call Medicare patients with 2 or more chronic conditions (Diabetes, CHF, Chronic Pain, COPD, etc.) on a monthly basis
 </li>
 <li>
 Build and maintain rapport with patients to help coach them to improved health through SMART goals and education on self-management strategies
 </li>
 <li>
 Implement and improve the Plan of Care by updating medications, appointments due, biometrics, symptoms, and interventions made
 </li>
 <li>
 Connect the patient with community resources as needed, including transportation, personal care needs, prescription/DME assistance, social services, etc.
 </li>
 <li>
 Conduct Transitional Care Management activities to high risk patients discharged from the hospital and the ER to reduce unnecessary readmissions.
 </li>
 <li>
 Close care gaps by encouraging and assisting with preventive care measures, i.e. annual well visits, vaccines, cancer screens, follow-up/specialist appointments, etc.

Primary Duties: 
<ul><li>
Receives and screens telephone callers, provides information per department policy, and/or routes calls to appropriate personnel. Takes and forwards messages as appropriate. Adult Inpatient Transplant Surgery NP

General Summary of Position 
Functions as a member of the patient-centered care team to provide direct patient care activities under the supervision of a Registered Nurse or medical staff/providers. Assists nursing and medical staff with procedures and/or tests and patients with Activities of Daily Living (ADLs). Performs specific care activities such as specimen collection phlebotomy intravenous (IV) catheter insertion electrocardiography (EKG) and documentation in the health record. Incumbents in this position may perform all or a combination of the duties described depending upon the assigned work area and the specific needs of the patients and the department.
Primary Duties and Responsibilities 
<ul><li>
Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality and safety standards and SPIRIT Values. Complies with governmental regulations and accreditation standards.</li><li>
Assists patients with activities of daily living (ADLs) and personal hygiene including bathing mouth care dressing skin care positioning feeding ambulation toileting and range of motion activities. Assists with patient mobilization and rehabilitation use of crutches and walkers and transfer/transport of patients in wheelchairs and stretchers.</li><li>

Maintains the comfort privacy and confidentiality and dignity of each patient in the delivery of services. Performs patient observation duty including 1:1 observation and application of restraints. Reports actual and potential problems concerning patients/families including changes in the patient's condition to the registered nurse and other care team members as necessary.
