The CReW Generalist Case Manager works in close collaboration with members of the CReW clinical treatment teams. This position&rsquo;s focus is on providing a range of supportive case management services to adults with behavioral health conditions, including: housing search and placement, housing stability planning, community resource linkages, skills coaching, care coordination with providers, and landlord-tenant mediation. The position requires strong interpersonal relationship and communication skills, common sense regarding personal safety, creative thinking, multitasking, and conflict resolution skills.</div><div></div><div>

Responsibilities:
<ul><li>As part of the treatment team, identify community resources to meet individuals&rsquo; needs, facilitate individuals&rsquo; access to services, and advocate for them on a continuing basis. Such needs may include accessing housing, financial and health benefits, primary health care, mental health treatment, substance use counseling, chore service, protective payees, transportation, food access, and legal services.</li><li>
Provide frequent and repeated contacts with individuals &ndash; both in person and through telecommunications&mdash;developing rapport and trust in order to assess their service needs and responding with appropriate referrals and linkages to resources.</li><li>
Plan for and provide support to individuals in identifying, locating and moving into housing. This may include doing standardized assessments required for Coordinated Entry systems of care, searching for available housing placements that would fit the individual&rsquo;s needs, completing referrals or applications for units based on their financial status and eligibility.</li><li>
Coordinate care across providers to ensure collaborative approach to intervention.</li><li>
Encourage social skill building through role modeling effective relationship and communication skills, affirmative and supportive communication strategies, and conflict resolution techniques.</li><li>
Represent the CReW program in a variety of settings to build awareness and develop community partners in coordinating new individual referrals.</li><li>
Attend CReW staff meetings and other meetings and clinical trainings as required.

Case Management, a program of SWYFS, will provide systems navigation and trauma intervention activities for young people of color who have experienced racial profiling, racial bias, and either personally experienced or witnessed prejudice and bias in the legal system, maltreatment, and police brutality toward friends, family, and community members of color.
The Case Manager will establish supportive, consistent relationships with young people, enroll them in case management, and create Individualized Service Plans (ISPs) with goals that are informed by the voices and choices of the young people served. ISPs help young people clearly identify their short and long-term goals, the steps they need to take to accomplish those goals, and the barriers they need to address to be successful.
The focus population for this position is people of Black/African American/African Descent.
Essential Functions:
Program Implementation<ul><li>
Develop/implement appropriate goals and objectives using Individual Service Plans</li><li>
Maintain an active caseload of 15-20 participants each, follow-up with them regularly, provide support and resources, and monitor their progress towards ISP goals.</li><li>

<div>Description: 
Case Management, a program of SWYFS, will provide systems navigation and trauma intervention activities for young people of color who have experienced racial profiling, racial bias, and either personally experienced or witnessed prejudice and bias in the legal system, maltreatment, and police brutality toward friends, family, and community members of color.
The Case Management Outreach Specialist is responsible for identifying, engaging, and supporting high-risk youth and young adults who are often disconnected from traditional service systems. This role blends proactive street/community outreach with intensive, person-centered case management. The primary goal for this role is to identify at-risk youth through a comprehensive outreach strategy and enroll them into the Case Management Program.
The focus population for this position is people of Black/African American/African Descent.
Essential Functions:
Program Implementation<ul><li>
Conduct consistent outreach in community &ldquo;hot spots&rdquo;, schools, and courtrooms to identify and build rapport with eligible youth.</li><li>
Utilize a &ldquo;meet-them-where-they-are&quot; approach to build trust with individuals wary of institutional authority.
</li><li>Coordinate and collaborate with network-identified agencies, as necessary, to ensure youth are actively engaged in programs</li><li>
Develop and maintain working relationships with participating programs to ensure successful referrals and communicating with direct service</li><li>
Develop/implement appropriate goals and objectives using Individual Service Plans</li><li>
Maintain an active caseload of 5-10 participants each, follow-up with them regularly, provide support and resources, and monitor their progress towards ISP goals.</li><li>
Meet with participants weekly and conduct formal ISP reviews with participants every 90 days.</li><li>
Removes barriers and empowers participants to take control of their lives through building self-confidence, identifying and achieving personal goals, and connecting participants to available resources.</li><li>
Advocate for young adult and work with community/educational/support service agencies to overcome access barriers</li><li>
Support young people to develop their voice and advocate for themselves in mainstream systems.</li><li>
Provide in-depth systems navigation including education, immigration, legal, public benefits enrollment, housing, utilities, employment systems navigation, and basic and emergency assistance.</li><li>
Coordinate and facilitate support group as needed, including Aggression Replacement Training</li><li>
Collaborate with other network case managers as needed</li><li>
Maintain client confidentiality.</li><li>
Use standardized forms for intake and assessment to enroll young adult</li><li>
Provide age and culturally competent services</li><li>
Attend community meetings, case management meetings, and other meetings as needed</li><li>
Maintain cooperative working relationships with educational, social, legal, and health service agencies</li><li>
Maintain contact with youth to identify additional needs or barriers to service access; to provide support and encouragement; and to follow up to ensure connection with program.</li></ul>
 Documentation and Performance<ul><li>
Maintain compliance with documentation with the agency&rsquo;s client database, ensuring all interactions are logged within 24 hours.</li><li>
Meet regularly with the Case Management team to coordinate services provided.</li><li>
Attend the monthly City of Seattle meeting</li><li>
Attend weekly case consultation and/or team meetings.</li><li>
Perform other duties as assigned by the Case Management Program Manager.</li></ul>
Requirements:

 Minimum Qualifications<ul><li>
Experience working effectively with diverse populations</li><li>
Experience providing referral, advocacy &amp; case management services</li><li>
Valid WA State Driver&rsquo;s license, proof of insurance and reliable transportation</li><li>
Clear driving record with no more than two moving violations in the last three years</li><li>
BA Preferred or equivalent work experience</li><li>
Computer skills required</li><li>
Must be willing to complete a Background Check and Fingerprinting
</li><li>Demonstrated ability to communicate in writing and verbally</li><li>
Demonstrated ability providing community outreach</li><li>
Demonstrated ability to utilize time management techniques to organize and prioritize
</li><li>Ability to be flexible and adapt to programming change</li><li>
Able to work evenings &amp; weekends based on programming schedule</li></ul>
