ESSENTIAL POSITION FUNCTIONS</b></p>

<li>Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act.</li>
<li>Completes history and physical examinations, by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record.</li>
<li>Manages acute and chronic health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications (to the extent permitted by licensure) and or treatments, obtaining consultations or making referrals and coordinating hospice care services.</li>
<li>Reviews clinical information, coordinates and oversees patient care in consultation with the palliative care interdisciplinary team and other healthcare practitioners.</li>
<li>Assures that patient receives appropriate measures to control symptoms through collaboration with interdisciplinary team members.</li>

<p>POSITION SUMMARY</p><p>The Client Services Case Manager provides direct, recovery-oriented support to individuals and families by completing assessments, coordinating services, and helping clients achieve stability, wellness, and self-sufficiency. Licensed Certified Public Accountant preferred</li><li>
Minimum of 3 to 5 years&rsquo; experience in public accounting or healthcare financial management</li><li>
Solid understanding of financial accounting, taxation, and physician practice management</li><li>
Important skills include time management and the ability to organize and prioritize work</li><li>
Must be detail oriented with high degree of accuracy; excellent organizational skills required</li><li>
Strong analytical and problem-solving skills</li><li>
Strong oral and written communication skills including ability to listen and effectively communicate via telephone</li><li>
Proficient in Microsoft Suite of products including Excel, Outlook PowerPoint and Word</li><li>
Ability to prepare monthly reports, make formal presentations and independently conduct monthly client business meetings</li><li>
