<li>Directly supervises Home Health Aides and LPN&rsquo;s. Carries out supervisory responsibilities in accordance 
with the organization's policies and applicable laws. Responsibilities include, but are not limited to, 
scheduling, planning, assigning, and directing work; appraising performance; addressing complaints and 
resolving problems. Performs supervisory visits with Home Health clients and conducts and authorizes inservices.</li>
<li>Conducts client assessments, suggesting level of needed service packages to client and their family.</li>
<li>Develops plans of care with the participation and consent of the client and/or caregiver/family member or legal representative.</li>
<li>Sets up medications for clients and monitors side effects, drug interactions, dosages, and implements changes per physician instructions. Informs client and family of changes immediately. Orders medication for clients from pharmacy.</li>
<li>Listens and documents significant changes in condition of clients, including incidents, unexplained injuries, medication errors, loss of client property, or evidence of client or family dissatisfaction and communicates with supervisor.</li>
<li>Analyzes data gathered from assessment and records reviews, and acts promptly to resolve problems.</li>
<li>Participates in quality assessment and assurance activities including implementation of recommendations and monitoring of results.</li>
<li>Develops plans of care with measurable objectives and timetables and accommodates clients&rsquo; needs and preferences including choices about preferred intensity of medical treatment.</li>
<li>Maintains ongoing communication with other disciplines/departments to evaluate client responses to interventions, suggest alternative approaches when indicated and to revise and update plans of care to reflect progress toward goals as directed by supervisor.</li>
<li>Interacts with clients, caregivers/family members, legal representatives, and resolve concerns.</li>
<li>Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.</li>
<li>Communicates with the physician regarding the patient&rsquo;s needs and reports any changes in the patient&rsquo;s condition; obtains/receives physician&rsquo;s orders as required.</li>
<li>Instructs, supervises and evaluates home health aide care provided every two (2) weeks or per regulatory guidelines.</li>
<li>Demonstrates C.A.R.E. values to our clients, family members, customers and staff.</li>
<li>Complies with and adheres to the appropriate use of Personal Protective Equipment (PPE) required by the Bloodborne Pathogens Standards. Protective Personal Equipment (PPE), including personal protective equipment for eyes, face, extremities, protective clothing, and protective shield and barriers, will be provided, used, and maintained.</li>

Job Requirements:
<ul>
<li>Graduate of an accredited school of nursing with RN licensure in the state in which services will be provided.</li>
<li>One (1) year of home health care experience preferred.</li>
<li>One-year supervisory/management experience with direct care staff preferred.</li>
<li>Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; 
nursing skills per competency checklist.</li>
<li>Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies 
and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing 
to perform required nursing skills.</li>

Symbria is looking for a Remote Pharmacy Admissions EMAR Services Technician to join our growing team in Woodridge, IL!</div><div></div><div> 
POSITION SUMMARY: 
The primary purpose of the Admissions EMAR Services Technician role is to input computer data entry of medications and medical records into client&rsquo;s Electronic Medical Records (EMR) System.</div><div></div><div> 
WORK SCHEDULE:</div><ul><li>
1:30 PM-10:00 PM Mon-Fri with rotating weekends.</li><li>
The weekend rotation is: Saturday 12pm-7pm and Sunday 1pm-5pm, with one day off during the week (1-2 weekends per month).</li></ul>
Responsibilities:
<div>
FUNCTIONS AND RESPONSIBILITIES: 
 
<ul><li>Enter all medication orders and medical records into the client computer system accurately and timely as per facility specific Remote Data Entry training sheets.</li><li>Process all medication and ancillary orders at a minimum productivity standard as follows:</li></ul>
 o Length of employment - zero to three months: No minimum requirement &ndash; focus on data entry skills, order sorting and typing medication only admissions (see facility specific cheat sheet). 
 o Length of employment - four to six months: Minimum of 2 admission processing points per hour &ndash; focus on data entry and medical records entry skills while increasing speed and accuracy. *Admission Processing Points are the point value assigned to each facility according to average time spent to process that facilities&rsquo; orders. 
 o Length of employment - six months and beyond: Minimum of 3 admission processing points per hour &ndash; should be fluent in all aspects of Remote Data Entry and be able to assist in training of new employees. *Admission Processing Points are the point value assigned to each facility according to average time spent to process that facilities&rsquo; orders. 
<ul><li>Contact client for verification of orders as needed and document correspondence and follow up in the system.</li><li>Prioritize order entry of admissions according to patient arrival time into the community.</li><li>Perform reconciliation of skilled nursing community EMR orders against pharmacy order entry as assigned.</li><li>Report pharmacy medication errors or order entry discrepancies to supervisor.</li><li>Work collaboratively with other team members and supervisor to ensure that best-practices are shared.</li><li>May assist with research of lost or missing orders per internal department request.</li><li>Answer incoming calls promptly and provide high standard of customer services to the client.</li><li>Fill out and complete the admission tracking spreadsheet and turn in to supervisor on the 15th and last day of each month, or as assigned.</li><li>May provide backup support to other pharmacy groups based on business needs or production levels.</li><li>Use Microsoft Teams (instant messenger) and comply with standard requirements, including:</li></ul>
 o Communicate to the Data Operations group each time you are stepping away from your desk which includes, but not limited to, bathroom breaks, lunches, and at the beginning and end of each shift. Notify the group as soon as you return to work.</div>
Qualifications:

<ul><li>High School diploma or general education degree (GED).</li><li>Pharmacy Technician License and Certification (CPhT) as per State requirements.</li><li>Medical records and data entry experience; minimum one year.</li><li>General computer knowledge including Microsoft Office: Word, Excel and Outlook.</li><li>Strong knowledge of pharmacy data entry and medical records ancillary orders (diet, lab, therapy, diagnosis, etc.) and brand/generic medications.</li><li>Excellent time and data management skills and ability to prioritize workload.</li><li>Excellent customer service, verbal and written communication skills.</li></ul></div><div></div><div>
