Principal Duties and Responsibilities
Inpatient Medicine (General Medicine Ward Service)
The intern will learn to assume the role of the primary physician by being the primary presenter of the patient, the primary order writer and consult caller, the person to whom all nursing inquiries are referred, and by being the physician who knows the patient best. The intern will learn to have with them patient vital signs, medicine and doses, labs, x-ray and other test results.
Rounds: The intern will be expected to pre-round on their patients prior to rounds at 8:30 am each day, as well as re-evaluate any patient admitted to them more than two hours previously. Work rounds with the residents and attending will begin promptly at 8:30 a.m. Interns will have prepared for work rounds by having reviewed the chart for vitals, notes, nursing medication list, consults and events during the night, and by having examined the patient that morning. They will practice establishing a care plan for the day during work rounds and follow through with the plan during the day. Each intern must be present for Work Rounds daily and be prepared to give a thorough, but succinct oral presentation on each new patient admitted to his/her service. These presentations are not read from the chart. At the time of the presentation, the results of labs, tests and current medications should be known to the intern.
The intern will have two weeks of night float during the academic year. During his/her call, the intern is responsible for the management of admissions assigned to him/her, as well as the care of urgent problems that develop in patients followed by other ward team members. The intern must carefully document any diagnostic or therapeutic interventions undertaken on such patients (i.e., cross-coverage note), and verbally inform the appropriate intern the next day.
The intern must accomplish the initial evaluation of all assigned patients as soon as possible following admission. This evaluation includes:

All categorical Internal Medicine residents will be responsible for the coordination of medical care for an assigned panel of patients in the GBMA Clinical Office (Pavilion-East &ndash;203) every six weeks during their core continuity clinic rotation. The intern will be the designated Primary Care Physician for their patients and are expected to establish an ongoing clinical relationship with those patients.
As the designated Primary Care Physician (PCP), the intern should:<ul><li>
Regularly follow the patient at appropriate intervals for their medical problems.</li><li>
Respond to telephone calls from patients and document those encounters in the patient&rsquo;s file.</li><li>
Check your EPIC in-basket at least twice daily for lab and other test results.</li><li>
Arrange for a colleague to cover your ambulatory patients during vacations, critical care months, or other approved absences.</li><li>
Make appropriate referrals to specialists and follow-up on any such consultations.</li><li>

All categorical interns will be assigned to a Continuity Clinic throughout the year.
The intern&rsquo;s clinic session is supervised by an on-site internal medicine faculty preceptor whose only responsibility during the session is to the direction, supervision, and teaching of the residents in the clinic. Interns present each patient to the faculty preceptor and the resident and faculty jointly discuss the case and develop a management plan. The preceptor may choose to see or examine the patient with the intern to confirm or elicit further history or physical examination information. 
The faculty preceptor completes a note that includes summary comments that revise or confirm the findings of the intern&rsquo;s history/physical examination and medical decision-making. The preceptor is ultimately responsible for the quality of care rendered to all patients.

PERFORMANCE OF SONOGRAPHY 
Primary Responsibilities: 
<ul><li>Prepare exam room and ultrasound equipment to conduct sonography examinations in accordance with infectious disease, sterilization, and patient safety protocols, policies, and procedures.</li><li>Assume responsibility for the safety of the patient during the sonographic examination.</li><li>Review patient medical history and supporting clinical information.</li><li>Ensure that the sonography examination order is complete, contains relevant information, and meets the standard of &ldquo;medical necessity.&rdquo;</li><li>Evaluate any contraindications, insufficient patient preparation, and the patient&rsquo;s inability or unwillingness to tolerate the examination and associated procedures.</li><li>Communicate with the patient in a manner appropriate to the patient&rsquo;s ability to understand.</li><li>Use proper patient positioning tools, devices, equipment adjustment, and ergonomically correct scanning techniques to ensure patient comfort and safety, and to prevent compromised data acquisition or musculoskeletal injury to the sonographer.</li><li>Conduct patient clinical assessment and perform the diagnostic medical sonography examination ordered by the supervising physician in accordance with professional and facility protocols.</li><li>Apply independent, professional, ethical judgment, and critical thinking to safely perform diagnostic sonography procedures.</li><li>Apply independent judgment during the sonography examination to ensure that appropriate anatomical, pathological, and clinical conditions are accurately captured in the examination images.</li><li>As necessary, acquire additional ultrasound imaging to facilitate optimum diagnostic results for the interpreting physician.</li><li>Identify and document any limitations to the sonography examination.</li><li>Review examination images, synthesize examination results, and prepare preliminary findings.</li><li>Except as authorized by written facility policies and procedures, refer specific diagnostic, treatment or prognosis questions to the appropriate physician or healthcare professional.</li><li>Maintain patient privacy and confidentiality in accordance with privacy laws (e.g., HIPAA) and facility policies, and procedures.</li><li>Adhere to accepted professional and ethical standards.</li><li>Provide patient services consistent with the Scope of Practice and Clinical Standards for the Diagnostic Sonographer.</li><li>Provide preliminary reports of sonography examinations for use by the interpreting physician.</li><li>Support the interpreting physician by contributing technical impressions and professional knowledge to facilitate the completion of the final diagnostic report.</li><li>Directs and practices Universal Precautions for prevention of cross-contaminations.</li><li>Consults with referring physicians regarding medical history of the patient, discusses appropriate procedures required, maintains records and/or technical notations on each case as required by regulatory agencies and insures files and records are properly completed with correct information.</li><li>Insures immediate and accurate verification of charge code information into the appropriate computer system and determines the diagnostic efficacy and completeness of each examination.</li><li>Demonstrates and assists in instruction of vascular ultrasound exams and hospital procedures to other technical, non-professional and ancillary personnel.</li><li>Provides preliminary study report to interpreting physician</li><li>Provides ultrasound guidance for radiofrequency thermal ablation and chemical ablations in the Women&rsquo;s Outpatient Surgical Center.</li></ul>
Documentation and Reporting 
<ul><li>Document any incidental findings that might impact patient safety or patient care and provide that information to the interpreting physician.</li><li>Exercise responsibility for the maintenance and accuracy of facility reports, database records and logs.</li></ul></div><div>
Quality Assessment/Quality Improvement 
Primary Responsibilities: 
<ul><li>Ensure that the sonography examination results meet the facility standard for Quality Assurance before findings are released for interpretation.</li><li>Provide oversight for the assessment of sonography examinations performed within the vascular lab for technical and image quality, and accuracy.</li><li>Work with the Medical Director to ensure quality patient care.</li><li>Function as the clinical, technical leader/expert in sonography imaging within the vascular lab.</li><li>Accountable for the creation, oversight, and implementation of scanning protocols and quality control activities.</li><li>Provide technical, image quality, and accuracy oversight for the assessment of sonography examinations.</li><li>Ensure that appropriate accreditation is secured and maintained.</li><li>Carryout quality assurance protocols and quality improvement measures.</li></ul> 

Other Responsibilities: 
<ul><li>Assist in carrying out Quality Assurance protocols and Quality Improvement measures within the facility.</li><li>Set up quarterly vascular lab staff meetings to review QI for vascular lab and Vein Center accreditation by the Intersocietal Accreditation Commission</li><li>Assist with the pursuit and maintenance of cutting edge technical and clinical updates</li></ul></div><div>
Equipment Use &amp; Maintenance 
Primary Responsibilities: 
<ul><li>Perform quality control procedures to ensure that the ultrasound equipment operates at optimal levels.</li><li>Exercise responsibility to ensure that the ultrasound equipment is functioning accurately.</li><li>Provide education and training to sonographers in the utilization of new equipment features/benefits or scanning techniques.</li><li>Responsible for the preventive maintenance program for ultrasound equipment.</li><li>Ensure that appropriate ultrasound equipment is secured and provide opportunities for staff input during the equipment selection and purchasing process.</li><li>Provide education and training to sonographers in the utilization of new equipment features/benefits and scanning techniques.</li></ul></div><div>
Supervision and Management 
Primary Responsibilities: 
<ul><li>Responsible for day-to-day operation of sonography services within the facility.</li><li>Provide general supervision and evaluation of the sonographer staff.</li><li>Ensure staff compliance with clinical and operational protocols, policies, and procedures.</li><li>Ensure that sonographers have timely access to equipment, supplies and logistical support.</li><li>Exercise responsibility for the direct supervision of staff sonographers.</li><li>Provide input for the performance assessment of staff sonographers.</li><li>Exercise responsibility for sonography team building, morale, and performance.</li><li>Provide leadership and direction in developing new approaches for patient care services</li><li>Assist with the direction and guidance for departmental workflow.</li><li>Assist with workload scheduling.</li></ul>
